    BACKGROUND: In Japan, the lateral position method is known as a postural therapy for breech presentation wherein the mother lies down in lateral position according to the orientation of the fetal back. Few studies have formally tested lateral position management for breech presentation, and no method exists to prevent breech recurrence after cephalic version. We hypothesized that postural management comprising a combination of opposite-side lateral position for breech presentation and same-side lateral position after cephalic version demonstrates a clinically relevant effect size on breech presentation. METHODS AND FINDINGS: We conducted a stratified, open-label randomized controlled trial at an academic hospital in Kawasaki, Japan. A total of 200 women diagnosed with breech presentation between 28 +  0 and 30 +  0 gestational weeks were randomized to postural management (n =  100) or control (no intervention, n =  100) group. The intervention was instruction every 2 weeks on lying in the lateral position on the opposite-side of fetal back for breech presentation and on the same-side of fetal back for head-first presentation. The primary outcome was the rate of fetuses in breech presentation at 37 weeks of gestation, and the secondary outcomes were cesarean delivery, cesarean delivery for breech presentation, head presentation 2, 4, and 6 weeks later, breech presentation recurrence, and adverse events. Breech presentation rate at 37 gestational weeks was 11% in the intervention group, using the combination of the opposite-side and same-side lateral positions, compared with 19% in the control group. However, we found no statistical significance in the intention-to-treat analysis (11% [11/100] versus 19% [19/100]; relative risk, 0.58 [95% CI, 0.29 to 1.15]; p =  0.11). In the control group, 23 participants (23%) unknowingly took the same posture as the intervention group, and the prespecified per-protocol analysis excluding crossover found the same direction of effect but with statistical significance. In the intention-to-treat analysis, the intervention group had a higher cephalic version rate 2 weeks after the instruction (69% [69/100] versus 54% [54/100]; relative risk, 0.67 [95% CI, 0.47 to 0.96]; p =  0.029), and lower breech presentation recurrence rates (2% [2/91] versus 10% [9/88]; relative risk, 0.22 [95% CI, 0.048 to 0.97]; p =  0.031) than the control group. Regarding adverse events in the intervention group, three participants experienced discomfort and one participant complained of pain in the lateral abdomen; these symptoms resolved spontaneously. CONCLUSIONS: For breech presentation at the beginning of the third trimester, providing postural therapy instruction on opposite-side lateral positioning and same-side lateral positioning was associated with 8% reduction of breech fetuses at 37 gestational weeks compared with the control group, but this primary endpoint did not reach statistical significance. Regarding the secondary endpoints, the intervention group showed a significantly higher rate of cephalic version after 2 weeks and lower rate of breech recurrence. The direction of the effect of postural therapy based on fetal back position on breech presentation was promising, and further research to validate this approach, with consideration for unplanned participant crossover, may be warranted.     BACKGROUND: The purpose of this study was to determine whether cases of placenta previa and low-lying placenta previa in patients with retroverted uterus are significantly associated with endometriosis. METHODS: Participants were patients who underwent cesarean section at our hospital with a diagnosis of placenta previa or low-lying placenta previa within a 7-year period from January 2015 to December 2022. Of these, patients with multiple pregnancies and those without a complete uterine image in the medical record at less than 12 weeks' gestation were excluded. Included patients were divided into two groups according to the presence or absence of endometriosis. The presence of endometriosis was determined based on intraoperative findings. A retrospective case-control study was conducted by examining the presence or absence of retroverted uterus during early pregnancy and the presence or absence of posterior placenta. RESULTS: A total of 110 patients were included, 32 in the group with endometriosis and 78 in the group without endometriosis. There were 15 (46.9%) cases of retroverted uterus in the group with endometriosis and 17 (21.8%) in the group without endometriosis, indicating significantly more cases in the group with endometriosis (P=0.01). There were 15 (46.9%) cases of retroverted uterus with posterior placenta in the group with endometriosis and 16 cases (20.5%) in the group without endometriosis, indicating significantly more cases in the group with endometriosis (P=0.009). CONCLUSION: Placenta previa and low-lying placenta previa in cases of retroverted uterus are significantly associated with endometriosis.

