    BACKGROUND: Patients with lower-limb osteomyelitis (LLOM) may experience major adverse events, such as lower-leg amputations or death; therefore, early diagnosis and risk stratification are essential to improve outcomes. Ga-scintigraphy is commonly used for diagnosing inflammatory diseases. Although the diagnostic performance of planar and SPECT imaging for localized lesions is limited, SPECT/CT, which simultaneously acquires functional and anatomical definition, has resulted in significant improvements to diagnostic confidence. While quantitative Ga-SPECT/CT is an emerging approach to improve diagnoses, its diagnostic performance has not been sufficiently evaluated to date. Therefore, this study aimed to evaluate the diagnostic performance of Ga-SPECT/CT with quantitative analyses for patients with LLOM. METHODS: A total of 103 consecutive patients suspected of LLOM between April 2012 and October 2016 were analyzed. All patients underwent Ga-scintigraphy with SPECT/CT imaging. Findings were assessed visually, with higher than background accumulation considered positive, and quantitatively, using Ga-SPECT/CT images to calculate the lesion-to-background ratio (LBR), the maximum standardized uptake value (SUVmax), and total lesion uptake (TLU). Diagnoses were confirmed using pathological examinations and patient outcomes, and diagnostic performances of planar, SPECT, and SPECT/CT images were compared. To evaluate prognostic performance, all patients were observed for 5 years for occurrences of major adverse events (MAE), defined as recurrence of osteomyelitis, major leg amputation, or fatal event. Multivariate Cox regression was performed to evaluate outcome factors. RESULTS: The overall diagnoses indicated that 54 out of 103 patients had LLOM. LBR, SUVmax, and TLU were significantly higher in patients with LLOM (12.23 vs. 1.00, 4.85 vs. 1.34, and 68.77 vs. 8.63, respectively; p < 0.001). Sensitivity and specificity were 91% and 96% for SPECT/CT with LBR, 89% and 94% for SPECT/CT with SUVmax, and 91% and 92% for SPECT/CT with TLU, respectively. MAE occurred in 23 of 54 LLOM patients (43%). TLU was found to be an independent prognostic factor (p = 0.047). CONCLUSIONS: Ga-SPECT/CT using quantitative parameters, namely LBR and TLU, had better diagnostic and prognostic performances for patients with LLOM compared to conventional imaging. The results suggest that Ga-SPECT/CT is a good alternative for diagnosing LLOM in countries where FDG-PET/CT is not commonly available.

    OBJECTIVE: Current data indicate that the rate of trauma in children during gymnastic formation is increasing, especially while creating a structure with a certain height, such as the human pyramid. The goal of the present study was to clarify the clinical characteristics of these injuries. METHODS: In this single-institution review, all children treated for a gymnastic formation-related injury at Nippon Medical School Hospital from 2013 through 2015 were identified through the institution's registry. The injury mechanism was classified, and injury severity, interventions, and outcome were examined. RESULTS: Eight children were treated for a gymnastic formation-related injury. They were 7 boys and 1 girl aged 10 to 15 years (mean age, 13.1±1.8 years). Neurotrauma ranging from concussion to spinal cord injury without radiographic abnormality occurred in 6 patients (75%). No intracranial hemorrhagic lesions were detected. The Glasgow Coma Scale score on arrival was 15 in all 8 patients, and neurological deficits were present in 1 patient. No patient required surgical intervention. All patients made a full recovery after discharge from the hospital. No patients died. The average follow-up period was 2.1±0.9 weeks. CONCLUSIONS: Neurotrauma is a frequent result of gymnastic formation accidents in children. Healthcare workers and teachers should recognize this type of injury, and public education that targets parents should be introduced.

