＜文献概要＞はじめに 腱滑膜巨細胞腫は手に発生する軟部腫瘤のなかではガングリオンに次いで多い.腱滑膜巨細胞腫の手術に際して,指神経と指動静脈が皮切の直下に存在したり,腫瘍が組織間隙に入り込むように発育していたり,あるいは,靱帯や腱などに浸潤していたりなど,その局所浸潤型の発育様式に戸惑うことがある.腱滑膜巨細胞腫はより慎重な対応が必要な良性腫瘍の一つであり,手術においては,正確な診断,適切な準備,慎重な手術手技の三つをしっかり行うことが重要である.腱滑膜巨細胞腫が腫瘍性疾患なのか炎症性疾患なのか議論があるが,近年,腱滑膜巨細胞腫の63〜77%でCSF-1遺伝子の発現上昇が認められ,腱滑膜巨細胞腫はCSF-1遺伝子を異常発現している滑膜細胞由来の腫瘍性疾患とする説が有力である.CSF-1は多くの組織球を引き寄せるため,腱滑膜巨細胞腫内には組織球の集簇が認められる.腱鞘滑膜に発生することが多く,関節滑膜の発生は8〜19%と少ない.また,手指には単一の腫瘍塊を形成する限局型がほとんどであり,びまん型はまれである.腱滑膜巨細胞腫の発育様式は浸潤性である.つまり,被膜を有するものの隣接組織に容易に癒着したり浸潤したり,被膜がありながら被膜外に発育することがある.手指例でも数%は被膜をもたず発育する.また,浸潤性の発育様式のために,手術の際に腫瘍の取り残しが生じやすく再発率は2〜27%と高率である.報告されている再発因子は,母指指節間(IP)関節・その他の手指の遠位指節間(DIP)関節部発生,隣接関節の変形性関節症変化,腱鞘や関節内への浸潤など取り残しを生じやすい解剖学的因子であり,再発に影響を与える腫瘍の内的因子については確定したものはない.自験例においても,MIB-1抗体を用いた免疫組織化学的検索では再発と細胞増殖能との関連は見出せなかった.現在のところ手術が唯一の標準的治療である.なお,CSF-1阻害薬の有効性が報告されているが,わが国では保険適用となっていない.

We present a case of solitary chondrosarcoma arising from the proximal phalanx of the ring finger in an elderly man. The chondrosarcoma developed over a period of 14 years, during which the phalanx became progressively more deformed. Several radiographic investigations were carried out, but the patient declined further suggested diagnostic examinations (computed tomography, magnetic resonance imaging, biopsy). Eventually, the lesion became significantly enlarged, and radiographs showed osteolytic lesions in the phalangeal bone. Ray amputation of the finger was required to establish a wide resection of the chondrosarcoma. Most osteochondral tumors arising from the phalanges are benign tumors such as enchondromas, but primary chondrogenic malignant bone tumors (chondrosarcomas) occasionally occur. Chondrosarcoma of the phalanx is difficult to distinguish from enchondroma of the phalanx, because histological investigations of the two neoplasms often produce similar findings. Even with a combination of clinical, biopsy, and imaging findings, differentiating these neoplasms is still challenging, because the characteristic clinical and radiological features of chondrosarcoma do not appear until it becomes aggressive and starts to cause destructive changes. Once that happens, radical expanded resection of the tumor is essential. Therefore, longstanding enchondroma-like lesions should be actively treated in elderly patients, even if a definite diagnosis of chondrosarcoma cannot be made.

50歳女性。右大腿後面腫瘤を主訴に近医を受診し、軟部腫瘍が疑われ、当科へ紹介となった。初診時、右大腿後面に長径38cm、境界明瞭、弾性軟の巨大な腫瘤が認められた。画像所見および針生検にて本症例は坐骨神経を巻き込んだ巨大な大腿部異型脂肪腫様腫瘍と診断され、In situ preparation法を併用して腫瘍切除術が行われた。その結果、病理組織学的に硬化型異型脂肪腫様腫瘍であり、術後4年4ヵ月経過現在、再発や転移は認めず、歩容にも異常はみられていない。

66歳女性。右示指の屈曲障害を主訴に当科へ紹介となった。初診時、右手掌中央部に2cm大の可動性と軽度の圧痛を有する腫瘤がみられ、MRIでは水平断にて示指の屈筋腱に接する2.0×1.5cm大の卵円形腫瘤が認められた。また、T1強調像では均一に筋組織とほぼ等信号、T2強調像で不均一な軽度高信号を示し、中央部は高信号を呈していた。更に矢状断では腫瘤の遠位および近位部分に反応層が認められたが、細胞診では悪性所見は確認されなかった。以上、これらの所見を踏まえて、本症例は結節性筋膜炎や粘液腫などが疑われ、腫瘤摘出術が施行された。その結果、病理組織学的に虫様筋結節性筋膜炎と確定診断され、術直後より示指自動完全屈曲が可能となった。

＜文献概要＞はじめに わが国の癌の年間罹患数が100万人を超えるようになり,また分子標的治療薬の登場によりこれまで予後不良であった癌の生存期間が伸長していることで骨転移患者数は増加している.患者の多くは癌や骨転移と共存し,薬物治療を続け,経過によっては治験治療を受けながら,闘病生活を継続している人が多い.そのような患者に骨転移による病的骨折や麻痺が生じてしまった場合には,日常生活動作は極端に悪化し,またパフォーマンスステイタスの低下により薬物治療の継続に支障が生じる.近年,骨転移の治療は集学化がすすんでおり,骨転移の患者には,化学療法,手術的治療,放射線療法のみならず,早期緩和ケア,早期リハビリテーション,精神的ケア,歯科医,看護師,栄養サポート,退院・転院・就労支援など多面的に多くの職種が携わっている.整形外科医は骨の専門家であることからその主要なメンバーであり,託されている役割は骨転移の診断,生検,手術,治療方針(装具,手術,放射線治療,骨修飾薬など)の提案,安静度,荷重制限,病的骨折・麻痺リスク評価,リハビリテーション前評価,装具作成指示,経過観察など多岐にわたる.しかし,整形外科医は骨転移診療の集学化にやや乗り遅れているようにもみえる.われわれは骨転移診療に対して2014年から積極的な取り組みをはじめ他科からの紹介患者が約2倍になった経験をもとに,骨転移診療を積極的対応に転換する際の留意点を3ステップに分けて述べる.

＜文献概要＞はじめに 穿刺吸引細胞診(fine needle aspiration cytology:FNAC)は,乳癌や甲状腺癌など表在にある腫瘍では以前より広く行われており,その診断学的価値は生検に匹敵するとされている.軟部腫瘍も表在腫瘍の一つであり,軟部腫瘍に対するFNACは欧米において広く行われているが,わが国では十分普及していると言い難い.その理由は軟部腫瘍の発生母地が多岐にわたりその種類も膨大であり,かつ良悪性の境界病変もあるため,病理組織診断そのものが難解であるからである.われわれは約30年にわたって使用し,軟部腫瘍のマネジメントにおける有用性を実感している.本稿では軟部腫瘍に対するFNACの必要性,目的,特徴,MRIとの相補性,実際の方法について述べる.

＜文献概要＞はじめに 整形外科外来には,さまざまな痛みを理由に患者が受診する.痛みの原因の一つに腫瘤性病変がある.多くは足底の粉瘤や末梢神経近傍に生じたガングリオンなど周囲組織との関係が痛みの発生に関与している場合が多いが,中には腫瘤性病変そのものが痛みの原因となっている場合がある.いわゆる有痛性軟部腫瘍は発生部位に関係なく痛みを生じることがある.しかし,下肢の皮下に好発する小腫瘤である血管平滑筋腫や上腕皮下に触れる小さな脂肪腫のような腫瘤(血管脂肪腫)が疼痛の原因になりうることはあまり知られていない.また,尺骨神経など太い神経幹に発生する神経鞘腫や爪下腫瘍としてのグロムス腫瘍はよく知られているが,太い神経幹とは無関係な神経鞘腫や爪下以外に発生するグロムス腫瘍の存在はあまり知られていない.そのため,痛みの原因をほかに求めて過剰な検査を行ったり,患者が痛みを主張することに対して懐疑的になり腫瘍が治療されずに放置されたりすることもある.これらの腫瘍の治療は単純な摘出で十分なので,整形外科医が有痛性軟部腫瘍について知ることが重要である.本稿では,有痛性軟部腫瘍のうち,特にその小ささに不釣り合いな痛みを伴うことが多い血管平滑筋腫,血管脂肪腫,グロムス腫瘍の診断について述べる.

