  • Evaluation of Perioperative Cardiovascular Event Risk in Gastrointestinal Surgery - Predictive Modeling and Risk Stratification Using Machine Learning.

    BACKGROUND: Preoperative risk assessment is very important to ensure surgical safety and predict postoperative complications. However, no large-scale studies have evaluated the risk of perioperative cardiovascular events in Japan. This study evaluated perioperative cardiovascular events using real-world data. In addition, the applicability of machine learning to risk stratification was examined to develop a predictive model for perioperative cardiovascular events. METHODS AND RESULTS: This was an observational cohort study using the Japan Medical Data Center database, which includes claim and health examination data in Japan, between January 2005 and April 2021. In all, 133,634 gastrointestinal surgeries were included in the analysis. The primary outcome was 30-day risk of major adverse cardiovascular events (MACE). The 30-day MACE incidence rate following surgery was 3.8%. Machine learning was used to perform a binary classification task to predict MACE occurrence within 30 days after surgery. A clustering algorithm was developed based on the Shapley additive explanation values obtained from training data, and generalizability was evaluated using test data. Of the variables, age, history of ischemic heart disease or heart failure, history of stroke, diabetes, hypertension, atrial fibrillation, cases of malignancy, and pancreatic biliary surgery were identified as factors associated with MACE occurrence. CONCLUSIONS: A machine learning model built from basic clinical information, comorbidities, and surgical information demonstrated the capacity to stratify MACE risk in patients undergoing gastrointestinal surgery.

    BACKGROUND: The Revised Cardiac Risk Index (RCRI) has been incorporated into preoperative assessment guidelines and is used for simple preoperative screening; however, validation studies within large populations are limited. Moreover, although sex differences in perioperative risk are recognized, their effect on the performance of the RCRI remains unclear. Therefore, in this study we evaluated whether sex differences exist in the risks within the strata classified by the RCRI. METHODS AND RESULTS: The Japan Medical Data Center database based on claim and health examination data in Japan between January 2005 and April 2021 was used. A total of 161,359 noncardiac surgeries performed during hospitalization were analyzed. The main outcome was the 30-day risk of major adverse cardiovascular events. Although there was no significant sex difference among those with an RCRI ≥1, males had a significant hazard rate (1.32 [95% confidence interval, 1.03-1.68]) of postoperative events in the low-risk group with an RCRI of 0. However, this significant difference was not detected in the population excluding those who underwent breast and gynecological surgeries. CONCLUSIONS: The RCRI achieved reasonable risk stratification in validation using Japanese real-world data regardless of sex. Although further detailed analysis is necessary to determine the sex differences, the validity of using the RCRI for screening purposes is supported at this stage.

    Large Language Models (LLM) are increasingly multimodal, and Zero-Shot Visual Question Answering (VQA) shows promise for image interpretation. If zero-shot VQA can be applied to a 12-lead electrocardiogram (ECG), a prevalent diagnostic tool in the medical field, the potential benefits to the field would be substantial. This study evaluated the diagnostic performance of zero-shot VQA with multimodal LLMs on 12-lead ECG images. The results revealed that multimodal LLM tended to make more errors in extracting and verbalizing image features than in describing preconditions and making logical inferences. Even when the answers were correct, erroneous descriptions of image features were common. These findings suggest a need for improved control over image hallucination and indicate that performance evaluation using the percentage of correct answers to multiple-choice questions may not be sufficient for performance assessment in VQA tasks.

    BACKGROUND: It is unclear whether an intra-aortic balloon pump (IABP) or percutaneous ventricular assist device (Impella) in combination with extracorporeal membrane oxygenation (ECMO) is better. METHODS: Using the Japanese Diagnosis Procedure Combination database from September 2016 to March 2022, we identified inpatients who received an Impella or IABP in combination with ECMO (ECPella or ECMO+IABP group, respectively). The primary outcome was in-hospital mortality, and the secondary outcomes included the length of hospital stay, length of ECMO, total hospitalization cost, complications, and durable mechanical circulatory support implantations. Propensity score matching was performed to compare the outcomes between the groups. RESULTS: Of 14 319 eligible patients, 590 (4.1%) received ECPella and 13 729 (96%) received ECMO+IABP. The mean age of patients was 65 years, 77% were men, and 57% had acute coronary syndrome. After propensity score matching, the patient characteristics were well balanced between the groups. The 14-day mortality rate was lower in the ECPella group than in the ECMO+IABP group (28.0% versus 36.8%; risk difference, -8.2% [95% CI, -13.8 to -2.7]), whereas there was no significant difference in in-hospital mortality between the groups (58.3% versus 56.6%; risk difference, 2.4% [95% CI, -3.5 to 8.2]). The ECPella group had a higher total hospitalization cost, increased renal replacement therapy during hospitalization, and more durable mechanical circulatory support implantations than the ECMO+IABP group. CONCLUSIONS: This nationwide inpatient database study showed no significant difference in in-hospital mortality between the groups, but ECPella was associated with a higher total hospitalization cost, increased renal replacement therapy during hospitalization, and more durable mechanical circulatory support implantations than ECMO+IABP.

