    INTRODUCTION: Keloids are a fibroproliferative, multifactorial, cutaneous disorder whose pathophysiology is not completely understood. Various factors such as high blood pressure, pregnancy, female gender, mechanical tension of local sites, and prolonged wound healing are known to worsen keloids. Childhood-onset keloids are keloids that form before 10 years of age, before various factors in adulthood come into play, and thus studying childhood-onset keloids may provide additional insight into the underlying mechanisms that lead to keloid formation. METHODS: Retrospective chart review was performed on all patients with childhood-onset keloids who were evaluated at our plastic surgery clinic (one of the largest keloid referral centers in Japan) over a 1-year period. RESULTS: Of the 1443 patients with diagnosis of keloids, 131 patients had childhood-onset keloids. Of these, 106 patients (80.9%) were female, 38.9% of patients had family history of keloids, and 48.9% of patients had allergies or allergy-related conditions (asthma, atopic dermatitis, or allergic rhinitis). Vaccination (47.5%) and chickenpox (19.9%) were the most common triggers. Of vaccinations, BCG was the most common trigger. The majority of keloids from BCG were in female patients (92.9%). The most common location was the chest in male patients (30.0%) and the arm in female patients (41.1%). CONCLUSION: To our knowledge, this is the largest report in the literature on childhood-onset keloids. There was overall female predominance in childhood-onset keloids, and even more significant female predominance in BCG-induced keloids.

    外科系医師を中心に継続的な就労の要因としてのワーク・ライフバランスを理解し,必要な支援を考えることが本研究の目的である。大学の同窓会や地域の医師会に呼びかけ,現役の医師に対してワーク・ライフバランスとともに,職業環境,家事負担,生活や仕事への満足度,健康状態等に関する調査を行った。また,医学教育による意識の変化を測るため,医学生に対し2年時と卒業前の6年時におけるワーク・ライフバランスの価値について質問した。女性医師は男性医師に比し家事育児による負担感からワーク・ライフコンフリクトの生じる可能性が示された。特に外科系医師は,そうでない医師に比べて時間的な問題で仕事由来のコンフリクトが大きいことが示唆された。医学生のワーク・ライフバランスの重要性に関する意見は,女子医学生のほうが重要と考えるものが多かったが,卒前の6年時の調査結果で男女間の差は小さくなった。家事育児の性別役割分担への考えは,生育家庭から影響を受けることが示唆され,労働時間など職場環境の改善のほか,大学の医学教育の充実も,医師などの専門職の就労継続に効果のある可能性が示唆された。(著者抄録)

    INTRODUCTION: Keloids are a cutaneous fibroproliferative disorder. Despite the fact that keloids are relatively common lesions, the statistics of patient with keloids especially sex difference remain unknown. To better understand it, we conducted an extensive cross-sectional analysis of a large cohort of patients with keloids (n = 1659). The study showed for the first time that female sex may be an inherent keloid risk factor. METHODS: This cross-sectional study of 1659 consecutive patients with keloids who attended a plastic surgery outpatient clinic in Japan in 2014 analyzed age at keloid onset, age at the first medical examination for keloid, and the influence of sex on these variables. RESULTS: In both male and female patients, the keloids were most likely to start in puberty and there was no significant difference in the mode value for age of onset (16 vs. 20 years). Though female patients were twice as prevalent as male patients at nearly all onset ages, female patients predominated over male patients with a gender ratio of 2.7:1 in cases of onset before the age of 15 years. Moreover male and female patients did not differ in terms of the mean ± SD duration between keloid onset and the first medical examination. This finding shows that female patients do not get their keloids examined earlier than male patients. These observations together suggest that female sex may promote early keloid development due to physiological, not social, reasons. CONCLUSION: This is the first report to suggest that female sex may drive keloidogenesis because of physiological reasons. Individuals were most likely to seek a medical examination almost 10 years after onset, regardless of sex. These findings provide new insight into the importance of sex in the development and progression of keloids. Future studies should address the influence of sex hormones on the keloid.

