    BACKGROUND: Compared to standard hospital meals, nutritional intervention using recovery K5 (RK5), a concentrated liquid diet, offers a comprehensive immunonutritional profile, suggesting its potential effectiveness in preventing surgical site infections (SSIs) after gastrointestinal surgery. AIM: To investigate the usefulness of RK5 in patients undergoing elective colorectal cancer surgery, focusing on postoperative infections and nutritional status. METHODS: This single-center, open-label, randomized, parallel-group comparative trial was conducted at Department of Gastrointestinal Surgery, Nippon Medical School Hospital, between February 2023 and August 2024. Forty patients with colorectal cancer were randomly assigned in a 1:1 ratio to either the nutritional intervention or the control group. The intervention group received 800 kcal/day of RK5 administered orally instead of breakfast and dinner (400 kcal per serving) 2 days prior to surgery, whereas the control group received only standard meals. Postoperative infection, nutritional status, and bowel habits were assessed. RESULTS: No cases of remote infection were observed. SSIs occurred in one of the 17 patients (5.9%) in the intervention group and six of the 18 patients (33.3%) in the control group, with an odds ratio of 0.125 (95% confidence interval: 0.013-1.181, P = 0.0695). Energy intake and percentage of target energy intake were significantly higher in the intervention group. No significant differences were observed between the two groups regarding nutritional status, bowel movement frequency, or the incidence of diarrhea. CONCLUSION: Supplemental nutrition using RK5 may help prevent SSIs in patients undergoing elective colorectal cancer surgery and should be considered as a potential option for perioperative nutritional management.

    PURPOSE: To compare the safety and efficacy of barbed and non-barbed sutures for fascial closure in abdominal surgery. METHODS: A systematic literature search through February 2025 identified studies comparing overall surgical site infections (SSI), fascial complications, and hospital stays between barbed and non-barbed sutures. A meta-analysis using random-effects models calculated odds ratios (ORs) or mean differences (MDs) with 95% confidence intervals (CIs). RESULTS: Seven studies involving 12,278 patients (barbed group, n = 4912; non-barbed group, n = 7366) were included. The overall SSI rates were 1.9% and 4.0% in the barbed and non-barbed groups, respectively. Barbed sutures significantly reduced overall SSIs (OR, 0.41; 95% CI: 0.31-0.53; P < 0.001) without statistical heterogeneity. Barbed suture also significantly reduced the length of hospital stay (MD, - 1.13; 95% CI: - 1.42- - 0.83, P < 0.001) without statistical heterogeneity. No significant difference was observed in fascial complications between the groups (OR, 0.66; 95% CI: 0.36-1.22, P = 0.19). CONCLUSIONS: This is the first meta-analysis to focus specifically on barbed sutures for abdominal fascial closure. Barbed sutures significantly reduce SSI and hospital stay without increasing fascial complications, thus suggesting that they are safe and efficient options for abdominal wall closure.

    The epithelial and mesenchymal features of colorectal adenocarcinoma (CRAC) cell lines were compared in two-dimensional (2D) and three-dimensional (3D) cultures. In 2D cultures, the three CRAC cell lines exhibited epithelial characteristics with high E-cadherin and low vimentin levels, whereas two exhibited mesenchymal traits with opposite expression patterns. In 3D cultures using low-attachment plates, mesenchymal cells from 2D cultures showed reduced vimentin mRNA levels. Morphologically, the five CRAC cell lines appeared similarly shaped in 2D culture but formed different structures in 3D culture. Epithelial DLD-1 and mesenchymal COLO-320 cells produced large granular spheres, whereas epithelial HCT-15 cells formed small solid spheres. Tubular structures were observed in epithelial CACO-2 and mesenchymal SW480 spheres. Desmosome-like structures developed in epithelial CRAC cells, whereas entosis was observed in CACO-2, HCT-15, and SW480 cells. The Ki-67-positive proliferating cell count varied in 2D and 3D cultures of epithelial cells but remained high and unchanged in mesenchymal cells. These findings suggest that while CRAC cells display distinct epithelial and mesenchymal properties in 2D cultures, they form diverse 3D structures, irrespective of these traits.

    INTRODUCTION: Anorectal malignant melanoma (ARMM) is an extremely rare and aggressive cancer that lacks specific tumor markers, making tumor dynamics difficult to monitor. Circulating tumor DNA (ctDNA), which contains cancer-specific gene mutations, has emerged as a promising biomarker for monitoring various malignant tumors. We herein report the clinical utility of ctDNA measurements in 2 patients with ARMM. CASE PRESENTATION: Case 1: A 64-year-old woman diagnosed with clinical Stage III ARMM underwent laparoscopic abdominoperineal resection with lateral lymph node dissection. Pathological findings confirmed R0 resection, and genetic analysis identified a neuroblastoma RAS viral (v-ras) oncogene homologue (NRAS) mutation (p.G12S). She remained recurrence-free for 7 years postoperatively. In a longitudinal ctDNA study, the preoperative ctDNA level (28.7 copies/mL) became undetectable after curative resection and remained undetectable throughout follow-up. By contrast, lactate dehydrogenase (LDH) currently the only blood-based tumor marker for mucosal melanoma, including ARMM, remained within the normal range (124-222 U/L) both preoperatively and postoperatively. Case 2: A 69-year-old woman diagnosed with clinical Stage III ARMM underwent laparoscopic abdominoperineal resection with lateral lymph node dissection. Pathological findings confirmed R0 resection, and genetic analysis identified two KIT proto-oncogene receptor tyrosine kinase (KIT) mutations (p.Y553H and p.Y646C) in exons 11 and 13. One year postoperatively, lung metastases were confirmed via positron emission tomography-computed tomography, and nivolumab monotherapy was initiated. The lung metastases remained stable for 7 months. In a longitudinal ctDNA study, the preoperative ctDNA level (28.7 copies/mL) became undetectable following curative resection. However, ctDNA re-emerged (9.05 and 7.2 copies/mL, respectively) at the time of lung metastasis detection. After initiating nivolumab treatment, ctDNA again became undetectable. Throughout the clinical course, the LDH level remained consistently within the normal range. CONCLUSIONS: We demonstrated the utility of ctDNA monitoring in patients with ARMM, a rare tumor. This method has the potential to be applied to other rare tumors if tumor-specific mutations can be identified.