This review examines the following aspects of tenosynovial giant cell tumors (TSGCTs): the use of multiple names, the complex relationship between tumor growth pattern and location, the high rate of postoperative recurrence, local invasiveness, use of nonsurgical therapy with molecularly targeted drugs, and best current treatments. This tumor has been referred to by various names, but is now most frequently referred to as TSGCT. TSGCT is classified as localized and diffuse, in accordance with its growth characteristics. Most TSGCTs of the fingers are localized. TSGCT is likely a neoplastic process arising from synovial lining cells, in which tumor cells express the colony stimulating factor 1 (CSF1) gene. The postoperative recurrence rate of TSGCT is approximately 15%. The intrinsic characteristics of recurrence are not clear, and complete resection of the lesion is still the treatment mainstay. Moreover, TSGCT commonly grows out of a pseudocapsule. Therefore, to perform complete resection of TSGCT, surgery must be performed cautiously after appropriate preparation, by using anesthesia, a tourniquet, surgical loupe, and surgical microscopy. After accurate preoperative diagnosis, meticulous planning by surgeons is necessary. The lesion should be resected along with approximately 1-mm of healthy tissue at the adhesion site. In addition, because satellite lesions might be present near the tumor, careful dissection and observation of the color of surrounding tissue are important. International clinical trials of CSF1 receptor inhibitors for TSGCT treatment are ongoing.

BACKGROUND: Magnetic resonance imaging (MRI) and fine-needle aspiration cytology (FNAC) are useful in the diagnosis of soft-tissue tumors and can be performed on outpatients. These modalities are complementary: MRI examines a large area, while FNAC assesses a highly specific region; MRI displays only signal intensities, while FNAC visualizes actual tumor cells. We investigated the combined use of these methods for differentiating malignant and benign tumors. METHODS: 148 patients (153 lesions: 137 benign, 16 malignant) underwent preoperative MRI and FNAC. A diagnosis was judged to be correct if one or both diagnoses were correct, incorrect if at least one diagnosis was incorrect, and indeterminate if both diagnoses were indeterminate or if MRI was indeterminate and the FNAC sample was insufficient. RESULTS: The diagnostic yields for MRI only, FNAC only, and their combination were 81.7%, 84.3%, and 92.2%, respectively, indicating that the diagnostic performance of MRI and FNAC was significantly improved when the methods were combined. CONCLUSIONS: As compared with either modality used alone, combined preoperative use of MRI and FNAC improved diagnosis of soft-tissue tumors.

BACKGROUND: Angioleiomyomas are usually small and relatively rare painful soft tissue tumors, and are usually less than 2 cm in diameter. Magnetic resonance (MR) imaging features of typical angioleiomyomas are not well understood, and their association with histologic subtypes is not clear. In the present study, MR images of average sized angioleiomyomas were compared with regard to specific histologic subtypes. METHODS: We performed a retrospective review of medical records, MR imaging data, and histologic specimens of 18 consecutive patients with angioleiomyomas that were extirpated at our hospital between January 2006 and December 2013. RESULTS: Most of the lesions showed homogenous iso-signal intensities compared to skeletal muscle on T1-weighted images; however, on T2-weighted images, solid and venous types showed heterogenous and iso- to slightly high-signal intensity, while the cavernous type showed high-signal intensity. Most of the lesions had a low-signal intensity rim, and two thirds of them had adjacent vessels. CONCLUSIONS: Our results suggested that MR findings of angioleiomyoma vary depending on histologic subtypes. T2-weighted images of solid and venous types of angioleiomyoma show specific MR findings which allow for differentiation from other soft tissue tumors such as soft tissue sarcomas. These mostly show iso- to slightly high-signal intensity compared to skeletal muscle, while the cavernous type show non-specific findings. Thus, solid and venous types of angioleiomyoma could be diagnosed preoperatively via clinical findings and MR imaging.

BACKGROUND: Cancer diagnoses are increasing rapidly due to the aging population of Japan. Pedicle sign is a valuable radiographic indicator of metastases as the pedicle is the most commonly affected part of the vertebra upon radiographic assessment. However, few detailed studies examining the morphological features of pedicle signs have been performed. To improve the capacity of medical professionals to diagnose symptomatic spinal metastases, we retrospectively examined the morphological characteristics of pedicle signs and their relationship with clinical and radiological features. METHODS: In total, 186 patients with symptomatic spinal metastases who visited our department between January 1, 2011, and December 31, 2017, were enrolled in the study. Pedicle sign was defined as a pedicle that had disappeared or had become obscured on an anteroposterior radiographic image. Radiographic images were evaluated for pedicles and other structures of the vertebrae. Clinical and other radiological features were compared among the types of pedicle signs identified. RESULTS: Pedicle signs were classified according to whether they had completely disappeared (complete type, 26 patients), partially disappeared (partial type, 40 patients), or were obscured by the osteoblastic background (blastic type, 28 patients). The disappearance of the bone cortex in addition to the pedicle was observed in almost half of the patients with complete and partial types of pedicle signs. Complete types were associated with significantly longer survival rates. CONCLUSION: Understanding of the various types of pedicle sign is required to properly diagnose bone metastases.

BACKGROUND: Although knowledge of the natural history of retroperitoneal schwannoma, including growth rate, would be useful when determining the indications and timing of surgical treatment for such nonpalpable tumors, the existing evidence is unclear. We examined the natural history of retroperitoneal schwannoma and assessed whether magnetic resonance imaging (MRI) and clinical findings predict future growth. METHODS: Among patients with retroperitoneal schwannoma treated in our department during the period from January 1, 2015 through December 31, 2015, eight who underwent follow-up assessment by MRI and did not undergo tumor resection for longer than 1 year were enrolled. Changes in lesion size were examined in relation to selected clinical and MRI findings. In cases of pressure erosion in the vertebral body, change in the size of the intraosseous region was compared to change in the size of the whole lesion. RESULTS: Median absolute growth rate (AGR) was 1.9 cm3 per year, median relative growth rate (RGR) was 5.6% per year, and median tumor volume doubling time (VDT) was 3.1 years. AGR, RGR, and VDT were not associated with any clinical variable. RGR and VDT values in the intraosseous region were about twice those of the whole lesions. CONCLUSIONS: The growth rates of retroperitoneal schwannomas varied. Additionally, no MRI or clinical findings predicted growth of retroperitoneal schwannoma. Therefore, careful follow-up is necessary for this tumor type, especially for lesions with bone erosion.

BACKGROUND: The rapid aging of the Japanese population is leading to an increase in the number of patients with bone metastases. Since 2014, our orthopedics department has promoted multidisciplinary hospital activities, including offering lectures to hospital staff on multidisciplinary approaches for bone metastases and holding regular cancer board meetings on bone metastases. This study investigated whether these activities were effective in promoting multidisciplinary approaches and improving outcomes of patients with bone metastasis. METHODS: To investigate the effects of changes in medical practice on patients with bone metastases, we compared patient clinical characteristics after (January 2014 through December 2017) and before the start of the activities (January 2011 through December 2013). RESULTS: The semiannual numbers of first-visit, referral, and orthopedic surgical patients, the number of patients with slower growing primary cancers, and the number of patients with milder pain were significantly higher post-activity than pre-activity. The number of patients without paralysis was higher after the start of the activity than before the activity, but the difference was not significant. Survival after the first visit to the orthopedics department was significantly longer after the start of the activity than before the activity. CONCLUSIONS: The potential demand for a multidisciplinary approach to bone metastases is high, and orthopedic specialists should actively participate in this approach.