    It is predicted that in the near future robot simulated patients (SPs) will come into use. Through the impressions of five SPs about robot SPs, we explored their vision of the future of medical education as an indicator of what ordinary citizens think. The opinions of SPs were collected using a semi-structured focus group interview, after which the perspectives provided were explored using a qualitative research method called Steps for Coding and Theorization (SCAT). Although SPs accepted the introduction of robot SPs, they regard them as potential substitutes. The use of robot SPs raised concerns about the level of response to human diversity and the level of emotional intelligence. The problem of how much diversity among patients and doctors is acceptable in the field of education was identified. On the part of citizens, there is not much expectation that artificial intelligence (AI) will lead to sophisticated machines capable of human conversation. However, looking ahead to the AI era, real SPs anticipate that, along with the evolution of AI, the next generation of SPs will have thought deeply about their role within a program employing both humans and robots.

    International accreditation of medical education was introduced in Japan in 2013 and is planning to be applied in late 2014 or 2015. Students will need to cope with the resulting changes and to recognize by what route they will learn medicine. Therefore, a freshman orientation course, which was based on problem-based learning (PBL) and had been held for first-year students, was modified as an awareness reform program in which students would learn "how to learn medicine." We investigated whether this program has led to useful changes in students' recognition of the way of learning in medical school and their directions as learners. The program was held for 114 first-year medical school students in 2013 and consisted of PBL tutorials, large-classroom lectures, simulation learning using role-play with simulated patients, and team-based learning (TBL), presented in this order. Learning modules that is made with an integration of the clinical sciences with the basic biomedical and the behavioral and social sciences were provided. A nonanonymous questionnaire survey asking "what learning methods are effective for you?" was conducted before and after completion of the course. Furthermore, group answers obtained in TBL were investigated. The score for the question "To what extent can you imagine your route of learning during your 6 years?" significantly increased from 3.1 +/- 0.99 (mean +/- SD) before the course to 3.5 +/- 0.88 (p<0.01) after the course. The score for the question "To what extent is the small-group learning, such as PBL, useful for you?" significantly increased from 3.9 +/- 0.73 to 4.2 +/- 0.71 (p<0.05). Group responses in TBL sessions indicated that students desired classes that presented tasks and regarded "emphasis on reflection" and "observation of senior physicians as role models" as the most important methods for learning interview skills. We believe students should acquire active learning attitudes as adults early in their 6 years of medical school. The level of understanding of "how to learn as adults" was 3.7 and indicated a moderate result. This course employed many educational strategies, and we believe it helped students understand what they learn and how to learn during their 6 years of medical and to get an overview of the learning roadmap.

    As a fundamental study for the development of adaptive neurological limbs prosthesis using a computer controlled human machine interactive system, a neuro-physiological experiment was carried out using rats. The rats were surgically implanted with electrodes in the motor cortex, spinal cord and peripheral nerves. Neuro-electrical signals were recorded in voluntary and involuntary movements of the legs. Obtained data under two sets of conditions were compared and analyzed. Using our experimental model, neurophysiological characteristics of motor generation were revealed. Based upon these results, conceptual design of a new computer controlled prosthesis for disabled was discussed. © 2012 IADIS.

    The purpose of this study is to investigate the characteristics of Visual Evoked brain Potentials (VEP) for application to a new Brain - Computer Interface (BCI) for disabled people with verbal communication disorders such as Amyotrophic Lateral Sclerosis (ALS). In this study, an experiment using VEP recordings was carried out in healthy adult subjects. As a stimuli for the VEP, squared figures were displayed on the LCD of a personal computer. Two experimental conditions that were presented in (1) central visual field and (2) peripheral visual field were used. As a result, two different VEP waveforms were obtained between two stimulus conditions. From a comparison of the VEP waveforms, subjective stimulus position in his/her visual field could be discriminated objectively. Utilizing this phenomenon, VEP based BCI for binary decisions can be constructed. Concerns about the feasibility of this method as a BCI were also discussed. © 2012 IADIS.