    •Hypofibrinogenemia frequently occurs in extracorporeal cardiopulmonary resuscitation (ECPR) and may heighten bleeding risk. This subanalysis of a multicenter observational study (SAVE-J II) included 2,100 adult patients receiving ECPR after out-of-hospital cardiac arrest at 36 facilities in Japan.•Overall, 7.5% of patients experienced non-cannulation hemorrhagic complications, and those with fibrinogen levels below 140 mg/dL at ECPR initiation faced significantly higher bleeding risk. These findings underscore the importance of monitoring and managing fibrinogen levels in ECPR to mitigate hemorrhagic events and improve patient outcomes.

    Assessing the risk of postoperative cardiovascular events before performing non-cardiac surgery is clinically important. The current risk score systems for preoperative evaluation may not adequately represent a small subset of high-risk populations. Accordingly, this study aimed at applying iterative random forest to analyze combinations of factors that could potentially be clinically valuable in identifying these high-risk populations. To this end, we used the Japan Medical Data Center database, which includes claims data from Japan between January 2005 and April 2021, and employed iterative random forests to extract factor combinations that influence outcomes. The analysis demonstrated that a combination of a prior history of stroke and extremely low LDL-C levels was associated with a high non-cardiac postoperative risk. The incidence of major adverse cardiovascular events in the population characterized by the incidence of previous stroke and extremely low LDL-C levels was 15.43 events per 100 person-30 days [95% confidence interval, 6.66-30.41] in the test data. At this stage, the results only show correlation rather than causation; however, these findings may offer valuable insights for preoperative risk assessment in non-cardiac surgery.

    Abstract

    Background

    The micro-axial flow pump Impella, a new mechanical circulatory device for cardiogenic shock, is still only available in a limited number of hospitals, due to the facility certification requirements and insufficient evidence of the benefit of introducing Impella in hospitals. This study aimed to evaluate the impact of introducing Impella in hospitals on in-hospital mortality of patients treated with extracorporeal membrane oxygenation (ECMO).

    Methods

    Using a nationwide Japanese inpatient database, we identified patients who received ECMO during hospitalization between 1 April 2014 and 31 March 2021. A hospital-level propensity score–matched cohort was created matching hospitals that introduced Impella (exposure group) to those that did not introduce Impella (control group). The inclusion period in each hospital was divided into two time periods according to the time of Impella introduction in the exposure group and the corresponding hospital in the control group (before and after exposure). The primary outcome was in-hospital mortality. Uncontrolled and controlled interrupted time-series analyses involved before–after exposure comparison and exposure–control comparison.

    Results

    Out of 34,379 eligible patients, we created a matched cohort of 8351 patients from 86 hospitals with Impella introduction (exposure group) and 7230 patients from 86 hospitals without Impella introduction (control group). In-hospital mortality before and after exposure was 62.5% and 59.3, respectively, in the exposure group; and 66.8% and 63.7%, respectively, in the control group. Uncontrolled interrupted time-series analysis showed no significant level change or trend change in the before–after exposure comparison in both the exposure and the control groups. Controlled interrupted time-series analysis also showed no significant level change (−0.01%; 95% confidence intervals −5.36% to + 5.33%) or trend change (+ 0.10%, −0.30% to + 0.40%) after exposure in the exposure–control comparison.

    Conclusions

    This nationwide inpatient database study showed no association between Impella introduction in hospitals and in-hospital mortality of patients who underwent ECMO. Because this study confined itself to analze of the impact of the introduction of Impella solely at the hospital level, further detailed studies are warranted to assess its efficacy at the patient level.

    Abstract

    Background

    In some cases of patients with out-of-hospital cardiac arrest (OHCA) who underwent extracorporeal cardiopulmonary resuscitation (ECPR), negative pupillary light reflex (PLR) and mydriasis upon hospital arrival serve as common early indicator of poor prognosis. However, in certain patients with poor prognoses inferred by pupil findings upon hospital arrival, pupillary findings improve before and after the establishment of ECPR. The association between these changes in pupillary findings and prognosis remains unclear. This study aimed to clarify the association of pupillary examinations before and after the establishment of ECPR in patients with OHCA showing poor pupillary findings upon hospital arrival with their outcomes. To this end, we analysed retrospective multicentre registry data involving 36 institutions in Japan, including all adult patients with OHCA who underwent ECPR between January 2013 and December 2018. We selected patients with poor prognosis inferred by pupillary examinations, negative pupillary light reflex (PLR) and pupil mydriasis, upon hospital arrival. The primary outcome was favourable neurological outcome, defined as Cerebral Performance Category 1 or 2 at hospital discharge. Multivariable logistic regression analysis was performed to evaluate the association between favourable neurological outcome and pupillary examination after establishing ECPR.

