    BACKGROUND: Traditional evaluations of maternity care have predominantly relied on outcome-based indicators. However, quality should also be evaluated based on women's psychological and emotional responses. Seven key concepts reflecting enhanced midwifery care from women's perspectives were identified. As existing tools do not fully capture these elements, the Quality of Midwifery Childbirth Care from Women's Perspectives (QuMiCW) scale was developed. This cross-sectional study aimed to examine the validity and reliability of the Japanese version of the QuMiCW scale. METHODS: A web-based survey was conducted among Japanese women who had given birth within the past 10 years. Structural validity was examined through an exploratory factor analysis (EFA) using the least squares method and promax rotation on half of the sample, followed by confirmatory factor analysis (CFA) using covariance structure analysis on the remaining half. Test-retest reliability was evaluated by calculating the Spearman's rank correlation coefficient (ρ) between the initial and 1-month follow-up scores. Internal consistency was assessed using Cronbach's α. For hypothesis testing, correlations between the QuMiCW and both the Japanese version of the Birth Satisfaction Scale-Revised (BSS-R) and the Japanese Wijma Delivery Expectancy/Experience Questionnaire (JW-DEQ) Version B were examined. The study was conducted in accordance with COnsensus-based Standards for the selection of health Measurement Instruments guidelines. RESULTS: A total of 714 women were included in the analysis. EFA of the 18 items identified three factors: “fulfilling needs and support," “protection of dignity," and “empowerment and reassurance." The CFA model demonstrated a good fit (GF index = 0.990, adjusted goodness of fit index = 0.987, normed fit index = 0.987, relative fit index = 0.985, and standardized root mean square residual = 0.047). Test-retest reliability was high, with a ρ of 0.71 for the total score. Internal consistency was also high (Cronbach's α coefficient = 0.94). A positive correlation was observed with the BSS-R (ρ = 0.53), whereas a negative correlation was found with the JW-DEQ version B (ρ = −0.44). CONCLUSIONS: The QuMiCW demonstrated reliability and validity among Japanese mothers. It may be used to evaluate and improve midwifery childbirth care from a woman's perspective.     Background/Aims

    Midwifery care is crucial for women's physical and psychological wellbeing after birth. However, diverse evaluation indicators make it challenging to establish a unified concept of intrapartum midwifery care. Women's opinions can provide insights for better midwifery care. This review aimed to define and explore midwives’ intrapartum care from women's perspective.

    Methods

    This review searched PubMed, the Cumulative Index to Nursing and Allied Health Literature, Web of Science and Ichushi-Web for articles published after 2010 in English and Japanese. Rodgers’ conceptual analysis was used to outline the antecedents, attributes and consequences of intrapartum care.

    Results

    A total of 87 articles were included. Analysis highlighted seven attributes, five antecedents and eight consequences of intrpartum care.

    Conclusions

    From women's perspective, intrapartum midwifery care should address their needs, empower them and provide warmth and security. It should also protect their dignity while supporting their decisions and sense of control during birth.

    Implications for practice

    Improving practice will ensure that women receive high-quality care from midwives during childbirth. Developing a scale based on the concepts identified in this study could enable objective assessment of care. Such an approach may enhance midwifery care and improve childbirth experiences across different cultural contexts.