    PURPOSE: Despite advances in therapeutic angiogenesis by bone marrow cell implantation (BMCI), limb amputation remains a major unfavorable outcome in patients with critical limb ischemia (CLI). We sought to identify predictor(s) of limb salvage in CLI patients who received BMCI. MATERIALS AND METHODS: Nineteen patients with CLI who treated by BMCI were divided into two groups; four patients with above-the-ankle amputation by 12 weeks after BMCI (amputation group) and the remaining 15 patients without (salvage group). We performed several blood-flow examinations before BMCI. Ankle-brachial index (ABI) was measured with the standard method. Transcutaneous oxygen tension (TcPO2) was measured at the dorsum of the foot, in the absence (baseline) and presence (maximum TcPO2) of oxygen inhalation. (99m)technetium-tetrofosmin ((99m)Tc-TF) perfusion index was determined at the foot and lower leg as the ratio of brain. RESULTS: Maximum TcPO2 (p = 0.031) and (99m)Tc-TF perfusion index in the foot (p = 0.0068) was significantly higher in the salvage group than in the amputation group. Receiver operating characteristic (ROC) curve analysis identified maximum TcPO2 and (99m)Tc-TF perfusion index in the foot as having high predictive accuracy for limb salvage. CONCLUSION: Maximum TcPO2 and (99m)Tc-TF perfusion index in the foot are promising predictors of limb salvage after BMCI in CLI.

    Plaque disruption, which may be associated with some coronary risk factors, plays a key role in the development of acute coronary syndromes and progression of atherosclerosis. However, the clinical profile of asymptomatic plaque disruption in stable ischemic heart disease has not been well evaluated. The aim of the present study was to investigate the frequency and determinants of silent plaque disruption (SPD) in patients with stable ischemic heart disease using coronary angioscopy. Forty-one patients with stable angina or old myocardial infarction (OMI) without any complaints within 3 months were included in the present study. Angioscopy was successfully performed through 49 nonischemic related coronary arteries. The presence of SPD and coronary risk factors were recorded. Silent plaque disruption was found in 12 patients with stable ischemic heart disease (12/41, 29.3%), and the frequency of SPD in nonischemic related coronary arteries was 26.5% (13/49). A significantly higher frequency of SPD was noted in yellow plaques than in white plaques (35.3% versus 6.7%, P = 0.043). Overall, the independent clinical risk factors of SPD in nonischemic related coronary arteries were diabetes mellitus (P = 0.018; OR, 18.8209; 95% CI, 1.6525 to 214.3523) and hypertension (P = 0.0313; OR, 6.6485; 95% CI, 1.1850 to 37.3019). These results suggest silent plaque disruption was commonly observed in nonischemic related coronary arteries in patients with stable ischemic heart disease and its determinants were diabetes mellitus and hypertension.

    都心部総合病院救急外来を受診した外国人患者の臨床的特徴を調査し,外国人診療における課題を検討した。2017 年 4 月から 9 月にかけて,日本医科大学付属病院救急・総合診療センターを受診した初診外国人患者 158 例を対象に,年齢,来院方法,緊急度トリアージ,疾患分類,出身国,在留方法,保険の有無を後方視的に調査した。平均年齢 38 ± 16 歳,来院方法では walk-in 131 例(83 %),救急搬送 27 例(17%)であり,トリアージでは緊急 4 %,準緊急 23 %,低緊急 50 %,非緊急 23 %であった。疾患分類では感染症 22 %,消化器疾患 11 %,外傷 8 %,精神疾患 5 %の順であった。出身国では中国 41 %,韓国 13 %,ネパール 8 %,フィリピン 8 %,アメリカ 6 %の順であり,在日外国人は 83 %,健康保険加入率は 75 %であった。患者は旅行者よりも,在日外国人が多く,健康保険の加入率も高かった。東京オリンピック・パラリンピックに向けて,外国人旅行者の急増が予想される中,医療通訳をはじめ,医療環境整備に取り組んでいく必要がある。