    Results

    Out of the 2,157 patients enrolled in the SAVE-J II study, 723 were analysed. Among the patients analysed, 74 (10.2%) demonstrated favourable neurological outcome at hospital discharge. Multivariable analysis revealed that a positive PLR at ICU admission (odds ration [OR] = 11.3, 95% confidence intervals [CI] = 5.17–24.7) was significantly associated with favourable neurological outcome. However, normal pupil diameter at ICU admission (OR = 1.10, 95%CI = 0.52–2.32) was not significantly associated with favourable neurological outcome.

    Conclusion

    Among the patients with OHCA who underwent ECPR and showed poor pupillary examination findings upon hospital arrival, 10.2% had favourable neurological outcome at hospital discharge. A positive PLR after the establishment of ECPR was significantly associated with favourable neurological outcome.

    Abstract

    We aimed to establish a new method of obtaining femur anteroposterior radiographs from live rats. We used five adult male Sprague–Dawley rats and created a femoral fracture model with an 8 mm segmental fragment. After the surgery, we obtained two femoral anteroposterior radiographs, a novel overhead method, and a traditional craniocaudal view. We obtained the overhead method three times, craniocaudal view once, and anteroposterior radiograph of the isolated femoral bone after euthanasia. We compared the overhead method and craniocaudal view with an isolated femoral anteroposterior view. We used a two-sample t-test and intraclass correlation coefficient (ICC) to estimate the intra-observer reliability. The overhead method had significantly smaller differences than the craniocaudal view for nail length (1.53 ± 1.26 vs. 11.4 ± 3.45, p < 0.001, ICC 0.96) and neck shaft angle (5.82 ± 3.8 vs. 37.8 ± 5.7, p < 0.001, ICC 0.96). No significant differences existed for intertrochanteric length/femoral head diameter (0.23 ± 0.13 vs. 0.23 ± 0.13, p = 0.96, ICC 0.98) or lateral condyle/medial condyle width (0.15 ± 0.16 vs. 0.13 ± 0.08, p = 0.82, ICC 0.99). A fragment displacement was within 0.11 mm (2.4%). The overhead method was closer to the isolated femoral anteroposterior view and had higher reliability.

    BACKGROUND: Although several clinical guidelines recommend vasodilator therapy for non-occlusive mesenteric ischemia (NOMI) and immediate surgery when bowel necrosis is suspected, these recommendations are based on limited evidence. METHODS: In this retrospective nationwide observational study, we used information from the Japanese Diagnosis Procedure Combination inpatient database from July 2010 to March 2018 to identify patients with NOMI who underwent abdominal surgeries on the day of admission. We compared patients who received postoperative vasodilator therapy (vasodilator group) with those who did not (control group). Vasodilator therapy was defined as venous and/or arterial administration of papaverine and/or prostaglandin E1 within 2 days of admission. The primary outcome was in-hospital mortality. Secondary outcomes included the prevalence of additional abdominal surgery performed ≥3 days after admission and short bowel syndrome. RESULTS: We identified 928 eligible patients (149 in the vasodilator group and 779 in the control group). One-to-four propensity score matching yielded 149 and 596 patients for the vasodilator and control groups, respectively. There was no significant difference in in-hospital mortality between the groups (control vs. vasodilator, 27.5% vs. 30.9%; risk difference, 3.4%; 95% confidence interval, -4.9 to 11.6; p=0.42) and no significant difference in the prevalences of abdominal surgery, bowel resection ≥3 days after admission, and short bowel syndrome. CONCLUSIONS: Postoperative vasodilator use was not significantly associated with a reduction in in-hospital mortality or additional abdominal surgery performed ≥3 days after admission in surgically treated NOMI patients.

    BACKGROUND: Neuromuscular blocking agents are used to control shivering in cardiac arrest patients treated with target temperature management. However, their effect on outcomes in patients treated with extracorporeal cardiopulmonary resuscitation is unclear. METHODS: This study was a secondary analysis of the SAVE-J II study, a retrospective multicenter study of 2175 out-of-hospital cardiac arrest patients treated with extracorporeal cardiopulmonary resuscitation in Japan. We classified patients into those who received neuromuscular blocking agents and those who did not and compared in-hospital mortality and incidence rates of favorable neurological outcome and in-hospital pneumonia between the groups using multivariable regression models and stabilized inverse probability weighting with propensity scores. RESULTS: Six hundred sixty patients from the SAVE-J II registry were analyzed. Neuromuscular blocking agents were used in 451 patients (68.3%). After adjusting for potential confounders, neuromuscular blocking agents use was not significantly associated with in-hospital mortality (aHR 0.88; 95% CI, 0.67-1.14), favorable neurological outcome (aOR 0.85; 95% CI, 0.60-1.11), or pneumonia (aOR 1.52; 95% CI, 0.85-2.71). The results for in-hospital mortality (aHR 0.89; 95% CI, 0.64-1.25), favorable neurological outcome (aOR 0.94; 95% CI, 0.59-1.48) and pneumonia (aOR 1.59; 95% CI, 0.74-3.41) were similar after weighting was performed. CONCLUSIONS: Although data on the rationale for using neuromuscular blocking agents were unavailable, their use was not significantly associated with outcomes in out-of-hospital cardiac arrest patients treated with extracorporeal cardiopulmonary resuscitation and targeted temperature management. Neuromuscular blocking agents should be used based on individual clinical indications.

