Background: To develop high-quality human resources with the competence required for nursing, contents of undergraduate education programs must be reviewed and enhanced. Assessments of competence levels at the time of graduation can be used to evaluate nursing education programs. International comparisons using a common assessment instrument can help identify and endorse common features, as well as highlight areas that need reform in nursing education programs from a wide range of perspectives. Objective: To develop the Japanese version of the Nurse Professional Competence Scale Short-Form and assess its psychometric properties. Methods: This study used a descriptive cross-sectional design to validate an instrument, conducted at eight university schools of nursing in mainland Japan. A total of 714 graduating nursing students were invited and informed about the study in mid-February 2022, with 299 students (41.88%) responding to all questions and submitting their answers via online survey forms. After item analysis, confirmatory factor analysis was conducted. Concurrent validity and internal consistency were also assessed. Results: Respondent ages ranged from 21 to 51 years, with an average age of 22.4 ±1.97 years. Most of the respondents were female (97.99%). The mean score for the scale was 55.70 (±9.98), and the scores for the six competence areas ranged from 48.04 (±14.07) to 64.73 (±10.67). Item analysis revealed that all items met each criterion, except for Pearson’s correlation coefficients for Items 34 and 35. Confirmatory factor analysis showed the CMIN/df value was 2.46, and the root mean square error of approximation value was 0.07. Concurrent validity analysis showed significant moderate correlations (r = 0.45 and 0.49, p < 0.001). The Cronbach’s α values for the scale and six competence areas ranged from 0.75 to 0.95.Conclusions: The Nurse Professional Competence Scale Short-Form Japanese version, comprising 35 items across six competence areas, demonstrated reasonably acceptable validity and reliability for use with graduating nursing students in Japan. The highest- and lowest-scoring competence areas were Value-Based Nursing Care and Medical Technical Care, respectively.

Objective This study aimed to clarify the relationship between organizational justice and work engagement among nurses. Methods Japanese nurses working in a medium-sized hospital in the Tokyo metropolitan area were surveyed using a self-administered questionnaire. We conducted a stepwise multiple regression analysis with scores from the Japanese version of the Utrecht Work Engagement Scale (UWES-J) as the dependent variable and scores from the Japanese version of the Organizational Justice Scale (OJS-J), age, sex, position, employment status, shift work, self-efficacy, social support, work control, and work quantitative load as the independent variables. Additionally, a stepwise multiple regression analysis was conducted for each OJS-J subscale score. Results The questionnaire was distributed to 270 nurses, of whom 219 (83.0% response rate) provided valid responses. With the UWES-J as the dependent variable, Model 1 adjusted only for age and sex; Model 2 adjusted for position, employment status, shift work, and self-efficacy score; and Model 3 adjusted for the social support, work control, and work quantitative load scores. Model 2 and Model 3 showed a significant association to the positive direction between UWES-J and OJS-J (Model 3: β=0.202, P<0.01, R2=0.363). Furthermore, when the same analysis was conducted for each OJS-J subscale score, a significant association was found between procedural justice scores and the UWES-J (Model 3: β=0.165, P<0.05, R2=0.383). Neither model found a significant difference between distributional equity scores and information equity scores. Conclusion The results of this study examining the relationship between work engagement and organizational justice among hospital nurses showed that work engagement was associated with organizational justice, especially procedural justice. These results suggest that maintaining and improving organizational justice is important for improving nurses' work engagement.