Our previous studies showed that early diagnosis of painful bone metastasis is difficult and requires improvement in the diagnostic accuracy of plain radiography during an initial patient consultation. In this preliminary study, we evaluate the usefulness of educational material used to improve diagnosis of bone metastasis with plain radiography. This study included imaging data from 129 consecutive patients who visited our orthopedic clinic during the period January 2011 through December 2014. First, we prepared a test to measure the reading ability of orthopedic practitioners, after which the educational material was created. Then, the effectiveness of the educational material was verified by having orthopedic trainees take a pre-test and post-test. The test contained plain radiographic data from 12 patients with lesions and 6 without lesions. The educational material included plain radiographic data from 30 patients with typical findings of bone metastasis, as well as diagnostic magnetic resonance images or computed tomography scans, accompanied by a lecture. The accuracy and sensitivity of diagnosis significantly improved after the lecture; however, specificity decreased. Although the educational material was effective for improving the ability of orthopedic trainees to read plain radiographs of bone metastasis, some aspects of the program need to be improved and revised.

背景:脊索腫は、仙骨や頭蓋底など軸骨格を中心に発症する比較的まれな腫瘍であり、組織学的に確定診断にいたるには担空胞細胞の同定が重要な手掛かりとなる。われわれは、術前細胞診で担空胞細胞を認めず、診断に苦慮した脊索腫の1例を報告する。症例:69歳、女性。転倒後の仙骨部領域の持続的な疼痛を訴え、当院を受診。骨盤部レントゲン、MRIで、仙骨領域に分葉化する腫瘤を認めた。穿刺吸引細胞診では、多量の粘液物質内に多形性や核異型を有する紡錘形細胞の増生を認めたが、特徴的な担空胞細胞は同定できず、軟骨肉腫を第一に考えた。その後、生検、手術検体で、好酸性の胞体をもつ上皮様異型細胞の増生を認め、脊索腫と確定診断された。結論:脊索腫の細胞診では、担空胞細胞が同定できない場合、診断が困難となる。臨床、画像的に脊索腫が示唆される場合、典型的な細胞像を欠いても、脊索腫の可能性があることを念頭におく必要がある。(著者抄録)

BACKGROUND: Early diagnosis of bone metastasis is difficult. The aim of the present study was to determine whether symptoms related to skeletal-related events (SREs) can be used for the diagnosis of bone metastasis in the absence of screening tests. METHODS: We reviewed 81 patients with bone metastasis to evaluate their SREs at diagnosis. SREs were arbitrarily classified as moderate or severe. Moderate SREs included radiation to the bone before pathological fracture or paralysis, bone surgery before pathological fracture or paralysis, and hypercalcemia without dialysis. Severe SREs included pathological fracture, spinal cord compression, and hypercalcemia necessitating dialysis. RESULTS: The complication rates of SREs at the time the bone metastasis was diagnosed were 59.3% and 24.7% for severe and moderate SREs, respectively, and only 16.0% of cases were uncomplicated. The clinical factors that showed a significant relationship with the severity of SREs were age and history of malignancy. However, there was no significant relationship between the complication rate of total SREs and the presence or absence of a malignancy history (83.3% vs. 85.2%, respectively, p=0.83). CONCLUSION: The results of the present study suggest that symptoms related to SREs can be used to diagnose bone metastasis in the absence of a screening test. Bone metastasis should be diagnosed as often as possible based on symptoms related to moderate SREs and should be treated as soon as possible before patients develop severe SREs.

BACKGROUND: Diagnosing bone metastasis in patients without a history of cancer remains challenging. Diagnostic evaluation may be prolonged owing to difficulties in distinguishing between bone metastasis and common orthopedic diseases. We hypothesized that bone metastasis due to occult cancer would be more difficult to diagnose than bone metastasis in patients with a history of cancer. Few studies exist on the difficulty of diagnosing bone metastasis in patients without a history of cancer. Therefore, we reviewed the clinical course of patients with bone metastasis between January 2011 and December 2014. METHODS: We reviewed patients with bone metastasis to determine the diagnostic rate at first visit, period from symptom-onset to first visit, period from first visit to diagnosis, and presence of severe skeletal-related events at diagnosis, and compared these between 27 patients without a history of cancer (Group A) and 54 patients with a history of cancer (Group B). RESULTS: The diagnostic rate at first visit was significantly lower (11.5% vs. 52.4%, p=0.00069), the period from first visit to diagnosis was significantly longer (median, 7 weeks vs. 3 weeks, p=0.018), and the presence of severe skeletal-related events at diagnosis was significantly higher (81.4% vs. 50.0%, p=0.05) in patients without a history of cancer compared with those with a history of cancer. CONCLUSIONS: The results of this study indicate that it is difficult to diagnose bone metastasis in patients without a history of cancer. This must be considered in the early diagnosis of bone metastasis to prevent severe skeletal-related events.

症例は6ヵ月男児、3ヵ月検診で右腓腹部の皮下腫瘤を指摘された。MRIで脂肪腫様所見を認め経過観察も増大傾向が持続するため摘出し、病理組織検査にてfibrous hamartoma of infancyと診断した。fibrous hamartoma of infancyのMRI像は、組織学的特徴である線維組織、未分化間葉系細胞巣、成熟脂肪組織の3要素の割合、局在を反映する。自験例では成熟脂肪組織の割合が高くMRIも脂肪腫様を呈した。(著者抄録)

BACKGROUND: To prevent and minimize skeletal-related diseases, early diagnosis of bone metastases is important. However, previous reports have shown that plain radiography has low sensitivity and fails to screen multiple asymptomatic lesions. Limited investigations have been reported on the value of plain radiography in the diagnosis of symptomatic bone metastases. Therefore, this study aimed to investigate the diagnostic utility of plain radiography for symptomatic bone metastasis. METHODS: Two experienced orthopedic surgeons retrospectively evaluated the plain radiographs of 39 patients with symptoms during their first visit between 2011 and 2014 for bone metastases. Another 2 experienced orthopedic surgeons then reviewed the data using 2 reference standards, the clinical results and the retrospectively evaluated results, in a blinded manner. The data were then reviewed by 2 certified orthopedic surgeons and 7 orthopedic surgeons in training with differing years of experience in a blinded manner. RESULTS: The overall sensitivity of diagnosis of symptomatic bone metastasis using plain radiography at the clinic first visit was 71.4%. Upon blinded evaluation, the accuracy, sensitivity, and specificity were 55.8%, 54.3%, and 68.8% and 77.6%, 73.0%, and 85.7% for clinical results and results from 2 experienced orthopedic surgeons as a reference standard, retrospectively. There was a strong and significant correlation between the accuracy and observers' years of experience in orthopedic surgery among the orthopedic surgeons in training (R=0.942, p=0.0015). CONCLUSIONS: Plain radiography around the time of the first visit has a definitive role in the early diagnosis of symptomatic bone metastasis.

BACKGROUND: Many patients with bone metastases whose diagnoses came only after they had suffered aggravated conditions are still frequently encountered. However, there have been few studies regarding the early diagnosis of such metastases. We retrospectively reviewed the clinical courses of cases we experienced between 2004 and 2014 to clarify the practical situation of diagnosis of such bone metastases. METHODS: We undertook a retrospective review of 56 of our patients with bone metastasis who had no history of malignancy at their first visit, who visited our departments between 2004 and 2014. The initial diagnoses at the first visit to any clinic, the period from the first visit to any clinic to diagnosis of bone metastasis, the process to make the diagnosis, the frequency of severe skeletal-related events at diagnosis, and the examination serving as the basis for diagnosis were evaluated. RESULTS: The diagnosis of bone metastasis was made at the first visit in only 6 of the 56 patients. Pathological fractures, paralysis and/or calcemia were seen in 62.5% of the patients at diagnosis of bone metastasis. The median period from the first visit to any clinic to diagnosis was 7.0 weeks. Typically, the diagnosis of bone metastasis was made only after aggravation. The most frequent examination to serve as the basis of diagnosis was magnetic resonance imaging. CONCLUSIONS: Diagnosis of bone metastasis is challenging in patients without a history of malignancy at their first visit. For early diagnosis, it is important to recognize this challenge and to keep it in mind together with ongoing observation.