    Injuries of the celiac artery and its branches are rare, but potentially lethal. Ligation of these arteries is performed to control significant hemorrhage. However, few reports have described the adverse effects of ligating these arteries. A 69-year-old woman with a self-inflicted stab wound was brought to our hospital. Her blood pressure could not be measured, therefore aortic cross-clamping was performed, and epinephrine was administered for resuscitation, an emergency laparotomy was performed, and the roots of splenic artery and common hepatic artery were ligated. The left gastric artery which was anomalous and arose directly from the aorta, was also injured and had to be ligated. Norepinephrine was required after the surgery. Enhanced computed tomography performed on hospital day 4 revealed a disrupted celiac artery. The patient developed gastric necrosis on hospital day 23 and, hence, underwent total gastrectomy was performed. The possibility of delayed stomach necrosis should be considered during the postoperative management of patients who undergo ligation of all of the celiac artery branches and experience global hypoperfusion after the surgery.

    BACKGROUND: The prevalence of extracorporeal cardiopulmonary resuscitation (ECPR) in patients with out-of-hospital cardiac arrest (OHCA) has been increasing rapidly worldwide. However, guidelines or clinical studies do not provide sufficient data on ECPR practice. The aim of this study was to provide real-world data on ECPR for patients with OHCA, including details of complications. METHODS: We did a retrospective database analysis of observational multicenter cohort study in Japan. Adult patients with OHCA of presumed cardiac etiology who received ECPR between 2013 and 2018 were included. The primary outcome was favorable neurological outcome at hospital discharge, defined as a cerebral performance category of 1 or 2. RESULTS: A total of 1644 patients with OHCA were included in this study. The patient age was 18-93 years (median: 60 years). Shockable rhythm in the initial cardiac rhythm at the scene was 69.4%. The median estimated low flow time was 55 min (interquartile range: 45-66 min). Favorable neurological outcome at hospital discharge was observed in 14.1% of patients, and the rate of survival to hospital discharge was 27.2%. The proportions of favorable neurological outcome at hospital discharge in terms of shockable rhythm, pulseless electrical activity, and asystole were 16.7%, 9.2%, and 3.9%, respectively. Complications were observed during ECPR in 32.7% of patients, and the most common complication was bleeding, with the rates of cannulation site bleeding and other types of hemorrhage at 16.4% and 8.5%, respectively. CONCLUSIONS: In this large cohort, data on the ECPR of 1644 patients with OHCA show that the proportion of favorable neurological outcomes at hospital discharge was 14.1%, survival rate at hospital discharge was 27.2%, and complications were observed during ECPR in 32.7%.

    Vascular Ehlers-Danlos syndrome is a rare connective tissue disease with a high risk of severe complications. Because of these complications, the median life expectancy for patients with vascular Ehlers-Danlos syndrome is estimated at 48 years. However, the optimal management of these complications remains unclear. A 25-year-old man with abdominal pain was transported to our hospital by ambulance. He had undergone Hartmann's operation at 22 years of age for a first-time colonic perforation. At that time, a genetic test revealed germline variants in COL3A1, which encodes type III procollagen; therefore, the patient was diagnosed with vascular Ehlers-Danlos syndrome. When the patient presented to our hospital, we suspected another colonic perforation and thus performed an emergency operation. Open abdominal management, transcatheter arterial embolization, and negative-pressure wound therapy were performed as life-saving measures. Notably, these procedures should initially be avoided in patients with vascular Ehlers-Danlos syndrome because of tissue fragility. Open abdominal management, transcatheter arterial embolization, and negative-pressure wound therapy may be useful for patients with vascular Ehlers-Danlos syndrome who develop panperitonitis and massive intra-abdominal bleeding.