    BACKGROUND: The coronavirus disease 2019 (COVID-19) pandemic has forced people to make major life changes, and there is concern that depressive tendencies have increased in pregnant women during the pandemic. This study used the Edinburgh Postnatal Depression Scale (EPDS), administered in the second trimester, to investigate the effect of COVID-19 on the mental health of Japanese women during pregnancy and to provide early intervention. METHODS: The study included 1,320 pregnant women (663 pre-COVID-19 and 657 during COVID-19) with similar background characteristics and compared the results for the COVID-19 period (September 2020-August 2021) and control period (September 2018-August 2019). Women treated for psychiatric disorders were excluded. The EPDS cutoff score was 13. RESULTS: The median EPDS scores were 3 (1-6) points during the control period and 3 (1-5) points during the pandemic (p = 0.166) for the control and pandemic periods. Fourteen patients (2.1%) during the control period and 20 (3.0%) during the pandemic scored ≥13 points; however, the difference was not significant (odds ratio, 1.455; 95% confidence interval: 0.692-3.143). CONCLUSIONS: COVID-19 did not change mid-pregnancy EPDS scores at a single Japanese center.

    Accessory cavitated uterine mass(ACUM)はまれな子宮嚢胞性疾患で,30歳未満の若年女性に好発し,重症の月経困難症や慢性的な下腹部痛を症状とする.ACUMの多くは,ホルモン療法に抵抗するといわれている.今回,低用量エストロゲン・プロゲスチン配合剤(LEP)投与により疼痛が軽快し保存療法を継続できているACUMを強く疑う1症例を経験したので報告する.症例は23歳,未妊の女性で,1年前から月経痛と持続下腹部痛を認め,経腟超音波断層像では子宮は正常大を示し,子宮左側壁に内腔を有する約3cm大の腫瘤を認めた.骨盤部MRIでは子宮筋腫や子宮腺筋症は検出されず,左子宮円索起始部付近にT1強調画像で高信号,T2強調画像でshadingを示す内腔を有する嚢胞を認めACUMを強く疑った.治療方針として手術を提案したが希望されず,保存的治療としてLEPを投与した.投与開始2ヵ月後には,疼痛は減少した.ACUMが疑われるが手術を希望されない症例にLEP投与が有効となりうると考えられた.(著者抄録)

    AIM: This study aimed to determine the efficacy of postural management in the lateral position for primiparous breech presentation. METHODS: A retrospective cohort study was conducted at a single institution from January 2020 through December 2020. Participants were singleton primiparous pregnant women diagnosed with breech presentation between 28 + 0 and 29 + 6 weeks of gestation. The exclusion criteria were scheduled cesarean delivery, uterine malformation, transverse position, and scheduled delivery at another hospital. A doctor instructed the women in the intervention group to lie on their right sides several times a day if the fetal back was on the left side or lie on their left sides if the fetal back was on the right side. The knee-chest position and other methods were not recommended. The control group received expectant management care. The primary endpoint was the percentage of fetuses in a cephalic presentation 2 weeks later. RESULTS: Of the 56 women included in the study, 17 women were instructed to lie in the lateral position, and 39 women received expectant management care only. After 2 weeks, women who were instructed to lie in lateral position had a higher rate of fetal cephalic version than the control group (82.4% [14/17] vs. 43.6% [17/39], p = 0.017). No study participants experienced adverse effects. CONCLUSIONS: Two weeks of postural management in the lateral position without the knee-chest position significantly reduced the rate of primiparous breech presentation in the third trimester of pregnancy.