47歳男性。32歳時に右中指の腫瘤を自覚し、37歳時に著者らの施設へ初診となった。受診後10日で軟部腫瘍の診断のもと腫瘍切除術が施行され、病理組織学的に限局型腱滑膜巨細胞腫の診断に至った。その後、47歳時に左示指に同様の腫瘤を自覚し再診、受診後14日で軟部腫瘍の診断にて腫瘍切除術が施行された。その結果、前回と同じく病理学的に限局型腱滑膜巨細胞腫であった。尚、本症例を含めて、過去に報告された限局型腱滑膜巨細胞腫症例の文献的検索を行ったが、本腫瘍が多発する要因を見い出すことはできなかった。

小皮切内視鏡確認掻爬術を施行した手指骨の内軟骨腫13例の治療経験について後ろ向きに検討した.術後X線像上十分な骨強度が得られたと思われる新生骨形成時期は中央値2.5ヵ月(1〜4ヵ月)であり,また,再発はみられなかった.しかし,作成した骨窓(3mm径または5mm径)は術中操作によりその長径が中央値7mm(4〜11mm)と拡大していた.骨窓径を5mmとした症例(6例)は3mmとした症例(7例)と比べ術後の骨窓の拡大はほぼ同等であることが推測された.3mm径の1例に術中骨折を認めた.骨窓径を5mmとやや大きめにすることは操作性及び安全性の面から有効であることが示唆された.小皮切内視鏡確認掻爬術は皮切が小さく軟部組織の損傷も少ないが,小さな骨窓を通しての掻爬操作のため骨窓の意図しない拡大が生じやすいので慎重な手術操作が必要である.(著者抄録)

PURPOSE: Despite producing pain, angiolipoma is sometimes misdiagnosed as an ordinary small lipoma, which is usually not associated with pain. Few reports have described magnetic resonance (MR) imaging findings of angiolipoma. The aim of the present study was to clarify the MR imaging features of angiolipoma. MATERIALS AND METHODS: The MR imaging findings of 11 lesions in 7 patients were reviewed and compared with histopathological findings. RESULTS: The MR imaging features of these lesions were the presence of fat nodules with or without areas of low signal intensity on T1- and T2-weighted images. The location of the low-signal-intensity areas varied. The low-signal-intensity areas were mainly in the peripheral portion of 3 lesions and in the central portion of 5 lesions. No or few low-signal-intensity areas were observed in 3 lesions. In the lesions with peripheral low-signal-intensity areas, lesion marginations were well defined, and the lesions were easily recognized as mass lesions. In the lesions with only central low-signal-intensity areas, marginations were poorly defined, and the lesions were not easy to recognize as mass lesions. In the lesions with few or without low-signal-intensity areas, marginations were invisible. Histopathological studies indicated that the low-signal-intensity areas on T1- and T2-weighted images corresponded to areas of dense capillary proliferation. In lesions with few or without low-signal-intensity areas on MR images, capillaries were thinly spread over almost the entire lesion area. CONCLUSION: The MR imaging features of angiolipoma are fat nodules with or without low-signal-intensity areas of various size and location on T1- and T2-weighted images.

痛みは軟部腫瘍の数少ない臨床所見の一つであり臨床診断上重要である.6年間に経験した軟部腫瘍および腫瘍類似病変199病変を対象に痛みとの関連について検討した.手術症例では約4割の患者は痛みを有した.肘から手指においては肩から上腕に比べ有痛性病変の比率が有意に高かった.有痛性病変は非有痛性病変に比べ有意に小さかった.有痛性病変には,神経幹と関連する病変,いわゆる有痛性腫瘍,炎症性疾患,巨大で周囲を圧迫する病変,刺激を受けやすい部位の病変などが含まれていた.放散痛がある場合,約3分の2は神経鞘腫であるが残りはガングリオンなどによる神経圧迫病変であった.巨大で周囲を圧迫する病変では悪性腫瘍の可能性が高かった.痛みを軟部腫瘍の診断に生かすためには,上記のような軟部腫瘍の疼痛の特徴を総合的に理解することに加え,血管平滑筋腫,血管腫,血管脂肪腫,グロームス腫瘍などいわゆる有痛性腫瘍の個々の病態理解が重要である.(著者抄録)

To clarify the magnetic resonance (MR) features of the pure form of intravascular papillary endothelial hyperplasia, MR images (MRIs) from five patients were retrospectively reviewed and compared with histological findings. The images showed a heterogeneous, iso- to slightly high signal intensity mass on T1-weighted images and a mass with a central heterogeneous, iso- to slightly high signal intensity area completely or incompletely surrounded by peripheral high signal intensity areas on T2-weighted images. Heterogeneous enhancement was observed after gadolinium administration. Histological studies indicated that the central heterogeneous area on T2-weighted images corresponded to thrombi (organized and/or hyalinized) and/or papillary endothelial proliferation, and also that the peripheral high signal intensity area corresponded to vascular blood space and/or papillary endothelial proliferation. The pure form of intravascular papillary endothelial hyperplasia showed relatively characteristic features on MRIs.

指に発生した血管内乳頭状内皮過形成(IPEH)の治療経験について報告した。症例は5例で、発生指は母指1例、示指1例、環指3例であった。病変の自覚から受診までの期間の中央値は8ヵ月で比較的長期であった。病変の長径の中央値は8mmであった。いずれも皮膚から青色に透見された。5例で圧痛、3例で安静時痛を有した。MRIは全例に施行し、T1強調像で不均一で筋肉に比べ等信号からやや高信号を示し、T2強調像で中心部に等信号からやや高信号をまた周辺に高信号を示す傾向を示した。穿刺吸引細胞診は3例に施行し、血液を背景に紡錘形細胞が散在性に、また、無構造物も認め、異型性はなく良性病変と考えられたがIPEHの診断には至らなかった。術前診断は血管腫または血腫であった。全例摘出手術を施行した。4例は皮下に存在した。1例は滑膜性腱鞘の下に存在し腱と一部癒着していた。平均経過観察期間21ヵ月で再発の兆候は認めていない。

2005年1月〜2011年4月までの間に当科で入院加療を行った大腿骨近位部骨折患者187例のうち両側骨折群27例をretrospectiveに検討した。両側骨折は片側群に比べて高齢、低身長、低体重、低BMI、女性患者に多く発症していた。骨折間隔が短い症例では認知症患者の割合が有意に高かった。施設入所者の割合も高い傾向にあったが有意差を認めなかった。再骨折までの期間は中央値が36ヵ月であり、1年以内(30%)の発症が一番多かった。原因は転倒(93%)が一番多く、受傷場所は自宅・施設が多かった。1年以内死亡は3例(15%)であり、機能予後は11例(40%)で歩行能力の低下を認めた。再骨折前に実施されていた骨折予防は骨粗鬆症治療薬使用の3例(11%)のみであった。骨折の予防がなされていない症例が多く、我々整形外科医は骨折治療のみならず骨折予防にもっと配慮する必要がある。(著者抄録)

The purpose of this study was to clarify clinical and radiographic features of bone changes associated with soft-tissue tumors of the hand. We reviewed clinical records and radiographs of 115 patients who had undergone operations for soft-tissue tumors or tumorous conditions of the hand. Bone changes were detected in the radiographs of 21 of the 115 patients. Giant cell tumor of tendon sheath was the most common histological type to be associated with bone changes. The most vulnerable part of the hand was the palmar side of the phalanx from the distal shaft to the head. Most of the bone changes were erosion with clear margins. The erosions tended to extend deeper into the bone with limited widening. "Steep deep" erosions were found in 5 patients; these erosions did not require reconstruction after surgical curettage, and tumors did not recur.

