    BACKGROUND: The number of people diagnosed with dementia worldwide is set to increase significantly. Patients with dementia often have comorbidities, particularly diabetes, and patients with type 2 diabetes mellitus (T2DM) have a high risk of cognitive decline. This study investigated whether older people with T2DM have disease-specific cognitive deficits. METHODS: The Montreal Cognitive Assessment is a well-known tool for examining mild cognitive impairment, and the modified Japanese version (MoCA-J) has been confirmed as effective. Using the MoCA-J, we assessed the cognitive function of Japanese adults aged ≥75 years with and without T2DM and analyzed the results. RESULTS: Thirty-three patients with T2DM and 23 non-DM patients completed the examination, and MoCA-J total scores differed between these groups (T2DM mean, 21.4 ± 3.5; non-DM mean, 23.5 ± 3.6). Only 9% of patients with T2DM and 39% of those with non-DM had scores ≥26, which is the cutoff point for mild cognitive impairment, although all patients were capable of self-care. Additionally, delayed recall scores were significantly lower for the older patients with T2DM had for the non-DM group. CONCLUSIONS: Patients aged ≥75 years with T2DM might have worse cognition than those without T2DM; the inability to perform delayed recall in T2DM patients suggests a decline in cognitive function. Therefore, patients aged ≥75 years with T2DM should receive explanations of their care that are individualized in relation to their cognitive status.

    Abstract

    This study aims to implement a Shared Decision-Making (SDM)nursing practice model for older patients with dementia in acute care hospitals among acute care nurses and evaluate its feasibility. As a preliminary step, a hybrid concept analysis was conducted, and a SDM nursing practice model was developed and refined based on the results of semi-structured interviews with nursing practitioners and researchers. In this study, five acute care nurses were recruited as research participants to confirm the feasibility of this model. Data analysis involved conducting content analysis from verbatim transcripts of individual interviews with the five participants after practice to assess feasibility. Additionally, both before and after the practice, surveys based on SDM-Q9 and Group’s SDM, validated tools for assessing the SDM process, were administered to the five research participant nurses and five health care professionals involved in support (MD, MSW), generating data from the perspectives of users and stakeholders. The results indicated the feasibility of implementing the SDM process within the constraints of the acute care setting. Moreover, the study demonstrated that the SDM nursing practice model effectively adheres to the SDM process during acute phases, thereby clarifying the role of clinical nurses in facilitating the Sharad decision-making for older patients with dementia.

    Abstract

    Background

    Dementia patients often have comorbidities like Diabetes Mellitus, kidney disease, and cardiovascular disease that require patient decision-making about care, despite the challenges in judgment that stem from dementia. The study aimed to determine what Shared Decision-making is for older patients with dementia in acute care hospitals.

    Methods

    The study used the Hybrid Model of concept analysis method. This comprises a theoretical phase, a fieldwork phase, and an analytical phase. In the initial phase, the concept is selected. The fieldwork phase collects qualitative data to further analyze the selected concept. The third phase analyzes the empirical observations and writes up the findings. We searched Pub-Med, CINAHL, and PsycINFO for the keywords: “shared decision-making” and “patients with dementia.”

    Results

    We identified 14 articles that gave rise to over 100 codes. We defined shared decision-making for older patients with dementia in acute care hospitals as “The older person with dementia can be involved in collaborative activities in the care network, including delegation of authority as active decision-making. Open-minded communication based on person-centered care can be the foundation for considering feasible options. Clarify the role of family caregivers and identify decision-making participants in the care network to view the best alternative for the patient’s subsequent life”.

    Conclusions

    We developed a working definition of shared decision-making for patients with dementia. We clarified critical elements of the decision-making process, like based as person-centered care, open-minded communication, and collaborative decision-making activities between the person and the care network members, including delegation of authority.