    Rupture of a racemose hemangioma causing dilatation and tortuosity of the bronchial artery can result in massive bleeding and respiratory failure. Bronchial artery embolization (BAE) can treat this life-threatening condition, as we show in two cases. The first case was of an 89-year-old female complaining of sudden-onset chest and back pain. Bronchial artery angiography demonstrated a racemose hemangioma with a 2 cm aneurysm. The second case was of a 50-year-old male with hemoptysis and dyspnea, eventually requiring intubation. Bronchial arteriography showed a racemose hemangioma and a bronchial artery-pulmonary arterial fistula. BAE was successfully performed in both cases, with no recurrent hemorrhage. Therapeutic interventions in bronchial artery racemose hemangiomas include lobectomy or segmentectomy, bronchial arterial ligation, and BAE. BAE should be considered as first-line therapy for bleeding racemose hemangiomas of the bronchial artery because of its low risk of adverse effects on respiratory status, minimal invasiveness, and faster patient recovery.

    Background

    The diagnosis of nonocclusive mesenteric ischemia (NOMI) is always challenging in critically ill patients. Herein, we aimed to report a case of NOMI associated with a hyperosmolar hyperglycemic state (HHS). A small amount of hepatic portal venous gas (HPVG) triggered the diagnosis of NOMI.

    Case Presentation

    A 77‐year‐old man was transferred due to shock and disorder of consciousness. He was diagnosed with an HHS. We suspected intestinal ischemia due to a small amount of HPVG revealed by computed tomography (CT). Peritoneal signs were revealed after treatment for the HHS. Computed tomography was carried out again 5 h after admission, which showed a large amount of HPVG, remarkable bowel dilatation, and pneumatosis intestinalis. We performed an emergency laparotomy and resected the small bowel necrosis resulting from NOMI.

    Conclusion

    An HHS can cause NOMI, and the presence of HPVG on CT is an important finding that suggests mesenteric ischemia, even in small amounts.

    BACKGROUND: Coronavirus disease 2019 (COVID-19) and heat related-illness are systemic febrile diseases. In the summer during the COVID-19 pandemic, a differential diagnosis between the two conditions is important. However, no studies have compared and distinguished heat-related illness from COVID-19. We aimed to compare the data between patients with early-stage heat-related illness and those with COVID-19. METHODS: This retrospective observational study included 90 patients with early-stage heat-related illness selected from Heatstroke STUDY 2017-2019 (nationwide registries of heat-related illness in Japan) and 86 patients with laboratory-confirmed COVID-19 with complaints of fever or fatigue and were admitted to one of two hospitals in Tokyo, Japan. RESULTS: Amongvital signs, systolic blood pressure (119 vs. 125 mmHg, p = 0.02), oxygen saturation (98% vs. 97%, p < 0.001), and body temperature (36.6 vs. 37.6 °C, p<0.001) showed significant between-group differences for the heatstroke and COVID-19 groups, respectively. Numerous intergroup differences in laboratory findings were present, including white blood cell counts (10.8 vs. 5.2 × 103/μL, p<0.001), creatinine (2.2 vs. 0.85 mg/dL, p<0.001), and C-reactive protein (0.2 vs. 2.8 mg/dL, p<0.001), although a logistic regression model achieved an area under the curve (AUC) of 0.966 using these three factors. A Random Forest machine learning model achieved accuracy, precision, recall, and AUC of 0.908, 0.976, 0.842, and 0.978, respectively. Creatinine was the most important feature of this model. CONCLUSIONS: Acute kidney injury was associated with heat-related illness, which could be key in distinguishing or evaluating patients with fever in the summer during the COVID-19 pandemic.

    門脈気腫および腸管気腫はまれではあるが重篤な所見と考えられ,腸管壊死はその最も多い原因である.外傷を契機とし,これらの所見を認める症例は報告されているが,治療中,遅発性に発生した症例はみられないため報告する.症例は70歳代男性,交通事故にて受傷し,現着時,意識障害とショックのため当救命センターに搬送された.重症頭部外傷を認め保存的加療を行っていたが,第4病日にショックを来し,CTにて門脈気腫および腸管気腫を認めた.腸管壊死による敗血症性ショックを疑い,試験開腹術を施行したが壊死を認めなかった.術後,抗生剤投与と血液浄化法により全身状態は改善し,下腿骨骨折の手術後,第41病日にリハビリ目的に転院となった.(著者抄録)

    AIM: Assessment of brainstem function plays a key role in predicting the neurological outcome after cardiac arrest. However, the relationship of the two quantitative brainstem assessment methods-automated infrared pupillometry (AIP) and auditory brainstem response (ABR)-with neurological prognoses remains unclear. This study compares the prognostic value of AIP and ABR after cardiopulmonary arrest. METHODS: This retrospective observational study included 124 comatose patients after cardiopulmonary arrest. ABR and AIP measurements were performed simultaneously within 72 h after return of spontaneous circulation. Neurological outcome was assessed at discharge by estimating the cerebral performance category (CPC) score; favourable neurological outcome (CPC score, 1-2) or poor neurological outcome (CPC score, 3-5). The correlation of each AIP parameter and ABR I-V wave latency was tested using Pearson's product moment correlation coefficient, and the prognostic value was compared using the area under the receiver operating characteristics curves (AUC). RESULTS: Pupillary light reflex (PLR) was not detected in 69 patients, and ABR wave V was not detected in 47 patients. All these patients had poor neurological outcome. Among those whose PLR and ABR could be measured, each AIP parameter had a tendency to be correlated with ABR I-V wave latency. Pupil constriction velocity provided the greatest AUC (0.819), with 81% sensitivity and 77% specificity. ABR I-V wave latency provided extremely low AUC (0.560). CONCLUSIONS: Although AIP and ABR were correlated, the AIP measures were superior in predicting the neurological outcome after cardiac arrest as compared with the ABR measures.