We report on a patient with an unusual, slowly enlarging hematoma of the left buttock. A 62-year-old man presented with a 6-year history of an enlarging mass of the left buttock. He had first noted the mass 6 years earlier and had had sciatica of the left lower limb for the last 2 months. He denied any history of antecedent trauma. The lesion extended over 3 gluteal muscles (the gluteus medius, gluteus minimus, and piriformis). On microscopic examination, the lesion showed typical signs of chronic expanding hematoma. The sciatica was relieved after surgical removal of the lesion. The lesion had not recurred at the last follow-up 4 years after the operation. The present case suggested that chronic expanding hematoma can extend into multiple muscles due, perhaps, to long-term growth and the anatomical and functional conditions of the affected muscles. Our case also suggests that chronic expanding hematoma can be a cause of piriformis syndrome.

Lipoma of the finger is rare, and there have been few case reports of lipoma causing bone erosion in the finger. We report on a patient with a long-standing lipoma that caused deep erosion of the distal phalanx of the index finger. A 38-year-old man presented with a painful mass of the left index finger. Anteroposterior radiographic images showed a radiolucent area in slightly distal to the center of the distal phalanx. Magnetic resonance imaging revealed a large soft-tissue mass with high signal intensities and slight irregularity on both T1- and T2-weighted images which suggested lipoma. The patient underwent marginal excision of this tumor and curettage of the bone surface of the erosion. The pain resolved after surgery. The diagnosis of lipoma was confirmed with postoperative histological examination. Lipoma should be included in the differential diagnoses of a finger tumor, even if radiography shows bone erosion.

大腿骨近位部骨折患者において周術期の心的リスクを検討した。50歳以上の大腿骨近位部新鮮骨折患者128例を対象とした。術前内科的合併症は113例に認め、循環器疾患が一番多く、心疾患は56例に認めた。治療は121例に手術、7例には保存的治療を実施した。入院後重大合併症は14例に発生し、敗血症、肺炎が多くみられ、各1例は30日以内に死亡した。受傷直後、周術期に既存の心疾患・心機能低下が著しく増悪した症例や新たに著しい心機能障害が発生した患者は皆無であった。ASA分類やEagle分類、術前エコー検査の有無で合併症の発生に有意差は認めなかった。早期(30日以内)死亡率は2.3%で、保存治療中に肺炎を併発した1例、末期食道癌で亡くなった1例と術後、肺炎、敗血症による1例の計3例であった。早期死亡原因は肺炎や癌死が多いとする文献と一致する結果であった。

In the diagnosis of soft tissue tumors, an incomplete biopsy for malignancy interferes with subsequent therapy. Therefore, we perform fine needle aspiration cytology (FNAC) for soft tissue tumors and consider the following four issues: 1) Is the tumor benign or malignant?; 2) In malignancy, is the tumor primary or metastatic? Furthermore, if possible; 3) In a primary malignant tumor, the histological type and degree of malignancy; 4) In a metastatic malignant tumor, the histological type and the primary site. In FNAC, we diagnose soft tissue tumors based on the pattern of cytological findings, such as small round cells, pleomorphic cells, spindle cells, and epithelioid cells, and furthermore based on the findings of the background, such as mucin, vessels, multinucleated giant cells, and inflammatory cells while referring to clinical information. In soft tissue tumors that are difficult to diagnose only by cytology, immunochemical staining for multiple antibodies using the cell block method and/or cell transfer method is useful to make a definitive diagnosis. If necessary, rapid cytological diagnosis is currently performed for soft tissue tumors in our department.

75歳女性。患者は転倒受傷後に右肩関節痛が出現し、著者らの施設へ受診となった。単純X線にて肩甲骨関節窩骨折を伴う肩関節前方脱臼と診断され、徒手整復を試みたが整復困難で、K-wireを経皮的に上腕骨より肩甲骨関節窩まで刺入し固定した。術後3週にてK-wireを抜去し、術後4週より可動域訓練を開始したものの、右関節痛が出現し、単純X線では上腕骨頭の軽度亜脱臼がみられた。一方、術後18週の血液検査ではCRPの軽度上昇が認められ、単純X線では上腕骨の変化が急速であったが、関節穿刺結果は陰性であった。その後、CTおよびMRI、関節鏡検査で上腕骨壊死と診断され、受傷後33週に人工骨頭置換術が行なわれた。置換術後1年現在、軽度疼痛の残存や右肩関節可動域の挙上が90度であるが、JOAスコアは術前30点から術後71点へ改善しており、ADL上の問題はみられていない。

骨膜性軟骨腫は比較的まれな良性骨腫瘍である。今回われわれは中手骨、中足骨に発生した骨膜性軟骨腫の2例を経験した。その2例に対して、腫瘍切除と骨皮質の掻爬を行い良好な結果を得た。骨膜性軟骨腫は健常骨皮質を含めて切除せずとも、腫瘍の切除と腫瘍底である骨皮質の掻爬で良好な結果が得られることが示唆された。(著者抄録)

PURPOSE: To review and compare the postoperative outcomes of 3 types of shoulder reconstructions: prosthetic arthroplasty, clavicula pro humero reconstruction, and allograft arthrodesis. METHODS: Records of 25 shoulder reconstructions following tumour resection were retrospectively reviewed. Perioperative data, oncological prognoses, postoperative complications, and functions were assessed. RESULTS: We performed 10 prosthetic arthroplasties, 7 clavicula pro humero procedures, 4 allograft arthrodeses, and 4 soft tissue reconstructions. Eight patients died of their diseases. Three developed complications and underwent revision surgery. 19 patients achieved stable shoulders. Ten patients attended for functional assessments. Respective mean scores using the Musculoskeletal Tumor Society-International Symposium on Limb Salvage and the Toronto Extremity Salvage Score were: 77% and 82% for prosthetic arthroplasties, 67% and 62% for clavicula pro humero procedures, 83% and 70% for allograft arthrodeses, and 93% and 98% for soft tissue reconstructions. CONCLUSION: A stable construct is the treatment goal for shoulder reconstructions, as it enables effective function of the arm and hand.

手関節以遠の手部発生の軟部腫瘍に対し術前に穿刺吸引細胞診を施行し、術中標本から組織学的に腱鞘巨細胞腫と診断された25例を対象として、穿刺吸引細胞診による術前診断の有用性を検討した。穿刺吸引細胞診は21ゲージ針、20mlシリンジ、千葉大一外科式吸引ピストルを使用して行い、全例で細胞採取可能で、24例が陰性(良性)、1例が判定困難であった。22例(88%)にて組織型の推定が可能で、組織像は単核間質細胞、泡沫細胞、多核巨細胞からなる特徴的なものであり、臨床像と併せて比較的容易に推定できた。また、穿刺吸引細胞診による合併症はみられなかった。穿刺吸引細胞診は腱鞘巨細胞腫の術前診断に有用であることが示唆された。

切除術を行った手部腱鞘巨細胞腫35例(男15例・女20例・平均40歳)の成績を報告した。手術では辺縁切除を原則とし、腫瘍との癒着がみられた腱鞘、関節包、滑膜、静脈などは合併切除した。また、穿刺吸引細胞診による術前診断と、MRIによる術前病巣範囲の検索を行った。初回手術が31例、他医での手術後再発が4例であった。罹患指は母指4例、示指12例、中指11例、環指5例、小指3例で、発生部位はDIPあるいは母指IP関節10例、中節部掌側6例、PIP関節11例、基節部掌側5例、MP関節3例であった。腫瘍の長径は0.5〜4.0cmで、骨変化19例、関節腔内への病変6例、腱鞘腔内への病変12例を認めた。術後24〜180ヵ月の経過観察で再発は2例に認め、再発までは術後7年、10年であった。両症例とも再手術を行ったが、1例は軽度の屈曲拘縮を残した。最終観察時の合併症としては、再発1例を含む計3例に関節可動域制限を認めた。