    In order to estimate the mental workload, electroencephalogram (EEG) was measured during the Figure Stroop Task performance from ten healthy adult voluntary participants. In the experiment, two kind of visual stimuli were prepared: congruent and incongruent stimuli (figure and number of the figure was congruent or incongruent). Participants were asked to answer the number of displayed figures by pressing designated key. In the task, two different instructions to the participants were used: (1) respond as soon as possible, and (2) respond accurately. Integrated theta component of the EEG were compared between under condition of (1) and that of (2). Relative power percentage of theta component in condition (2) was larger than that in (1). The result suggests that the theta component was related with subjective difficulty of the task.

    The authors have been studying psychophysiological workload of human interface (HI) with physiological measurements and analysis. In this study, we investigated a kind of mental workload produced by user's unexpected waiting period from the request input to the termination of data processing during personal computer (PC) operation. As the experimental setting of HI, we used interactive software containing easy questions with unexpected time interval between each question. The effects of progress indicator (PI) indicating during waiting period on psychophysiological status of users were analyzed by using respiration, finger plethysmogram (PTG), heart rate (HR) and electroencephalogram (EEG) measurements. Results showed that the theta wave component of the EEG increased in the non-PI condition, even though autonomic nervous system parameters showed no significant change. Negative correlation between preference score for HI and integrated theta component percentage was observed only in non-PI condition. It is supposed that the PI was controlling theta activity coused by waiting stress in experimental condition. Utilizing physiological indices for HI assessment, this experimental method could be available to waiting stress estimation.

    We develop a new digital plethysmograph using a sensor network. We can utilize autonomic indices for instance heart rate and heart rate variability from the plethysmogram recording. It makes possible to evaluate user's physiological state easily and on real time.

    In order to obtain a guideline for designing low-workload Human Interface (HI). We investigated assessment of visual fatigue induced by performing an interactive task as a model of HI software on personal computers utilizing measurement of Event Related Potentials (ERP) and accommodation. The ERP were measured in this study before and after a visual target detection task in healthy adult participants to assess visual fatigue of the central nervous system. As an index of accommodation, the ophthalmic near point was also measured using an accomodometer. As the experimental task, participants were instructed to click designated targets from a randomly arranged matrix of characters for one hour. All participants reported symptoms of visual fatigue after the task. Prom the averaged ERP waveform, P100 and P300 components were detected and their amplitudes and peak latencies were analyzed. Amplitude of the P100 component measured after performing the task was larger than that in the control condition. Near points after the task increased in comparison with those, before the task. Results indicated that visual fatigue by the experimental task evoked both eye-strain and altered function of the primary visual cortex. Feasibility of applying the present method to a low-workload software development is discussed.

    In order to analyze the autonomic functioning during sleep, polysomnographic measurements including electrogastrogram (EGG) were carried out with healthy adult subjects. The periodic change of the EGG activities calculated from integrated power spectra was not identical to the change of sleep stage. It is suggested that the EGG reflects different aspect of the autonomic profile of human sleep.

    Body sway variations were recorded in elementary school children, junior high school and university students. As an index of randomness, averaged waveforms of squared data of fine fluctuating segments in each school year subjects were calculated. Gradients of time dependent increase segments in the averaged curves of X-axis components were analyzed. Results showed that the index decreased in advance of their age. The role of the randomness fluctuation is discussed.

    In order to estimate the required time of the occurrence of Binocular Rivalry (BR), psychological experiments were carried out using a computer controlled CRT screen. Two square figures filled with stripes and displayed on the right and left sides of the screen were used as the stimulus for the experiment. Eight healthy adult subjects were instructed to press the designated key for each perceived state of right side stimulus, left side stimulus or a combination of them. As a result, the stimulus duration required for the occurrence of the BR was estimated to be at least 400ms.