    【背景】日本医科大学付属病院総合診療センターは,外傷を含む 1・2 次救急患者や診療科の特定できない初診患者の診療を主に行っており,2010 年度から初期臨床研修医による救急・総合診療研修が必修化された。【目的】研修医教育の取り組みに関するアンケート調査をもとに臨床教育の問題点,課題について検討した。 【方法】2011 年 4 月から 2014 年 3 月にかけて,当センターをローテイトした1年次初期臨床研修医を対象に研修・教育に関するアンケート調査を行った。
    【結果・考察】研修内容・教育の総合評価では,満足度は 5 点満点中平均 4.2 士 0.8 点であり,研修前後における救急・総合診療に対する関心度は,研修前 3.7 士 0.8 点から研修後 4.1 士 0.5 点と有意に増加した(p < 0.01 )。短期間に幅広い疾患を経験できたことや充実した指導体制が高評価の主因と考えられた。研修医の質を高め,診療レベルの向上を図るために,引き続き効果的な教育方法を導入し,客観的な教育評価を継続する必要がある。

    We performed the maggot therapy using by medical aseptic larvae for refractory wounds in clinical fields of Japan, and furthermore we established the bioventure business to develop and spread the maggot therapy in Japan. The maggot therapy has great debridement effect, and is very effect to infectious wounds for antimicrobial peptides etc. And so the maggot therapy will get the great effect of reducing the wound healing periods to growing up the healthy granulation for very short time. We also developed the maggot bag of high molecular compound which first maggots never escape from bag, second can’t see the maggots from outside, third less feel pain, fourth easy to attach and remove the bag. In our department the results of maggot therapy in 159 cases are 91.8% of the limb salvage rate, and 91.2% of survival rate in 7.1±0.2 years of mean follow-up periods. Especially the diabetic foot gangrene without peripheral arterial disease (PAD) is very effect and safe with any events and troubles. The maggot therapy is a novel wound treatment which everybody can perform easily, except out of national health insurance.

    Infected peripheral ulcers represent the major reason for leg amputation. We formed a multidisciplinary team for patients who needed to undergo amputation that combined the use of conventional interventions and maggot debridement therapy (MDT). Various factors were reviewed to clarify the problem and ideal application as well as determine the importance of multidisciplinary / MDT treatment for limb salvage.<BR> A questionnaire was provided to the nurse and patients. Seventy-eight patients underwent maggot debridement therapy (between Jan 2008 and November 2011). Prognoses including adverse events and conflicting clinical factors were analyzed by multiple regression analysis.<BR> Answers provided on the questionnaire indicated the importance of a multidisciplinary team approach. Multiple regression analysis revealed that patients on hemodialysis and those complicated with antibiotic-resistant bacterial infections had worse outcomes. Thus, intractable ulcers with MDT are best treated by a multidisciplinary team of physicians and co-medical experts, resulting in a 93% clinical successful rate (against death and limb amputation).

    再生医療としては初めて2003年6月に高度先進医療(現在は先進医療)に認可された「バージャー病,ASOによる治療抵抗性CLIに対する自己骨髄細胞移植による血管再生療法」を当院でも2002年4月より取り組み,全国3施設目として先進医療承認された(現在全国18施設の認定).当科ではプライマリーエンドポイントを自立歩行による退院としており,厚労省難治性疾患克服研究事業により適応を膠原病・アレルギー疾患(難病認定のPSS,アレルギー性肉芽腫性血管炎等)による治療抵抗性CLIにも適応拡大し,合計60例の分析では達成率88.3%(53例),平均追跡期間7.7年の経過における救肢率は77.7%,生存率は84.4%であった(9/60例下肢切断,8/60例死亡).疾患別では,ASOの予後は65.5%,ASO以外は96%であった.さらに私共は2008年内閣府先端医療開発特区(スーパー特区)採択課題での分担研究として,筋肉内注射のみで血管再生を可能とする「DDS徐放化b-FGFハイドロゲルによる血管再生療法」も現在まで15例実施し,10例での安全性・有効性を報告している(Tissue Eng 2011).特に全身性強皮症(SSc)による難治性潰瘍・壊疽症例に対する自己骨髄細胞移植による血管再生療法は,安全で有効であり,同治療法を実施した11例についての成績を報告する.<br>