PURPOSE: To assess the clinical results of pelvic reconstruction using a saddle prosthesis following limb salvage operation for periacetabular tumour. METHODS: 12 patients with sarcoma and 4 with metastasis involving the pelvis were treated using the saddle prosthesis between 1995 and 2003 inclusive. Wide pelvic resection was performed for all patients with sarcoma and one with a metastatic lesion; intralesional excision of the acetabulum was undertaken for the remaining 3 patients with metastatic lesions. Oncologic prognosis, operation time, postoperative function, and complications were assessed retrospectively. RESULTS: Of the 12 patients with sarcomas, 5 were alive without evidence of recurrence after a mean follow-up of 37 months, one was alive with disease, 3 died of the disease, and 3 of other medical conditions. Respective mean postoperative functional scores according to the Musculoskeletal Tumor Society-International Symposium on Limb Salvage system and the Toronto Extremity Salvage Score were 53 and 64% in patients undergoing wide acetabular resection, and 30 and 42% in patients undergoing intralesional excision of the acetabulum. In patients undergoing wide acetabular resection and intralesional excision of the acetabulum, the mean operation times were 391 and 162 minutes respectively, whereas the mean times to ambulation were 7 and 4 days respectively. Major complications included infection and dislocation. CONCLUSION: Saddle prosthesis arthroplasty is a useful option for pelvic reconstruction following resection of acetabular malignancies. It is associated with a short operation time, rapid recovery, and moderately good postoperative function, but a relatively high risk of complications.

The role of angiogenesis as a prognostic indicator in cancer has been extensively studied in recent times with several studies demonstrating a positive correlation for various malignant tumours. However, the role of angiogenesis in osteosarcoma remains a topic of debate. In this study, we aim to evaluate the significance of intratumoural microvessel density (MVD) and the degree of vascular epithelial growth factor (VEGF) expression as markers of angiogenesis and correlate this with disease outcome. Archival paraffin-embedded pre-treatment biopsy tissue of patients treated at St. Vincent's Hospital, Melbourne, with non-metastatic osteosarcoma at initial diagnosis was reviewed. Tissue was processed for immunofluorescent staining of the microvascular endothelial cells with antibodies directed against CD31 and CD34. The degree of angiogenesis was assessed, as determined by the microvessel density (MVD). Further histological examination was performed to assess the degree of VEGF expression. Histological observations were correlated with various clinicopathological factors and patient outcome in terms of recurrence, metastasis and death. Twenty-five cases were reviewed, 15 were male and 10 were female, and the median age was 26 years (range, 13-85). The mean follow-up was 21.5 months (range, 3-75 months). The median MVD was 43 microvessels/0.26 mm2 (range, 25-54) and 46 microvessels/0.26 mm2 (range, 30-58) for CD31 and CD34, respectively. Despite the moderate to high vascularity, there was no significant difference noted between the MVD and disease outcome factors for both CD31 and CD34. There was a trend towards a higher MVD in patients aged > 40 years compared to those < 40 years (p = 0.110 for CD31 and p = 0.097 for CD34). In terms of VEGF expression, 24 of 25 cases demonstrated either moderate or strong expression; however, no prognostic significance was determined. In this study, we were able to demonstrate that osteosarcoma is a relatively vascular tumour; however, the degree of MVD and VEGF expression does not provide prognostic information. It is likely that angiogenesis plays a key role in the pathogenesis of osteosarcoma and is, therefore, a potential target for novel anti-angiogenic therapies.

BACKGROUND: Osteosarcoma predominantly afflicts young people in their second and third decades of life. When osteosarcoma arises in patients older than 40 years, the prognosis is usually poorer compared to their younger counterparts. Although the clinical, histopathologic features and prognostic indicators are well defined for young patients, much less is known about affected adults. The purpose of this study is to describe our institution's experience with the management of osteosarcoma in patients greater than 40 years and also evaluate, by immunohistochemical analysis, the prognostic significance of microvessel density, as a marker of intratumoural angiogenesis. METHODS: A retrospective clinicopathological analysis was performed on 11 patients over the age of 40 years that were treated at our institution between 1996 and 2004. Archival pre-treatment biopsy tissue was retrieved for immunohistochemical staining against two endothelial cell markers (CD31 and CD34) and also against VEGF. Angiogenesis was assessed by determining the intratumoural microvessel density (MVD) and the degree of VEGF expression in these specimens. This was correlated with patient outcome in terms of local recurrence, metastasis and death. Histological results were also compared to a group of patients less than 40 years of age. RESULTS: Of the 11 patients, 9 were male and 2 were female and the mean age was 58 years (range, 42-85). In 7 patients, osteosarcoma arose secondarily from Paget's disease of the bone. The most common site involved was the humerus (7) followed by the femur (2) then pelvis (1) and ulna (1). At the time of diagnosis, 4 patients had metastatic disease. Preoperative chemotherapy was given to 4 patients, with a good response in 3 patients. Six patients underwent limb-sparing surgery, 4 had amputations and 1 was treated with radiotherapy alone. The mean follow up time was 31.5 months (range, 8-81). At this time, 4 patients (36%) had developed lung metastases and 5 patients (46%) had died. Overall survival was 54.5%. Intratumoural MVD was higher in patients over 40 years, although not statistically significant (p = 0.111, CD31; p = 0.134, CD34). VEGF was uniformly expressed in all sections, however no relationship was found between the degree of expression and patient age. CONCLUSION: The prognosis for older patients with osteosarcoma is generally poor. Initial presentation is commonly associated with metastatic disease and neoadjuvant chemotherapy is often avoided because of its side effects. Increased intratumoural vascularity may contribute to the poorer prognosis in these patients, however further studies are needed.

BACKGROUND: Metastatic disease to the shoulder girdle is a challenging problem because of the potential for pain, pathologic fracture and loss of function of that limb. Management of the bone disease centers around palliation, prevention of further complications and the preservation of residual function. A variety of surgical options exist for managing metastatic disease to the shoulder girdle and our experience with over 90 patients is reported. We focus on a preferred technique of combining rigid intramedullary nailing with cementation. METHODS: Patients with metastatic disease to the shoulder girdle were accrued over a 9 year period from 1996 to 2004. 93 patients were identified with 96 operations being performed. The median age was 63 years (range 33 - 89) and 54% were female. The commonest primary tumor to metastasize was breast, and the proximal and midshaft humerus was involved in 84% of cases. The median survival time was 8 months and at last review 82% of patients had died of their disease RESULTS: Operations performed were intramedullary nailing (n = 51), resection with or without prosthetic reconstruction (n = 34) or plate osteosynthesis (n = 9). The site of the metastasis was a guide to the most appropriate operation. Amputations (n = 2) were not done as the primary procedure. Median post operative hospitalization ranged from 3 to 6 days depending on the type of operation performed. Our preferred technique for diaphyseal lesions (intramedullary nailing plus cementation) achieved excellent results in terms of pain relief, functional restoration and minimal complications. Functional restriction was most notable for proximal humeral prostheses (35% of patients). CONCLUSION: Surgical treatment of metastases to the shoulder girdle can be successful, allowing prompt relief of pain and return to prehospital level of care. Proximal and midshaft humeral metastases are easily amenable to resection and reconstruction or intramedullary nailing with cementation. Relief of pain and preservation of function occurs for the majority of patients.

Epithelioid hemangioendothelioma (EH) of a hand bone is extremely rare. We describe a patient with a phalangeal EH. This was treated by resection of the entire middle phalanx with no recurrence within a 10-year postsurgical follow-up period. We recommend wide resection as the safest treatment for EH of the hand phalanx.

This study investigates whether the proliferative activity of giant cell tumour of tendon sheath is related to its recurrence rate and local aggressiveness. The clinicopathological and immunohistochemical features of 30 localized giant cell tumours of tendon sheath were studied and the influence of the MIB-1 staining index on recurrence, tumour extent around the phalanx and involvement of the bone were evaluated. No significant difference in the MIB-1 staining index was found between the lesions which recurred and those which did not. Also there was no significant association between local aggressiveness and the MIB-1 staining index. These results suggest that the proliferative activity of localized giant cell tumour of tendon sheath is not related to its high recurrence rate and local aggressiveness.

