    BACKGROUND: With the launch of anti-amyloid beta antibody drugs, physicians face the need to quickly diagnose dementia. In prehospital consultation services in Japan, the duration from memory-related consultations to confirmed diagnosis has not been sufficiently investigated. Therefore, this study investigated the duration from initial community-based dementia consultation to confirmed diagnosis and the factors associated with diagnostic delays across dementia subtypes. METHODS: This retrospective observational study included patients who consulted the Community Consultation Center for Citizens with Mild Cognitive Impairment and Dementia (CCCCMD) and subsequently received a confirmed dementia diagnosis at Nippon Medical School Musashi Kosugi Hospital between 2010 and 2024. Time to diagnosis was defined as the number of days from the initial CCCCMD consultation to diagnostic confirmation. We classified those who were diagnosed with dementia during outpatient visits amongst the consultees as Alzheimer disease (AD) or non-Alzheimer disease (non-AD). Diagnostic delay was further divided into pre-hospital interval and in-hospital phase. Group comparisons and multivariable Cox proportional hazards and logistic regression analyses were performed to identify factors associated with diagnostic delay. RESULTS: A total of 739 patients were included: 504 with AD and 235 with non-AD dementias. Time to diagnosis was shorter in the AD group than in the non-AD group. This difference was primarily attributable to prolonged delays during the pre-hospital interval amongst patients with non-AD; however, delays during the in-hospital diagnostic process were small and largely overlapping between the groups. Diagnosis of AD, older age, and the presence of a primary care physician were associated with a shorter time to diagnosis, whereas higher cognitive scores at the initial community consultation were associated with longer diagnostic delays. CONCLUSION: Diagnostic delay differed substantially by dementia subtype. These findings highlight the importance of community-based consultation pathways as key targets for reducing diagnostic delays, particularly for patients with non-AD dementias.

    BACKGROUND: The number of individuals with dementia in Japan is increasing rapidly, placing a significant burden on family caregivers. Following a diagnosis, caregivers often experience confusion and psychological distress. Although early support can help alleviate these challenges, time and geographic constraints frequently limit access. Telephone-based counselling may offer a practical alternative, particularly for older adults who are comfortable using telephones. METHODS: This prospective cohort study targeted 266 primary caregivers of individuals newly diagnosed with dementia or mild cognitive impairment at Nippon Medical School Musashikosugi Hospital between June 2022 and December 2023. A self-administered questionnaire was distributed following the initial consultation. Of the 125 respondents, 116 provided complete data and were included in the analysis. Variables assessed included caregiver demographics, care context, health status, social support and willingness to use telephone consultations. Chi-square tests were used to compare the characteristics of those who were willing versus unwilling to use telephone consultation support. RESULTS: A total of 48 caregivers expressed willingness to use telephone consultation support. This group exhibited significantly higher rates of depressive symptoms, lack of instrumental support and a greater likelihood of handling medical or service-related calls. Conversely, patients with chronic shoulder pain were less likely to use telephone consultation support. Many participants were in the early stages of caregiving, reported relatively low burden, yet demonstrated a high need to continue. CONCLUSION: Telephone consultation support appears particularly beneficial for caregivers who serve as primary communicators with health services, lack adequate support and exhibit signs of depression. Alternative support formats are needed for caregivers with physical limitations. Tailored, early-stage interventions are crucial to effectively support family caregivers in the period following their relative's dementia diagnosis.

    This case report highlights a rare occurrence of paraplegia following lumbar puncture (LP) in the presence of cervical disc herniation during cerebrospinal fluid (CSF) drainage. The patient was an elderly woman undergoing investigation for suspected neoplastic meningitis who experienced sudden acute paraplegia on the day following the LP procedure. LP is often contraindicated in patients with intracranial lesions. If patients exhibit myelopathy symptoms, such as increased lower extremity reflexes and positive Hoffmann and Trömner's signs without intracranial lesions, a thorough evaluation for occupying lesions, including cervical spondylotic myelopathy, is recommended before LP is carried out.

    OBJECTIVES: This explores the characteristics of patients with worsening dementia who did not receive a specialized medical examination or care. METHODS: This study utilized a mixed methods analysis. Of the 2712 people who received the Mini Mental State of Examination (MMSE) at the Community Consultation Center for Citizens with MCI and Dementia between December 2007 and December 2019, 1413 people who scored 23 points or less were included. Participants were categorized into mild, moderate, and severe groups, based on their MMSE scores. Participants' characteristics-gender, age, presence or absence of an escort, demographics, family type, and presence or absence of a family doctor-were compared between the groups. To further understand the severe group's characteristics, clinical psychologists recorded consultation forms were categorized. RESULTS: More than 80% of the patients in each group had a family doctor. Moreover, all the severe groups had escorts, and the role of family members and supporters was important for the consultation. In the severe group, 29 patients had never received specialized medical care. Their characteristics were coded "non-existence" (fewer people or opportunities to notice their needs), "connection failure" (a lack of access or connections to consultations), and "evaluation failure" (not recognized as a problem requiring consultation). CONCLUSIONS: It is necessary to improve primary physician education, disseminate knowledge, and raise awareness about dementia, besides building and strengthening networks to alleviate the isolation of dementia patients and their families. The psychological aspects of family members' denial regarding their family members with dementia must be addressed through intervention.