BACKGROUND: Cognitive behavioral therapy for insomnia (CBT-I) has been shown to improve depressive symptoms in older adults with comorbid insomnia and depression. However, it remains unclear whether brief CBT-I is effective for improving depressive symptoms in community-dwelling older adults with insomnia symptoms of varying severity. AIM: This study sought to examine the effectiveness of a brief CBT-I intervention delivered by public health nurses to improve depressive symptoms among older adults recruited from a community setting. METHODS: This randomized controlled study compared sleep status and depression at baseline and a 3-month follow-up using a wait-list control design. Depression was evaluated using the Geriatric Depression Scale short form (GDS-SF). RESULTS: The GDS-SF scores in the CBT-I group improved over time (p < .01), with an effect size (Cohen's d) of 0.34 (95% CI [0.16, 0.58]). The results demonstrated the effectiveness of a brief CBT-I intervention for depression, although the effect size was small. LINKING EVIDENCE TO ACTION: A brief CBT-I intervention consisting of sleep hygiene education, stimulus control, sleep restriction, cognitive restructuring, and relaxation is effective for improving depressive symptoms among older adults in the community.

Organizational justice (OJ) influences the well-being of employees of organizations. We conducted a randomized controlled trial to examine whether or not brief management training increases OJ for subordinates. Study participants were managers and subordinates working in the private manufacturing sector. Randomization at the departmental level generated an intervention group of 23 departments (93 managers and 248 subordinates) and a control group of 23 departments (91 managers and 314 subordinates). Managers in the intervention group received a 90-min training session to investigate the attitudes and behavior of managers and help increase OJ. Subordinates completed self-administered OJ questionnaire surveys on procedural, interpersonal, and informational justice before and 3 months after intervention. For all subordinates, the interaction between group and time in OJ scores obtained before and 3 months after intervention were not significant. However, in subgroup analyses of the lowest tertile group in relation to the baseline of each of the three OJ subscales and total scores, the lowest tertile group of the interpersonal justice subscale showed significant improvement. The results of this study suggest that brief management training in OJ for managers significantly improves a low rating from subordinates in interpersonal justice. Further studies are required to develop a specific intervention method to increase OJ.

BACKGROUND: Medical-error analyses have been conducted to determine the root cause of adverse events and near misses. More precise determination of the cause-and-effect relationship likely will require a prospective design path analysis including both direct and indirect effects. METHODS: The authors performed a 6-month prospective cohort study using structural-equation modelling (SEM). Of the 879 nurses approached, 789 (89.8%) were included in the final analysis. Potential predictors provided for analysis included age, years of nursing experience, mean frequency of night shifts per month, nursing-specific job stressors, degree of depression, frequency of feeling unskilled, feeling time pressure, feeling a lack of communication between self and other hospital staff members, frequency of suffering from sleep disturbance and frequency of feeling a decrease in attention. The authors regarded a latent variable composed of frequencies for near misses and adverse events as an outcome. RESULTS AND CONCLUSION: The SEM model constructed in this study suggested that potential root causes (exogenous variables directly or indirectly connected to the outcome which are not affected by other variables) were years of nursing experience, feeling unskilled, job stressors and sleep disturbance, with estimated standardised total (direct and indirect) effects of -0.22, 0.21, 0.008 and 0.005, respectively. A prospective design path analysis using the SEM model for both direct and indirect effects enabled a statistical exploration of root causes and estimation of their impact on the outcome. Our findings suggested such an analysis to be useful in devising countermeasures against medical errors.

OBJECTIVE: To investigate whether or not brief bright light (BL) exposure on workday mornings can improve health, performance and safety in nurses with rapidly rotating shifts. METHODS: We conducted a randomized crossover study involving registered nurses at a teaching hospital working a two-shift system including the night shift. Participants were instructed to expose themselves to BL for 10 min on workday mornings. RESULTS: A total of 61 participants were enrolled in the present study. Thirty-one participants received BL exposure in the first month, and the other 30 received it in the second month. Significant improvements were noted in the BL periods compared with the non-BL periods for self-assessed sleepiness at 10:00 on day-shift days evaluated using the Karolinska Sleepiness Scale, self-assessment of night sleep for day-shift days using the Visual Analogue Scale and for fatigue assessed using the Checklist Individual Strength Questionnaire. The estimated mean difference for each scale (95% confidence interval) was -0.55 (-0.91, -0.20), 0.37 (0.04, 0.70) and -2.13 (-3.78, -0.48), respectively. Mean response time evaluated using the psychomotor vigilance task test (PVT) showed significant improvement in the BL periods compared with the non-BL periods. No statistically significant differences were observed for sleepiness at 14:00, depression, number of PVT lapses or frequency of perceived adverse events and near misses. CONCLUSION: Our findings suggest that brief BL exposure on mornings preceding a day shift is effective in improving sleepiness and performance during day-shift work, subjective nighttime sleep on day-shift days, and perceived fatigue for the preceding two weeks in rapidly rotating shift nurses.

