革新的看護技術が医療現場に実装されるための因子の特定とその構造

根拠に基づく新しい看護技術・看護プログラムである看護イノベーションは、導入に成功しても継続しないとの指摘がある。本研究では、近年発展してきた普及と実装科学の知見を活かし、病棟などの一組織で看護イノベーションが継続することを持続可能性とし、導入後の継続や患者効果の持続を促す要因を明らかにすることを目的としている。
令和3年度は、1）文献検討による看護イノベーションの持続可能性に影響する影響要因の明確化と対象となる看護イノベーションの選定/決定、2）組織の持続可能性の程度と影響要因を測定する項目からなる質問紙作成、3）研究対象施設の選定を行った。
持続可能性を測定する項目は、先行研究を参考に普及と実装研究に精通した研究者、統計の専門家等と研究者で作成した。影響要因の測定には、Clinical Sustainability Assessment Tool, CFIR Inner Setting Measureの一部を使用する。開発者に使用許可を得て、順翻訳・逆翻訳はFACIT翻訳方法論に準じて実施した。翻訳した測定用具について、臨床看護師、看護研究者数名にインタビューした結果、内容については概ね理解できるが、欧米と日本における文化の違い、直訳では伝わりにくい箇所があり、一部質問項目を適応する必要があることが示唆された。質問項目の適応については、Clinical Sustainability Assessment Toolの開発者に許可を得て、一部の質問項目の適応を行った。
現在、質問紙を研究対象者に該当する病棟看護師、病棟管理者を対象に、内容妥当性に関する調査を実施している状況である。 -

A qualitative study was conducted, based on the diffusion of innovations theory, to develop nursing tools that help diffuse the sitting position without back support care for stroke patients among nurses. The following were found to be factors promoting the program: certified nurses or clinical nurse specialists acting as opinion leaders; development of core nurses; multidisciplinary/family cooperation; hospital ward culture; incorporation of care items in the electronic chart; program arrangement; and high relative advantage/trialability/compatibility. Impeding factors were: relocation of nurses; presence of nurses who refuse to use the sitting position without back support care; shortage of training materials; and low complexity/observability. Introduction, continuation, or discontinuation of the program depended on the number and intensity of promoting/impeding factors. It was considered important to modify the contents of the program.

With the rapid increase of the number of the nursing university, the importance of the evaluation of the nursing practice ability required by the end of graduation of the university increases.
Therefore, it is urgent to develop a common test available in the nursing university throughout Japan to evaluate quality of Student's competency and knowledge required for a nursing student by a stage to begin the clinical trial in hospitals.
I received a scientific research funds subsidy from 2008 through 2010 and, for the purpose of developing such common test to evaluate ability and knowledge required before the clinical trials in hospitals, 1,500 multiple choice items from 18 nursing domains were made and carried out monitor trials based on paper-pencil tests and computer tests (by means of CBT : Computer Based Testing) for the 730 students and 220 students respectively., By comparing both results,it follows that the usefulness of the computer examination was confirmed in comparison with of the paper-pencil tests,