    Guillain-Barré syndrome (GBS) comprises a group of polyneuropathies characterized by rapid progression of limb paralysis. Various subtypes of GBS have been reported. The oculopharyngeal subtype of GBS is currently understood to be primarily a cranial polyneuropathy without limb weakness or cerebellar ataxia. In our case of 62-year-old man, gastrointestinal infection was followed by paranesthesia of the hands. He had bilateral ptosis, pharyngeal disorder, and tongue and bifacial weakness. We diagnosed oculopharyngeal subtype of GBS. It responded to intravenous immunoglobulin. This case highlights the need for further characterization of unusual GBS subtypes.

    抗N-methyl-D-aspartate(NMDA)受容体脳炎後に発症した薬剤抵抗性てんかんに対し、外科的治療を行い経過が良好であった2例を経験したので報告する。症例は20代の男女で、全身性痙攣を認め入院となり、抗NMDA受容体脳炎と診断され、加療された。退院の後、てんかん発作が薬剤抵抗性となったために緩和的外科的加療を行い、奏功した。抗NMDA受容体脳炎後の症候性てんかんの文献的報告は少ないが、薬剤抵抗性の症例では、外科的治療が選択となり得る。(著者抄録)

    症例は45歳女性で、過睡眠、意識障害で受診し、MRIにて大脳皮質に異常病変を認めた。頭部MRIのFLAIR強調画像で視床、視床下部、脳梁膨大部に高信号領域を認めた。経過などからバソプレシン分泌過剰症(SIADH)による低Na血症を疑った。診断基準に沿って浸透圧や各種ホルモンなどの検査を追加し、診断基準と照合して矛盾しない所見が得られた。MRIにおける中枢性病変と抗AQP4抗体陽性が判明し、SIADHを併発した視神経脊髄炎スペクトラム疾患(NMOsd)と診断した。低Na血症に対する初期治療として高張食塩水の点滴静注を行い、NMOsdの診断後からはステロイドパルス療法を施行した。意識障害は改善したが見当識障害や計算能力の低下などの高次脳機能障害が残存したため、単純血漿交換を施行し、意思疎通が可能な状態にまで回復した。血漿交換後に施行したMRIは入院時と比較して高信号部位の減弱を認めた。

    症例は66歳男性。頭痛・眼痛に続き、複視も出現し入院。左眼に外転麻痺、眼球突出を認め、Tolosa-Hunt症候群を疑いメチルプレドニゾロン1g×3日で加療したが改善無し、造影MRI、髄液、IgG4、ANCAなどは異常を認めず、HbA1c9.2と糖尿病を認めた。発症1週後から、鼻背に帯状疱疹が出現し角膜病変を伴った。眼部帯状疱疹と診断し、アシクロビル15mg/kgを開始、プレドニゾロン1mg/kgから漸減した。眼痛・皮疹は速やかに改善、眼球運動は数ヵ月で徐々に改善した。眼部帯状疱疹で複視が皮疹に先行することは稀である。未治療糖尿病と強力なステロイド治療が、皮疹の出現を抑制していた可能性を推測した。(著者抄録)

    74歳女。手指末端の疼痛・潰瘍、下肢筋力の低下、下腿浮腫が出現し、徐々に増悪した。抗signal recognition particle(SRP)抗体が陽性で、筋炎を疑う画像所見から右大腿直筋より筋生検を施行した。単核球の浸潤に乏しく筋線維の壊死像を認め、抗SRP抗体陽性ミオパチーと診断した。また、尿蛋白・尿潜血を認めたため腎生検を施行し、免疫組織化学的にIgA主体の糸球体メサンギウム領域への沈着を認め、メサンギウム増殖性腎炎の組織像を呈しており、IgA腎症と診断した。プレドニゾロン45mg/日内服を開始したところ、筋力は改善し、高値であったCKも低下傾向を示した。しかし、CKの低下が500IU/lでとどまったため、大量ガンマグロブリン静注療法を施行し、その後カルシニューリン阻害薬を併用した。その結果、CKの正常化までに3ヵ月、自力歩行可能までに更に3ヵ月以上を要したが、7ヵ月後に自宅退院した。尿蛋白・潜血も陰性化し、ミオパチーの再燃は認めていない。

    心房細動根治術で切除された左心耳組織所見から心臓サルコイドーシスの診断に至った1例を報告する.症例は57歳女性.呼吸困難で来院し,うっ血性心不全・心房粗細動・非持続性心室頻拍で入院.心房中隔欠損症,僧帽弁・三尖弁閉鎖不全症を認め,外科治療を施行した.心房細動根治術で切除した左心耳に肉芽腫を認めた.心筋生検でもサルコイド結節を認める例は少なく,肉芽腫が心耳に形成されることは稀だが,不整脈例では常に心サルコイドーシスを念頭におくこと,手術で切除した心肺検体は全て病理学的検索を行うことが重要と考えた(著者抄録)