    INTRODUCTION: Patients with obstructive colon cancer (OCC) with distant metastases often present with a poor general condition, including malnutrition, anemia, and systemic inflammation. Traditionally, these patients undergo stoma creation and/or primary tumor resection followed by systemic chemotherapy. However, for patients with DNA mismatch repair-deficient/microsatellite instability high (dMMR/MSI-H) colorectal cancer, the emergence of immune checkpoint inhibitors (ICIs) has revolutionized treatment strategies, with remarkable antitumor effects. We report two cases of successful management of MSI-H OCC, achieving curative resection while avoiding decompressive procedures, including colostomy creation. CASE PRESENTATION: Case 1: A 29-year-old man diagnosed with MSI-H obstructive transverse colon cancer (cT4b stomach, N1b, M1c1) was treated with pembrolizumab monotherapy (200 mg/body, every 3 weeks). The colorectal obstructive scoring system score was 2 at pembrolizumab administration. The patient showed rapid improvement in his abdominal symptoms within 3 days and achieved clinical complete response after eight courses. Laparoscopic partial colectomy with D3 lymph node dissection was subsequently performed safely. He was discharged on postoperative day 8 without postoperative complications. Histopathological analysis confirmed pathological complete response, and the patient was recurrence-free 15 months after surgery without adjuvant chemotherapy. Case 2: A 58-year-old man diagnosed with MSI-H obstructive ascending colon cancer (cT4aN3M1a, LYM) was treated with pembrolizumab monotherapy. The colorectal obstructive scoring system score was 1 at pembrolizumab administration. The patient's abdominal symptoms improved within 5 days, with marked tumor shrinkage after nine courses. Laparoscopic extended right hemi-colectomy with D3 lymph node dissection was subsequently performed safely, and he was discharged on postoperative day 7 without postoperative complications. Histopathological analysis showed major pathological response (less than 10% viable cancer cells in the resected specimen), with no viable tumor cells in the primary lesion. The patient was recurrence-free 1-year post-surgery, without adjuvant chemotherapy. CONCLUSIONS: This report highlights the potential benefits of ICI treatment for dMMR/MSI-H OCC, particularly for rapid relief of obstruction-related symptoms and facilitating oncologically safe R0 resection. In cases of MSI-H OCC, ICIs can be highly effective as an alternative to traditional decompression procedures.

    消化器外科臨床実習においてVirtual Reality(VR)技術を活用したコンテンツの作成を試みた。今回、腹腔鏡下胆嚢摘出術を題材に見学者目線のコンテンツを作成し、実際に体験した医学生にアンケート調査を行った結果、26名の医学生全員より「満足である」との回答、25名から「VR手術見学は臨床実習の代替えになると思う」との回答が得られた。また、アンケート結果から、VR手術見学は消化器外科臨床実習の授業内容を補足するだけではなく、学習者の学習意欲を高める手段として有用であり、更に臨床実習を予習するための教材として効果的であることが示唆された。

    Prevention of postoperative anastomotic leakage in rectal surgery is still required. This study investigated the feasibility of endoscopic hand suturing (EHS) on rectal anastomosis ex vivo. By using isolated porcine colon, we prepared ten anastomoses 6-10 cm from the virtual anus. Then, we sutured anastomoses intraluminally by EHS, which involved a continuous suturing method in 5 cases and a nodule suturing method with extra corporeal ligation in 5 cases. Completeness of suturing, number of stitches, procedure time and presence of stenosis were investigated. Furthermore, the degree of stenosis was compared between the two suturing methods. In all cases, EHS were successfully completed. The median number of stitches and procedure time was 8 and 5.8 min, respectively. Stenosis was created in all continuous suturing cases whereas none was seen in nodule suturing cases. The shortening rate was significantly greater in the continuous suturing method than in the nodule suturing method. Intraluminal reinforcement of rectal anastomosis by EHS using nodule suturing with extra corporeal ligation is feasible without stenosis, which may be helpful as a countermeasure against possible postoperative anastomotic leakage in rectal surgery.

    症例は60歳代男性で、4年前に下咽頭癌に対し咽喉頭頸部食道摘出術+両側頸部郭清+甲状腺右葉切除術+遊離空腸再建術を行い、病理診断はSquamous cell carcinoma(SCC)、pT2N2bM0 pStage IVbで外来経過観察中であった。上部消化管内視鏡(EGD)で上切歯列(DL)27cmの食道内に0-IIb病変を認め、内視鏡的粘膜下層剥離術(ESD)を施行し化学放射線療法を追加した。7年後のEGDにてDL35cmの食道内に粘膜下層進展病変を認め、生検にてSCC、cT1bN0M0、cStage Iの診断で、光線力学療法(PDT)を施行した。PDT治療後、照射部位にバルーン拡張を要し、さらに経過観察中2回の食道表在性新規病変を認め各々ESDを施行し、病理結果はいずれもSCC、pT1a-LPM、ly0、v0、pHM0、pVM0、pCur Aであった。初回の食道癌治療から10年経過現在、胸部・腹部CTにて再発やリンパ節転移、遠隔転移を認めることなく経過観察中である。