    Objectives:

    Previous studies have suggested that vasodilator therapy may be beneficial for patients with nonocclusive mesenteric ischemia. However, robust evidence supporting this contention is lacking. We examined the hypothesis that vasodilator therapy may be effective in patients diagnosed with nonocclusive mesenteric ischemia.

    Design:

    Retrospective cohort study.

    Setting:

    The Japanese Diagnosis Procedure Combination inpatient database.

    Patients:

    A total of 1,837 patients with nonocclusive mesenteric ischemia from July 2010 to March 2018.

    Interventions:

    None.

    Measurements and Main Results:

    We compared patients who received vasodilator therapy (vasodilator group; n = 161) and those who did not (control group; n = 1,676) using one-to-four propensity score matching. Vasodilator therapy was defined as papaverine and/or prostaglandin E1 administered via venous and/or arterial routes within 2 days of admission. Only patients who did not receive abdominal surgery within 2 days of admission were analyzed. The main outcomes were in-hospital mortality and abdominal surgery performed greater than or equal to 3 days after admission. After propensity score matching, in-hospital mortality was significantly lower in the vasodilator group (risk difference, –11.6%; p = 0.005). The proportion of patients who received abdominal surgery at greater than or equal to 3 days after admission was also significantly lower in the vasodilator group (risk difference, –10.2%; p = 0.002).

    Conclusions:

    Vasodilator therapy with papaverine and/or prostaglandin E1 is associated with lower in-hospital mortality and prevalence of abdominal surgery in patients with nonocclusive mesenteric ischemia.

    A 74-year-old male was referred to our critical care department for refractory severe watery diarrhea with advanced leukocytosis (over 70,000/μl) after multiple administrations of eradication therapy against Helicobacter pylori (HP). He was diagnosed as having fulminant colitis due to Clostridioides difficile after antimicrobial eradication therapy. He was given intravenous metronidazole and oral vancomycin. He also received supportive therapy including continuous hemodiafiltration for severe metabolic acidosis. However, despite emergency open sigmoidectomy, he died. The C. difficile isolate recovered was PCR-ribotype 002, which was positive for toxins A and B but negative for binary toxin. HP eradication therapy for prevention of chronic gastritis and stomach cancer is now in widespread use. Although such secondary severe complications are rare, we consider it to be necessary to pay sufficient attention when administering HP eradication therapy.

    Background: Portal venous gas (PVG) and pneumatosis intestinalis (PI) are rare pathologic findings, and a delayed appearance of portal vein thrombosis (PVT) in such patients is extremely rare. Case Presentation: A 51-year-old man complaining of epigastric pain was referred to our hospital. Computed tomography (CT) at admission revealed massive PVG and extensive PI, but no PVT. Emergency laparotomy was carried out, but bowel resection was unnecessary. On follow-up CT on postoperative day 5, thrombosis was noted in the portal venous system, and anticoagulant was started immediately. This patient was discharged and continued to take the anticoagulant. Seven months after discharge, PVT had disappeared on CT without any thromboembolic complications. Conclusion: If acute PVT is detected, anticoagulant is needed to prevent bowel ischemia and/or portal hypertension due to the growth of the thrombus. Clinicians should be aware of the potential for such a complication, and make their best efforts to exclude this entity using CT or sonography.

    カイロプラクティックという軽微な外力により両側頸部内頸動脈解離を来したと考えられた1例を経験したので報告する.症例は41歳の女性,痙攣を主訴に救急搬送された.頭部CTでくも膜下出血を,脳血管撮影でくも膜下出血の原因と考えられる左内頸動脈瘤を認めた.また両側頸部内頸動脈解離を認めた.同日脳動脈瘤クリッピング術を行ったが,翌日脳梗塞が出現した.その後の病歴聴取で27日前からのカイロプラクティックへの通院歴が判明し,両側頸部内頸動脈解離の原因であると考え,脳梗塞の原因も左頸部内頸動脈解離からの血栓塞栓症と考えた.抗血小板薬で治療を行い第29病日にリハビリ病院に転院した.発症3ヵ月後のMRAでは両側頸部内頸動脈解離は軽快した.(著者抄録)