Near miss-based analysis has been recently suggested to be more important in the medical field than focusing on adverse events, as in the industrial field. To validate the utility of near miss-based analysis in the medical fields, we investigated whether or not predictors of near misses and adverse events were similar among nurses at teaching hospitals. Of the 1,860 nurses approached, 1,737 (93.4%) were included in the final analysis. Potential predictors provided for analysis included gender, age, years of nursing experience, frequency of alcohol consumption, work place, ward rotation, frequency of night shifts, sleepiness during work, frequency of feeling unskilled, nurses' job stressors, working conditions, and depression. Variables for multivariate analysis were determined by bivariable analysis. Ordinal logistic analysis showed that predictors of near misses and adverse events were markedly similar. Parameters that were significantly related to both near misses and adverse events were years of experience, frequency of night shifts, internal ward, and time pressure (p<0.05 for all). The present study suggested that there was a negligible difference between choosing near miss- or adverse event-based analysis when identifying possible causes of adverse events in the medical field.

Shift work, including night work, has been regarded as a risk factor for medical safety. However, few studies have investigated the difference in medical error risk between two- and three-shift systems. A total of 1,506 registered nurses working shifts at teaching hospitals participated in this study to evaluate the difference in medical error risk between two- and three-shift systems. After adjustment for potential confounding factors using a log Poisson generalized estimating equation model, the results showed significantly higher frequencies of perceived adverse events over 6 months in the three-shift than in the two-shift system, with estimated mean numbers of adverse events of 1.05 and 0.74, respectively. Shorter intervals after night shifts and greater frequency of night shifts in three-shift systems, which reduce the recovery time from night shift work, may be linked to increased medical errors by nurses.

Objectives: Minamata disease (MD), first discovered in 1956, is a neurological disorder caused by mercury poisoning due to daily intake of fish and shellfish that have been contaminated by methylmercury discharged from chemical factories. However, reports of ill health increased sharply following the 2004 Supreme Court ruling instructing the Japanese government to pay damages to MD patients. We examined the distribution of disability in activities of daily living (ADL), and the association between MD status in terms of compensation system and ADL disability among the general population of previously methylmercury-polluted areas.<br> Methods: Data were collected by two-stage stratified sampling of residents 40–79 years old in 172 postal-code areas on the Shiranui Sea coast, the endemic area of MD. Questionnaires were distributed to eligible subjects (n = 2100) and collected at a later visit or by mail. Information on demographic factors, basic ADL (BADL), and instrumental ADL (IADL) was obtained using questionnaires. We performed logistic regression analysis to assess the relationship between MD status in terms of compensation system and ADL disability.<br> Results: We classified the 1422 residents who completed the questionnaire in accordance with their MD status in terms of compensation system: Early (those who received MD compensation before the Supreme Court decision), Recent (those who applied for compensation after the Supreme Court decision), Not Yet (those who have not yet applied for compensation, but have health-related anxieties about MD effects), and Normal (those who have not applied for compensation, and do not have health-related anxieties about MD effects). Adjusting for confounding factors, MD status was significantly associated with the disability grades of BADL and IADL with an increasing trend in the order of Normal, Not Yet, Recent, and Early. The odds ratios (95% CI) based on Normal were 2.08 (1.08–4.01), 3.87 (2.14–7.01), and 4.50 (2.66–7.61) for BADL and 2.41 (1.62–3.61), 3.20 (2.12–4.85) and 3.68 (2.52–5.38) in Not Yet, Recent, and Early for IADL, respectively.<br> Conclusion: Early, Recent, and Not Yet had lower ADL grades than Normal. Moreover, the population with a low ADL grade and health-related anxieties had increased throughout the previously methylmercury-polluted areas. The issue of ill health among populations living in previously methylmercury-polluted areas should be addressed in the wider context of public and community health.<br>