    子宮内膜症合併妊娠において,様々な周産期合併症,特に前置胎盤のリスクが高いことが近年報告されている.しかし子宮内膜症が前置胎盤を引き起こす機序や予防については明らかにされていない.他方,子宮内膜症は手術以外の臨床所見や血液検査・画像所見のみで診断するには限界があり,分娩時に子宮内膜症を合併していたかどうかは不明である.今回我々は,帝王切開術を行った患者に着目し,帝王切開時の実際の腹腔内所見と既往歴を元に,胎盤位置異常群における子宮内膜症の合併率,胎盤位置異常群における子宮内膜症発生部位と子宮後壁癒着を伴う割合を算出し,子宮内膜症と前置胎盤の関連について検討した.子宮内膜症合併率は胎盤位置異常群で35.0%(42/120),対照群で16.3%(39/240)であり,胎盤位置異常群で有意に高かった(OR 2.77;95% CI:1.66〜4.61;P<0.001).胎盤位置異常群での子宮内膜症発生部位は,子宮後壁に多く,30.0%(36/120)で子宮後壁癒着を伴い,対照群3.8%(9/240)より有意に高かった(OR 17.8;95%CI:6.74〜47.1;P<0.01.adjusted OR 11.0;95%CI:5.08〜23.8).子宮内膜症による後壁癒着が胎盤位置異常をもたらす可能性が高い.(著者抄録)

    卵管間質部妊娠は子宮筋層を通過する卵管部分の妊娠であり、卵管間質部楔状切除術後の妊娠は、他の子宮手術既往妊娠と同様に手術瘢痕部の子宮破裂リスクがある。他方、卵管間質部楔状切除術後妊娠における妊娠管理や分娩方法に関して明確なコンセンサスは得られていない。さらに生殖補助医療(assisted reproductive technology:ART)の増加に伴って卵管妊娠が増加しており、卵管間質部妊娠の頻度も増加する可能性がある。今回、右卵管間質部楔状切除術後の妊娠で、妊娠32週に突然右下腹部痛を認め、常位胎盤早期剥離の疑いで母体搬送され、子宮手術既往妊娠陣痛発来による切迫子宮破裂の疑いで緊急帝王切開術を行ったところ、手術時に子宮破裂が判明した症例を経験した。十分な情報量がない状態の元、別の診断で救急搬送され、胎児心拍数陣痛図に異常を示さず、術前診断が困難であったケースであり、非常に教訓となった。本症例を通じて卵管間質部楔状切除術後妊娠における適切な妊娠、分娩管理に関して考察したので報告する。(著者抄録)

    卵管間質部妊娠は子宮筋層を通過する卵管部分の妊娠であり、卵管間質部楔状切除術後の妊娠は、他の子宮手術既往妊娠と同様に手術瘢痕部の子宮破裂リスクがある。他方、卵管間質部楔状切除術後妊娠における妊娠管理や分娩方法に関して明確なコンセンサスは得られていない。さらに生殖補助医療(assisted reproductive technology:ART)の増加に伴って卵管妊娠が増加しており、卵管間質部妊娠の頻度も増加する可能性がある。今回、右卵管間質部楔状切除術後の妊娠で、妊娠32週に突然右下腹部痛を認め、常位胎盤早期剥離の疑いで母体搬送され、子宮手術既往妊娠陣痛発来による切迫子宮破裂の疑いで緊急帝王切開術を行ったところ、手術時に子宮破裂が判明した症例を経験した。十分な情報量がない状態の元、別の診断で救急搬送され、胎児心拍数陣痛図に異常を示さず、術前診断が困難であったケースであり、非常に教訓となった。本症例を通じて卵管間質部楔状切除術後妊娠における適切な妊娠、分娩管理に関して考察したので報告する。(著者抄録)

    当院で2007〜2020年に腹壁子宮内膜症の疑いで外科的治療を受け組織学的検査で確定診断された9例を対象とし、「年齢」「既往手術」「既往手術から自覚症状出現までの期間」「主訴」「病変の大きさ」「外科的治療の内容」「手術時間」「術中出血量」などについて調査した。年齢は33〜45歳(平均38歳)、既往手術は帝王切開術が7例、虫垂炎手術2例であった。帝王切開例における手術から自覚症状出現までの期間は平均2.0±1.07年であった。主訴は腹部の疼痛が5例、疼痛+腫瘤が2例、腫瘤が2例であった。外科的治療の内容は全例が腫瘤切除術で、手術時間は平均1時38分(40分〜2時間58分)、術中出血量は全例「少量」であった。摘出病変の代表的な病理組織像を提示した。