    70代女性。自殺目的に千枚通し(約20cm)を腹部に刺した。約7時間後、体動困難となっているところを家族に発見され、千枚通しが刺さったまま救急搬送された。CT検査で腹部正中に千枚通し(成傷器)を認め、その周囲に少量の血腫を認めたが、明らかなextravasationは認めなかった。成傷器が胃を貫通していることは確認できたが、血管損傷の有無についてはアーチファクトにより判断困難であった。成傷器の除去と臓器損傷の確認・修復目的で緊急手術を行った。手術所見は、成傷器が幽門輪の口側の胃を貫通し、膵頭部下縁の後腹膜を通り下大静脈と腹部大動脈の間を貫き、椎骨で止まっていた。胃部分切除を行ったのち成傷器を抜去したところ、成傷器は脾静脈合流部直上の門脈を貫いていることが明らかになり、同部から出血を認めたため、門脈・脾静脈・下腸間膜静脈・上腸間膜静脈をクランプし修復した。術後経過は良好で、20日後にリハビリ目的で転院となった。

    40代男性。前医で全大腸型潰瘍性大腸炎と診断され、メサラジンとプレドニゾロンによる治療が開始されたが、病勢コントロールは不良であった。経過中に造影CTで直腸の穿通を認められ、当院へ搬送された。同日緊急でS状結腸双孔式人工肛門造設術+経皮的傍直腸周囲ドレナージ術を行い、可及的に大腸温存を図った。術後3病日に人工肛門から大量の血性排液を認めたため、経皮的動脈塞栓術で止血後、結腸亜全摘+回腸人工肛門造設術を行った。術後合併症として麻痺性イレウスを認めたものの内科的治療で軽快し、術後47病日に自宅退院した。患者は肛門温存を希望しなかったため、術後6ヵ月時に待機的に直腸切断術を行った。

    Kodamaea ohmeri is a relatively rare yeast isolated form clinical specimens, and it is known to be a causative fungus of severe invasive infectious diseases in immunocompromised hosts. Herein, we describe fungemia due to K. ohmeri in a patient with a severe extended burn. The isolate was obtained from not only blood specimens but also skin lesions. We should be aware of risk for fungemia including K. ohmeri in case of severe burn.

    54歳男。数日前からの発熱・下痢を主訴に前医受診し、血小板減少と高度の肝・腎機能障害がみられたため当院に搬送された。既往歴としてアルコール性肝機能障害とC型肝炎があることから、これらによる病態を疑い診療を開始した。しかし、画像検査で腹部臓器の器質的異常がみられないなど、既往疾患の急性増悪としては非典型的であった。そこで詳細な病歴聴取を行い、一般廃棄物運搬業に就労していることが判明した。不衛生な環境を背景に肝・腎障害と血小板減少があることからレプトスピラ症を疑い、血清学的検査により確定診断した。スルバクタム/アンピシリンの投与とCHDFを行い、改善が得られた。

    This study aimed to identify neurological and pathophysiological factors that predicted return of spontaneous circulation (ROSC) among patients with out-of-hospital cardiac arrest (OHCA). This prospective 1-year observational study evaluated patients with cardiogenic OHCA who were admitted to a tertiary medical center, Nippon Medical School Hospital. Physiological and neurological examinations were performed at admission for quantitative infrared pupillometry (measured with NPi-200, NeurOptics, CA, USA), arterial blood gas, and blood chemistry. Simultaneous blood samples were also collected to determine levels of neuron-specific enolase (NSE), S-100b, phosphorylated neurofilament heavy subunit, and interleukin-6. In-hospital standard advanced cardiac life support was performed for 30 minutes.The ROSC (n = 26) and non-ROSC (n = 26) groups were compared, which a revealed significantly higher pupillary light reflex ratio, which was defined as the percent change between maximum pupil diameter before light stimuli and minimum pupil diameter after light stimuli, in the ROSC group (median: 1.3% [interquartile range (IQR): 0.0-2.0%] vs. non-ROSC: (median: 0%), (Cut-off: 0.63%). Furthermore, NSE provided the great sensitivity and specificity for predicting ROSC, with an area under the receiver operating characteristic curve of 0.86, which was created by plotting sensitivity and 1-specificity. Multivariable logistic regression analyses revealed that the independent predictors of ROSC were maximum pupillary diameter (odds ratio: 0.25, 95% confidence interval: 0.07-0.94, P = 0.04) and NSE at admission (odds ratio: 0.96, 95% confidence interval: 0.93-0.99, P = 0.04). Pupillary diameter was also significantly correlated with NSE concentrations (r = 0.31, P = 0.027). Conclusively, the strongest predictors of ROSC among patients with OHCA were accurate pupillary diameter and a neuronal biomarker, NSE. Quantitative pupillometry may help guide the decision to terminate resuscitation in emergency departments using a neuropathological rationale. Further large-scale studies are needed.