    To investigate the cognitive aspect of binocular rivalry (BR), an experimental study was carried out using a computer controlled CRT screen. Two square figures filled with different stripes were displayed on the right side and left side of the screen as the BR inducing stimulus. Subjects were instructed to press designated keys for three kinds of received states the right side figure, the left side figure and a combination of them. During the experiment, a continuous line or a non-continuous line as the cue stimuli was displayed on a perceived or a non-perceived visual field. Results showed that the cue stimulus displayed on a non-perceived visual field induced a change in the dominance of BR. Information processing mechanisms for non-perceived visual stimulus are discussed.

    To investigate the relationship between sleep rating scales (OSA, KSS) and measured all night sleep EEG, experiments were carried out with healthy adult volunteers. Results showed that the KSS ratings correlated with time percentage of non-REM sleep state. The OSA ratings correlated with waking time in the experimental night. In evaluation of the quality of sleep, some attention should be paid to subjective ratings of the sleep.

    In order to analyze mechanisms of involuntary movement, readiness potentials of saliva swallowing were recorded in control and light sleep condition from healthy subjects. As a trigger signal for data averaging, surface EMG recorded from neck was used. Negative component of readiness potential before onset of swallowing was not observed clearly in sleep and control conditions. Sensory evoked potentials after swallowing were observed in both conditions. Result shows that it is possible to measure readiness potentials even in sleeping subjects.

    In order to assess psychophysiological state of the hyper arousal, pulse transit time during a mental task was analyzed using the ECG R-wave finger plethysmogram onset asynchrony (RPA) measurements. As the mental task, we used the Wisconsin card sorting test (WCST). As the experimental condition, a balloon burst setting with time limit and narration comprehension test were used. Under balloon burst setting experimental condition, measured RPAs were shorter than these in control condition. The result suggested that the RPA is a possible tool for the assessment of panic state through the autonomic responses.

    In order to reveal cognitive processing of sensory information in binocular rivalry, visual evoked potentials (VEP) were recorded using pattern reversal stimulus to received side and the other side of visual field. VEP components were observed by evoking stimulus to non-received side of visual field. Results show that the processed information projected to visual cortex does not always generate subjective visual experience.

    Three-dimensional perception is based on binocular vision. When different images are presented to both eye, binocular rivalry occurs. The purpose of this study is to investigate the interaction of the 3D-stereovision and the visual conflict caused by binocular rivalry. Ocular dominance was recorded using a personal computer in 3D stereogram with rivalry inducing figures. Our results showed,that under the condition of 3D stereogram with rivalry, the mean rate of fusion time was larger than that in the control condition. The mechanisms of binocular interaction for visual information are discussed

    We are apt to be disturbed and make mistakes under psychological and social stresses. This is characterized as the hyper-arousal state. To investigate the physiological aspects of the hyper-arousal state, an EEG and plethysmogram (PTG) were obtained and the pulse transit time was estimated using the R-wave of the electrocardiogram as the starting point of pulsation. Normal volunteers were instructed to shoot a model-gun toward their own heads. The pulse transit time after the shooting was significantly decreased (t<0.002) compared to pre-shooting period. Spectral analysis of the EEG also showed a relevant change in the shooting. Based on these biological indices, one will be able to access the psychophysiological states under certain stresses.

    The purpose of this study is to analyse the cognitive mechanisms of visual depth perception using a psychological experiment. As a model of depth perception, a 3-D random dot stereogram was used. The experimental task was to discriminate stereographically displayed stimuli which three different depths of visual cliff quickly changed on the tachistoscope. To estimate stereographic perception time, time reqired to respond correctly was measured. Results suggested that the stereographic perception was completed at least for 100 ms in healthy subject.