64歳男.左前腕部腫瘤からの出血を主訴とした.左前腕伸側に7×6×6cmの表面一部顆粒状で亀裂を認める弾性硬の有茎性腫瘤を認め,基部皮下にも9×10×3cmの腫瘤を認めた.MRI T2強調画像で腫瘍部分は中信号から一部高信号で,MRI造影T1脂肪抑制所見では腫瘍部分は高信号で描出された.血管造影所見では橈骨動脈,尺骨動脈,腫瘍部分に巨大なtumor stainを認めた.易出血性のため生検は困難と判断し,細胞診および画像所見より軟部悪性腫瘍と診断し左上腕切断術を施行した.病理組織学的および免疫組織化学的所見より悪性線維性組織球腫striform pleomorphic typeと診断した.術後3ヵ月半後に胸部CTにて肺転移を認め,ifosfamide単独投与による化学療法3クール施行したが著変なく,cisplatin,adriamycin療法に変更したところ,2クール施行で一部転移巣の縮小を認めた.術後11ヵ月で他臓器への転移は認めず,部分寛解である

41歳女.27歳時に右前腕悪性黒色腫と診断され,DAV療法などを受けたことがあった.3年前に右上腕に皮下腫瘤を自覚し,腫瘤は徐々に増大した.生検にて悪性黒色腫の転移と診断した.化学療法(DAC-Tam),放射線療法を施行したところ,腫瘍は80%の縮小を示し,5-S-CD値も正常範囲に下降した.右上肢の機能温存が可能と判断して切除術を行った.術後,化学療法(DAC-Tam+Feron)を追加し,術後1年4ヵ月を経た現在,再発を認めない.悪性黒色腫において,転移病巣が10年以上後に出現するlate recurrenceは稀である.DAV療法後,長期間を経て再発を来たした症例には,DAC-Tam療法は有効な治療法の一つであると考えられた

手の腱鞘巨細胞腫のMRI像について検討した.対象は,術前にMRI検査を行い術後組織学的に診断された手指の腱鞘巨細胞腫26例(男性10例,女性16例・平均43.4歳)であった.その結果,T1強調像22例中9例で筋肉と同等の信号強度を,13例で筋肉と脂肪の間の信号強度を示し,T2強調像20例中5例で筋肉と同等の信号強度を,15例で筋肉と脂肪の間の信号強度を示した.T1強調像では25例中19例で不均一像を,T2強調像では全例で低信号域を含む不均一像を認め,この低信号域は結節状が20例,隔壁状が8例,辺縁に存在が20例であった.一方,ガドリニウムによる造影効果は施行19例中17例に認め,指骨周囲の病巣範囲は平均157°であった.また,MRIによる骨,関節,腱鞘内病変の検出は,それぞれ術中に確認した7例中2例,5例中4例,10例中9例であった.以上,MRIは腱鞘巨細胞腫の術前診断,病巣範囲の術前評価に有用と考えられた

70歳女.約5年前より右中指に皮疹が出現し,徐々に増大した.右中指基部背面から側面にかけて暗赤色局面を認め,潰瘍を形成していた.臨床的に手指に生じ,緩徐に発育する病変で,潰瘍形成を伴っていたこと,生検標本の病理組織学的所見にて腫瘍細胞の結節状の増生は明らかではなかったが,多角形と紡錘形の2種類の細胞の増生がみられ,腫瘍巣内に壊死を認めたこと,酵素抗体法で腫瘍細胞はkeratin,CAM5.2,EMAの上皮細胞マーカーとvimentinの両方に陽性であったことより,類上皮肉腫と診断した.右示指,中指の切断術を施行し,術後11ヵ月の現在,再発及び転移は認められない.手術時標本の病理組織所見では,腫瘍細胞は結節状に増生し,真皮全層から皮下組織,骨まで及んでいた.腫瘍の下層では,一部に骨に浸潤する像を認めた.好酸性の胞体を有する類上皮細胞様の腫瘍細胞が増生し,腫瘍巣の中心部には壊死を認めた

Recently, intravenous therapy with pamidronate has been reported to be effective for treating fibrous dysplasia. However, very little is known about the efficacy of oral alendronate for fibrous dysplasia. We describe a patient with polyostotic fibrous dysplasia, who had complained of persistent pain for more than 20 years, that was treated with oral alendronate alone. Follow-up examinations were at intervals of 3 months for 2 years. The pain, radiographic findings, and bone turnover, which was monitored using various markers, improved. No adverse side effect was noted. Oral alendronate is suggested to be a useful option in the treatment of polyostotic fibrous dysplasia.

The purpose of this study was to evaluate the usefulness of fine needle aspiration cytology for the preoperative diagnosis of soft tissue tumours of the hand. Fine needle aspiration cytology was performed on 93 soft tissue tumours of the hand which were classified as malignant, benign or unclassified based on cytological findings. We also attempted to make specific diagnosis by cytology. The cytological diagnosis was then compared with the postoperative histopathological diagnosis. The cytological differentiation between benign and malignant tumours showed neither false-positive nor false-negative results. Of the 47 lesions with sufficient material for cytology and that were postoperatively diagnosed histologically, 35 (including one recurrent lesion) were correctly diagnosed by fine needle aspiration cytology. No complications were encountered. Fine needle aspiration cytology has a high degree of diagnostic accuracy and safety for soft tissue tumours of the hand.

OBJECTIVE: The purpose of this study was to clarify the usefulness of MR imaging for preoperative diagnosis and evaluation of the extent of localized giant cell tumor of tendon sheath (GCTTS). DESIGN AND PATIENTS: We retrospectively reviewed the MR images of 25 patients with surgically proved GCTTS (seven males and 18 females; mean age, 41 years) including five recurrences. T1- and T2-weighted imaging was carried out on 24 and 22 lesions, respectively. Gadolinium-enhanced images were obtained for 20 lesions. We evaluated the tumor extent around the phalanx (the degree of circumferential occupation by a tumor around the phalanx on an axial plane) and involvement of the bone, joint, and tenosynovial space by both MR imaging and surgery (gold standard). RESULTS: MR signal intensities of the GCTTSs were consistently equal to those of skeletal muscle or between those of muscle and fat on T1-weighted images; on T2-weighted images, the signal intensities tended to be between those of muscle and fat. Most lesions were inhomogeneous due to low-signal-intensity areas, and enhanced following gadolinium administration. The tumor extent around the phalanx was 168.5 +/- 99.2 degrees (63-360 degrees). MR imaging did not identify the bone involvement (five lesions), but depicted the involvement of the joint in four of five lesions and the tenosynovial space in nine of ten lesions. CONCLUSIONS: This study confirms that MR imaging is able to depict the characteristic internal signal of GCTTS. Moreover, it can accurately assess the tumor size and degree of extent around the phalanx, which can affect the type of surgical approach.