    巨大絨毛膜下血腫(Breus' mole)は絨毛膜板直下に生じる巨大血腫で,1,000〜2,000分娩に1例程度で生じる稀な疾患である.今回,妊娠19週から認められ,高度な胎児発育不全(FGR)を伴ったBreus' moleの1例を経験したので報告する.症例は31歳の初産婦,妊娠21週でFGRのため当院紹介となり,妊娠22週から入院管理となった.入院時超音波所見で推定体重271g(-2.65 SD)と高度な発育不全を認め,羊水ポケットは2.3cmであった.胎盤は全体的に肥厚し,胎盤胎児面に低エコー域を認め,Breus' moleに合併したFGRと診断し,慎重に経過観察とした.妊娠27週0日に臍帯動脈拡張期血流の途絶を認めたため,帝王切開分娩とした.児は体重470g,胎盤は実質が一部欠失し,絨毛膜下に5cm大の空洞化を認め,同部分に非凝固性の血液が貯留していたと推定された.空洞化部分の辺縁に臍帯付着部を認め,胎盤病理学的検査では梗塞巣や石灰化が散見され,胎盤血流不全が示唆された.Breus' moleは,胎盤血流が阻害されることによる子宮内胎児死亡(IUFD)やFGR,羊水過少などを高率に合併し,予後不良例が多いとされる.本症例を疑った際は,適切な娩出時期の決定が重要である.(著者抄録)

    当院における稀少部位子宮内膜症の特徴と治療をまとめ、その転帰について検討した。鼠径部子宮内膜症症例は39歳女性で、2妊1産。挙児希望のため鎮痛薬で右鼠径部の疼痛緩和を図ったが、疼痛は増悪傾向であった。不妊治療を行ったが妊娠成立せず、ジエノゲストの内服を開始し疼痛は軽快した。腸管子宮内膜症症例は43歳女性で、2妊1産。腹痛および嘔吐を主訴に受診し、腹部CT検査で絞扼性イレウスを疑い、緊急手術となった。回腸の固有筋層から漿膜下組織で子宮内膜様の腺管と内膜間質様組織を認めた。免疫染色ではサイトケラチンAE1/AE3で陽性、CD10で陽性であった。胸腔子宮内膜症症例は50歳女性で、0妊0産。本人希望により、症状増悪時に胸腔鏡またはホルモン療法を行う方針で経過観察とした。現在、内膜症性嚢胞は縮小傾向にあり、閉経していない状態で症状は軽快している。膀胱子宮内膜症症例は30歳女性で、0妊0産。骨盤部単純MRI検査の後、腹腔鏡補助下卵巣嚢腫摘出術を施行した。術後の経腟超音波検査で子宮腺筋症と16.6×7.6mmの膀胱子宮内膜症が疑われ、再度骨盤部単純MRI検査を施行したところ、膀胱底部正中に結節を認め、膀胱子宮内膜症の診断に至った。不妊治療を行い、妊娠成立した。腹壁子宮内膜症症例は39歳女性で、2妊2産。皮下腫瘍摘出術を行った。摘出した検体は真皮から筋層に子宮内膜組織を認めた。下腹部痛はGnRHa注射の後、低用量エストロゲン・プロゲスチン配合薬を投与し軽快した。

    内臓錯位症候群は臓器の左右分化障害により発症し、先天性心疾患を高率に合併する。今回、胎児期に診断した内臓錯位症候群の3例を経験したため報告する。症例1:39歳、1経妊1経産。妊娠29週で錯位が疑われ、38週で自然分娩。脾臓と胃を右、肝臓を左に認め、胆嚢を認めず、先天性胆道閉鎖症にて腹腔鏡補助下肝門部空腸縫合術を施行した。症例2:29歳、3経妊2経産。妊娠13週でNT 4mmだったが、羊水検査で染色体異常なし。24週で錯位が疑われ、31週に胎児機能不全で緊急帝王切開で出生。下大静脈欠損・奇静脈接合と心房中隔欠損、多脾、右胃、対称肝を認めた。症例3:34歳、1経妊1経産。妊娠30週で錯位が疑われ、40週で自然分娩。対称肝で胃と脾臓を右に認め、出生後は適応良好だった。胎児期に胃泡の位置異常を認めた場合は、出生後も慎重なフォローアップが必要である。(著者抄録)