    症例は69歳男性で、3日前から発熱があり、家人が自宅内で倒れているところを発見し、救急要請した。右上肢に限局したミオクローヌスを認め、血液検査で血小板減少と血液凝固異常を認めた。急性期DIC score 8点であり播種性血管内凝固(DIC)と診断した。また、髄液グラム染色でグラム陽性双球菌を認め、髄液肺炎球菌迅速抗原検査が陽性であったことから、肺炎球菌性髄膜炎と診断した。各種培養提出後に抗菌薬セフトリアキソン、バンコマイシンの投与とステロイド剤を開始した。また、DICに対してはAT-III製剤を開始した。第4病日に頭部CTを撮影したところ、mid line shiftを伴った左前頭葉皮質下出血、脳室内血腫を認めた。緊急開頭血腫除去術の方針とし、仰臥位、左前頭側頭開頭でアプローチし、皮質下血腫を顕微鏡下に除去した。肺炎球菌感染から血管炎、微小出血に至り、血液凝固異常も相まって出血が増大したと推測された。

    症例は47歳の男性。バイク走行中に普通乗用車と衝突し受傷した。ドクターカー接触時、SpO2の低下と動揺胸郭がみられたため気管挿管を行った。病院到着後に低酸素血症と右気胸のため、胸腔ドレーンを挿入した。CTでは右上葉と右下葉に巨大な外傷性仮性肺嚢胞を認めた。入院後、人工呼吸器管理を続け、呼吸状態は安定していたため徐々にウィーニングしたが、遅発性に嚢胞が拡大した。入院第5病日に抜管に向けて陽圧管理を終了したところ大量の気道出血により呼吸不全に至り、入院第6病日にも再度大量に気道出血を認めた。右上葉と右下葉の外傷性仮性肺嚢胞に対し、肺部分切除術を行った。低酸素血症が遷延したため、人工呼吸器の離脱に難渋したが術後10日目に抜管し、術後30日目に自宅退院した。外傷性仮性肺嚢胞は比較的稀な疾患として報告されていたが、外傷診療におけるCT検査の普及により診断される割合が増えている。基本的に保存的加療で軽快すると報告されているが、感染による肺膿瘍、気道出血のコントロールが不能な場合、胸腔内への破裂による気漏の持続時は手術適応となる。本症例において遅発性に嚢胞が拡大した誘因としては、損傷範囲が広く嚢胞径が大きかったこと、人工呼吸器による陽圧管理を施行していたこと、発症部位が胸膜下肺実質であり外側に進展しやすい形態であったことなどが考えられた。遅発性に気道出血を来した機序は、嚢胞の拡大により肺実質損傷が持続的に生じたこと、陽圧管理終了時に血腫が急激にドレナージされたことが想定された。本症例のように、遅発性の気道出血により呼吸不全を合併した外傷性仮性肺嚢胞症例では手術も考慮する必要がある。(著者抄録)

    門脈気腫および腸管気腫はまれではあるが重篤な所見と考えられ,腸管壊死はその最も多い原因である.外傷を契機とし,これらの所見を認める症例は報告されているが,治療中,遅発性に発生した症例はみられないため報告する.症例は70歳代男性,交通事故にて受傷し,現着時,意識障害とショックのため当救命センターに搬送された.重症頭部外傷を認め保存的加療を行っていたが,第4病日にショックを来し,CTにて門脈気腫および腸管気腫を認めた.腸管壊死による敗血症性ショックを疑い,試験開腹術を施行したが壊死を認めなかった.術後,抗生剤投与と血液浄化法により全身状態は改善し,下腿骨骨折の手術後,第41病日にリハビリ目的に転院となった.(著者抄録)

    外傷診療の進歩にかかわらず、出血性ショックに引き続いておこる多臓器不全は、いまだに重傷外傷における主要な死因である。キサンチンオキシダーゼ(XO)阻害薬は強力な抗酸化作用をもち、これを改善する可能性がある。XOは活性酸素の産生にかかわり、主要臓器や血管に広く分布することが知られている。我々はこれまで、XO阻害薬の抗酸化作用は脳虚血再灌流障害モデルにおいて臓器障害を軽減することを世界に先がけて見いだした。また、ラット出血性ショックモデルにおいて虚血腸管で産生される腸間膜リンパ液が肺障害に重要な役割を担うことを解明し、遠隔臓器障害における機序を見いだした。これらから、全身の虚血再灌流障害である出血性ショックにおいてXO阻害薬は有効であると考えられる。以上を踏まえ本研究では、ラット出血性ショックモデルにおいてXO阻害薬が全身の臓器障害を軽減するか否かを検討する。本年は酸化ストレス(MDA)と炎症性メディエータ(IL-6、TNF-α)、病理検査による評価を行った。キサンチンオキシダーゼ阻害薬投与群の肺において、酸化ストレスマーカー・炎症性メディエータが低下し、病理検査で肺障害が軽減していることが明らかとなった。