51歳男性.腫瘍の縮小に放射線療法が奏効した骨外性粘液性軟骨肉腫の1例を経験した.患者は10年前,右鼠径部の鶏卵大腫瘤を摘出し,その後,某大学病院で再発,肺転移を指摘され受診した.右鼠径部に一部淡褐色の色素沈着を伴う表面平滑,多房性,弾性軟の腫瘤を認め,細胞診でパパニコロウ染色で青色のmyxoidな間質中に小型の核小体の異型性に乏しい腫瘍細胞の柵状配列を認め,病理所見では粘液基質を背景に卵円形の核の細胞質に乏しい腫瘍細胞が索状ないし充実性に増殖していた.MRIでは右鼠径部に境界明瞭で内部不均一な多房性の腫瘤を認め,胸部X-P,CTで両肺野に1cm大の腫瘤を多数認めた.肺転移を伴う右鼠径軟部原発の骨外性粘液性軟骨肉腫と診断した.放射線療法で1年後には表面への突出はほぼ認めなくなり,残存腫瘤摘出および肺転移への化学療法を勧めたが,拒否され経過観察中である

手術を施行した手足部発症の腱鞘巨細胞腫19例を対象とし,再発傾向と局所浸潤性について細胞増殖能の面から検討した.腫瘍細胞の核に顆粒状の集積を示す陽性細胞を散在性に認めたが,陽性細胞の密度は同一標本内において均一ではなかった.MIB-1 indexは17.9±7.1%(6.6〜33.6%)であった.再発については,低MIB-1群と高MIB-1群の両者で再発を認めなかったので,初発例と再発例の間におけるMIB-1indexの比較検討を追加施行したが差はなかった.局所浸潤性は,骨内浸潤以外の項目(軟部組織における浸潤性)において,有意差はないが高MIB-1群の方で陽性率が高い傾向を示した.年齢と大きさについては両群に差はなかったが,受診迄の期間は高MIB-1群において短い傾向が見られた

58歳女.左腹部に腫瘤を自覚し,骨盤軟骨肉腫の診断で手術を勧められたが,温熱療法など保存的治療を選択していた.腹部膨満感,左大腿部痛,左下肢筋力低下,歩行障害が出現・増悪したため入院した.画像所見と前医での組織所見から,腸骨より発生した低悪性度軟骨肉腫と診断し,手術を施行した.術直後は腸管麻痺を生じたが保存療法で軽快し,術後6週より歩行訓練を開始した.術後1年4ヵ月の現在,再発,転移の徴候は認めず,歩行に片松葉杖を要するが,疼痛は殆どなく,通常の日常生活を過している.Ennekingの術後機能評価法は53%であるが,本人の満足度は高い

ロシアのイリザロフセンター製の創外固定器を紹介し,末節骨の仮骨延長を行った症例を呈示した.26歳女.右中指を末節骨レベルで切断し,美容面での改善を強く希望していた.外観上12mmの短縮とhook nail変形を認め,単純X線でも末節骨は健側比で11mm短縮していた.末節骨に1.2mm径のワイヤーを3本,中節骨に1.8mm径のワイヤーを2本刺入し,末節骨の一番近位のワイヤーと中手骨の2本のワイヤーを近位のボルトに,末節骨の遠位の2本のワイヤーを遠位のボルトに固定した.創外固定器装着後,末節骨の近位の2本のワイヤー間でcorticotomyを行った.術後4日目から1回0.125mm,1日4回で延長を開始し,9mmの延長後,6週で創外固定をはずした.横径はやや細いものの良好な仮骨が成熟していたが,爪も同時に遠位へ移動してcraw nail変形が残存したため,掌側皮膚の延長と爪の後退を追加した.術後3ヵ月の現在,隣接指とのバランスは良好である

We investigated the surgical results of lumbar disc herniation in the elderly. Ten elderly patients underwent surgical treatment between 1990 and 1999. There were 7 males and 3 females with a mean age of 68.2 years (range: 60~85 years). Thirteen patients in their 20s and 30s were used as a control. Preoperatively, severe leg pain and gait disturbance, and higher rates of negative straight leg-raising were shown in the elderly group. Higher levels of herniation and higher graded disc degeneration were shown compared with the young. The mean total of the Japanese Orthopedic Association score in the elderly group improved from 5.1 points to 13.1 points postoperatively, and that in the young group improved from 6.9 points to 14.5 points. The recovery rate was 82.6% in the elderly group and 94.3% in the young group. Postoperatively, the recovery of gait disturbance in the elderly group tended to be poor compared with the young group. The elderly patients would not tolerate bed rest and would often do poorly if kept recumbent and in pain for long periods of time. Satisfactory results are being sought with regard to the operative management of lumbar disc herniation in the elderly.

SAPHO syndrome stands for synovitis, acne, pustulosis, hyperostosis and osteitis. The common site of skeletal lesions in this syndrome is the sternocostoclavicular area. Spondylodiscitis is rarely described in published studies. In general, skin lesions develop before the onset of skeletal lesions. We report a case of SAPHO syndrome in which spondylodiscitis developed more than 1 year before the onset of pustulosis.

手に発生した骨・軟部腫瘍及び腫瘍様病変に対する穿刺吸引細胞診の有用性について検討した.対象は軟部病変51例,骨病変13例で,方法は全例にクラス分類を行い,そのうち可能な症例では推定診断を行った.結果は軟部病変における細胞採取率は92%,良悪性の正診率は100%,推定診断率は76%であり,又,骨病変における細胞採取率は92%,良悪性の正診率は100%,推定診断率は88%であった

A comparative study concerning the histogenesis of Ewing's sarcoma and its relationship with peripheral primitive neuroectodermal tumor (PNET) was undertaken using histological, immunohistochemical and ultrastructural techniques. The cases studied included 22 Ewing's sarcomas, 17 (ganglio) neuroblastomas, 3 peripheral PNETs, 1 peripheral neuroblastoma, 1 Askin's tumor and a case in which it was difficult to distinguish between Ewing's sarcoma and neuroblastoma. Almost all the cases of Ewing's sarcoma showed neural differentiations. Immunohistochemical study using antibodies for neuron-specific enolase, protein gene product 9.5, Leu 7 and neurofilament (200 kD) was most useful for examining the neural differentiation of these tumors. These results suggest that Ewing's sarcoma is in a broad sense one of such peripheral PNETs as (ganglio) neuroblastoma, peripheral neuroblastoma and Askin's tumor.

背景:脊索腫は、仙骨や頭蓋底など軸骨格を中心に発症する比較的まれな腫瘍であり、組織学的に確定診断にいたるには担空胞細胞の同定が重要な手掛かりとなる。われわれは、術前細胞診で担空胞細胞を認めず、診断に苦慮した脊索腫の1例を報告する。症例:69歳、女性。転倒後の仙骨部領域の持続的な疼痛を訴え、当院を受診。骨盤部レントゲン、MRIで、仙骨領域に分葉化する腫瘤を認めた。穿刺吸引細胞診では、多量の粘液物質内に多形性や核異型を有する紡錘形細胞の増生を認めたが、特徴的な担空胞細胞は同定できず、軟骨肉腫を第一に考えた。その後、生検、手術検体で、好酸性の胞体をもつ上皮様異型細胞の増生を認め、脊索腫と確定診断された。結論:脊索腫の細胞診では、担空胞細胞が同定できない場合、診断が困難となる。臨床、画像的に脊索腫が示唆される場合、典型的な細胞像を欠いても、脊索腫の可能性があることを念頭におく必要がある。(著者抄録)

45歳男。左大腿内側部に約12cm長の腫瘤を認めた。切開生検で、組織学的に骨外性粘液型軟骨肉腫(EMC)と診断し、広範切除術を施行した。単純X線像にて軟部腫瘤像と左大腿骨小転子部の骨侵食像を認めた。MRIにて、腫瘍はT1弾調画像にて筋とほぼ等信号、T2強調画像にて高信号を呈し強く造影された。腫瘤の内部にT1強調画像にて造影効果のない高信号域を認め、内部の出血性変化も疑われ悪性腫瘍と思われた。新鮮切除腫瘍組織を用いて捺印標本を作成した。組織学的所見では、線維性の結合織に分画された粘液様基質を伴った類円形細胞を主としたレース状・分葉構造の細胞増殖を認めた。G-band法にて染色体分析を行った。染色体相互転座、t(9;17)(q22;q11)を検出した。

20歳以上でMP関節に外傷の既往及び愁訴のない健常例80例80関節を対象とした.単純X線母指正側2方向についてX線計測を行った.正面像において,橈尺側両種子骨各々の長径(RL,UL)・短径(RS,US),両種子骨間距離(DS),両種子骨中心点間距離(DSc),基節骨橈・尺側最近位端点を結んだ直線と種子骨中心点を結んだ直線のなす角度(α角)を測定検討した.結果は,RL:5.0±1.0mm,UL:4.7±0.6mm,RS:3.5±0.7mm,US:4.2±0.6mm,DS:4.3±1.1mm,DSc:9.3±1.1mm,α角:6.3±2.5°であった