The improper handling of pesticides in agriculture has caused serious health problems in many developing countries. In this study, we report the pesticide usage condition among Indonesian farmers and its association with symptoms of pesticide toxicity. A questionnaire survey on personal history regarding agricultural labor, pesticide storage and disposal, pesticide use and health history was conducted using a structured questionnaire in rural Sundanese villages in West Java, Indonesia. The most frequently used pesticides included dithiocarbamates, pyrethroids and organophosphates. In approximately 80% of sprayings, category II pesticides (World Health Organization (WHO) categorization; "moderately hazardous") were used. Many of the subject farmers worked in a highly unsafe occupational environment; protective measures and safe handling were rarely observed, whereas smoking and drinking during spraying were frequently practiced. Correlation analysis revealed that farmers who wore a long sleeve shirt and headgear showed health symptoms less frequently. Moreover, farmers who had skin contact with the spray solution during measuring or mixing (excluding the hands), who wore wet clothing (skin exposure to pesticide), and who smoked and rubbed their eyes during spraying showed more symptoms. Among these factors, headgear use, wearing wet clothing (skin exposure to pesticide), and smoking during spraying were the significant determining factors for developing health symptoms. Preventing such behaviors will be an effective method of reducing health problems among the subject farmers.

The objective of this study is to develop an available empowerment model for workplace health promotion (WHP) in small and medium-sized enterprises (SMEs) and to evaluate its applicability and feasibility. Semi-structured interviews with employers and workers in SMEs were conducted to assess their actual requirements for support. The structure of our new empowerment model was discussed and established through several rounds of focus group meetings with occupational safety and health researchers and practitioners on the basis of results of our interviews. We developed a new participatory and action-oriented empowerment model based on needs for support of employers and workers in SMEs. This new model consists of three originally developed tools: an action checklist, an information guidebook, and a book of good practices. As the facilitators, occupational health nurses (OHNs) from health insurance associations were trained to empower employers and workers using these tools. Approximately 80 SMEs (with less than 300 employees) were invited to participate in the model project. With these tools and continued empowerment by OHNs, employers and workers were able to smoothly work on WHP. This newly developed participatory and action-oriented empowerment model that was facilitated by trained OHNs appears to be both applicable and feasible for WHP in SMEs in Japan.

This study was conducted to elucidate the caregiving-related needs of family caregivers looking after an elderly person with dementia. Caregiving-related needs were classified into six categories and their relationship with “care capacity” was elucidated. Items with a great need for professional medical and/or welfare support included medical treatment, medication guidance, physical observations, guidance on how to provide end-of-life care, and oral care. These caregiving-related needs were strongly affected by caregiving knowledge. A program of workshops for family caregivers was therefore held in collaboration with professionals from different disciplines over a four-year period to provide knowledge and build face-to-face relationships. Questionnaires and care capacity evaluations administered at each workshop revealed the efficacy of this care capacity acquisition support program provided through cooperation among different professionals.

The effects of long term exposure to arsenic on IQ (IntelligenceQuotient) or SC (social competence) of Bangladeshi children, who have been drinking arsenic-contaminated groundwater, were examined with more than 200 subjects in each agegroup of 4-5 years, 9-10 years, and 14-15 years old. Children who have been highly exposed to arsenic (those with high urinary arsenic concentration) showed lower IQ and SC scoresin each age group. The relation was consistent after controlling for parents’socioeconomic conditions (income, education and occupation). Therefore, exposure to arsenic (drinking arsenic contaminated groundwater) from early stage of life was considered to be critical for the mental and social development.