    BACKGROUND: Midwives and nurses are crucial in breastfeeding support. Few studies have explored appropriate language for nursing education on breastfeeding. We assessed the impact of the language used on breastfeeding attitudes among midwives and nurses. METHODS: A quasi-experimental study was conducted online in Japan among 174 midwives and nurses who had work experience in obstetrics or pediatrics. Participants were allocated to three groups to receive different text messages as the intervention (the benefit of breastfeeding for Group 1; the risk of formula feeding for Group 2; the importance of childcare for Group 3 as the comparison). The Japanese version of the Iowa Infant Feeding Attitude Scale (IIFAS-J) was used to assess breastfeeding attitudes before and after reading the texts. Also, participant reactions to the text were assessed by their responses to three statements. ANOVA, the chi-square test, and the t-test were used for outcome assessments. RESULTS: The post-test IIFAS-J score was significantly higher than the pre-test score only for Group 1 (p <  0.01). The percentage of participants who agreed with the content of the text was 70.7% in Group 1 and 48.3% in Group 2. The percentage of participants who reported discomfort with the text was 34.5% in Group 1 and 55.2% in Group 2. No significant difference among groups existed regarding interest in the text. In all three groups, participants who agreed with the text had a higher post-test IIFAS-J score than those who disagreed with the text (6.85 points higher, p <  0.01 in Group 1; 7.19 points higher, p <  0.01 in Group 2; 8.00 points higher, p <  0.02 in Group 3). Discomfort with the text and interest in the text were associated with a significantly higher post-test IIFAS-J score in Group 1 and Group 2 but not in Group 3. CONCLUSIONS: "Benefits of breastfeeding" language, which conveys the information in a positive manner, appears to be more appropriate than "risks of infant formula" language for creating a positive attitude toward breastfeeding in nursing education. TRIAL REGISTRATION: This study was registered in the University Hospital Medical Information Network Clinical Trials Registry (UMIN000023322). Registered 05/08/2016.

    近年、日本で出産する外国人女性が増え、言語的な障壁に起因する戸惑いや不安を抱えている。本研究では、教科書より分娩時に用いる基本的な用語を分析し、外国人産婦と看護職のアプリ開発への示唆を得ることを目的とした。国立国会図書館のデータベースにて「母性看護学」「助産学」と検索し、2010年から2020年に出版され、入手可能な教科書7冊を分析した。対象文献の分娩に関する箇所を全てテキストデータ化し、KH Coderを用いて、抽出語リストを作成し、共起ネットワーク分析を実施した。最頻出語は、「分娩」「産婦」「子宮」「胎児」「陣痛」「場合」「状態」「必要」「出血」「行う」であった。また、共に出現する言葉は「胎児・心拍・機能」「陣痛・強い・痛み・緊張・不安」等が挙げられた。最頻出語と共に出現する言葉は、分娩進行時の事象や産婦・胎児の状態であった。本研究に用いた分析方法を参考に、今後母親向けの本の分析や、臨床場面での産婦と助産師のコミュニケーションを分析することで、より具体的な示唆が得られるだろう。(著者抄録)

    知的障害者の家族が抱える課題と実際に行われている支援について質的文献について検討した。医学中央雑誌Web版を用いて2006年から2017年の文献を検索した。検索結果の中から、タイトルと抄録を確認するなどし、最終的に22件を分析対象とした。このうち親が対象の文献が18件、きょうだいが対象の文献が1件、複数の家族が対象の文献が1件、その他2件であった。課題と支援について211のデータが抽出され、内容を分析した結果、23のサブカテゴリー、「生活のなかでの困難」「困難に対する親の対応行動」「子どもとかかわることによる認識の変化」「家族にとって助かった支援」「潜在的な支援ニーズ」の五つのカテゴリーが生成された。家族が抱えているさまざまな困難が明らかになり、支援に関しては、既存の支援で効果的なものと、以前課題として残っている親のニーズがあることが明らかになり、今後の家族に対する支援への示唆を得ることができた。

    入院患者への看護師によるせん妄ケアのエビデンスをシステマティックレビュー、ランダム化比較試験(RCT)、ガイドライン、書籍・雑誌記事、質的研究の文献検討により収集し、統合することを目的に、Cochrane Library、医学中央雑誌Web版、PubMedなどを用い検索を行い、得られたわが国の看護師に適用可能と判断したシステマティックレビュー1件、RCT6件、ガイドライン10件、書籍・雑誌記事3冊、質的研究4件を分析した。その結果、システマティックレビューでは多職種による多面的介入がせん妄の発症を減少させる中程度のエビデンスが示され、個々のRCTでは、多職種による介入の具体的な内容がまとめられていたが、わが国で行われたRCTが存在しないことから、これらの多面的介入に含まれる内容から、各々の実践現場で適用可能な内容を選択し、多職種と連携しながら行っていくことが望ましいと考えられた。
