好酸球性副鼻腔炎(ECRS)は、気管支喘息と同様のType2炎症が主な役割を担っており、近年エンドタイプに基づいた生物学的製剤による治療が注目されている。今回、われわれは、経口ステロイド連日投与でもコントロール不良であったECRS術後再発例に対し、Type2サイトカインであるIL-4、13のシグナル伝達を阻害するデュピルマブを投与することで良好な経過を得た。また、高度の嗅覚障害にも有効であった。デュピルマブ投与により、既存治療に抵抗性のECRSの改善率向上に寄与できる可能性が示唆された。(著者抄録)

Abstract

Anatomy education is a crucial part of the curriculum for medical students, and it is essential for surgeons to perform safe surgery. The anatomy of the sinus cavity is complicated and cannot be easily understood through cadaver dissection practice and reading of surgical anatomy textbooks. Therefore, the development of methods to effectively teach the anatomy of such complex structures is essential for performing safe medical procedures. This study aimed to investigate whether the use of virtual reality (VR) in an educational course on paranasal sinus anatomy could contribute to the understanding and learning efficiency of dissection of the complex sinus anatomy. We devised a system to observe the paranasal sinuses in the VR space by polygonizing the shape of the sinus cavity using computed tomography images of the sinuses. Medical students moved around the VR space and viewed the sinuses from different angles using the VR system; notably, this is impossible in a conventional anatomy laboratory. A questionnaire was administered to 27 medical students (15 men and 12 women; mean age, 24.9 years) to evaluate their understanding of the sinus anatomy and the number of times they had experienced VR and used VR systems. We found that more than 70% of the students had never used VR before, but they could easily use it even if they were unfamiliar with the operation of the VR system, and the learning process was enjoyable and efficient. Thus, the VR application could be a useful tool for educating medical students.

PURPOSE: To investigate the clinical utility of q-Powders-a retronasal identification screening test. METHODS: A total of 156 subjects (92 females, mean age: 54.5 years ± 17.3 years) completed a 3-item q-Powders retronasal identification test and a 16-items Sniffin' Sticks orthonasal identification test. We analyzed whether the q-Powders test could differentiate between subjects with normosmia and subjects with an olfactory disorder. RESULTS: Our data indicated that subjects with an olfactory disorder scored lower in the q-Powders test than subjects with normosmia. The analyses revealed q-Powders test sensitivity of 84% and a test specificity of 64.9% with a score of 2 points taken as a cutoff for olfactory disorders. CONCLUSION: The 3-item q-Powders retronasal test may be used for screening purposes in clinical research. LEVEL OF EVIDENCE: 4.

＜文献概要＞65歳,男性。初診時,咽頭・喉頭に多数のびらんがあり,左上腕と上背部に小紅斑が散在していた。背部紅斑の病理組織像は表皮下水疱,水疱内と真皮血管周囲のリンパ球,組織球,好酸球の浸潤を呈した。蛍光抗体直接法では表皮真皮境界部にIgG,C3が沈着し,蛍光抗体間接法は患者血清のIgGがsplit skinの表皮側に反応した。免疫ブロット法では患者血清のIgGがBP180のC末端,NC16a,120kDa LAD-1に反応した。抗BP180型粘膜類天疱瘡と診断した。ステロイド全身投与のみで症状は改善せず,血漿交換,免疫グロブリン大量静注療法で改善したが,ステロイド減量に伴い再燃し,ミゾリビン追加により症状は改善した。

The choice of an appropriate surgical approach for accessory parotid gland tumors is controversial because of the rarity of this type of tumor. In this retrospective study, we analyzed and assessed our experience of surgery by a direct approach using facial incision.<br>From 2010 to 2013, we performed five surgeries for accessory parotid gland tumor. Three of the patients with these tumors were male patients and two were female. The mean age was 55, ranging from 40 to 66. Three were benign tumors and the other two were malignant. During these operations, the facial nerve was easily identified in all cases and preservation of the nerve was not difficult except in one case with myoepithelial carcinoma. In this case, partial excision of the masseter muscle was necessary, but this approach provided wide exposure and good flexibility. Postoperative facial scar demonstrated good cosmetic results and was satisfactory to all the patients.<br>These results indicate that the direct approach using facial incision is feasible and preferred from both aspects of the incidence of postoperative facial palsy and cosmetic results as compared with the trans-parotid approach by S-shaped incision.

過去2年間に当科アレルギー外来にてアレルゲン免疫療法を開始した134例を対象とし、年齢性別、治療抗原、治療内容、注射回数、副作用、治療効果につき報告した。患者の年齢は30歳から50歳に多く、人数に男女差はみられなかった。治療抗原はスギ単独が54例(40.3%)、スギ+ハウスダストが36例(26.9%)と多かった。各抗原とも2段階、3段階のステップアップを行ったが、開始量、維持量には個人差がみられた。合計3,481回の注射で5回(0.14%)、134例中5例(3.7%)に副作用がみられた。脱落症例は22例(16.4%)であり、うち11例が抗原増量中の脱落であった。スギのアレルゲン免疫療法を行った患者のうち約27.7%が2012年スギシーズン中の薬物療法を必要としなかった。(著者抄録)

We experienced a case of basaloid carcinoma of the esophagus with a solitary lung metastasis for which thoracoscopic partial lung resection was performed. To the best of our knowledge, this is the first reported case of basaloid carcinoma of the esophagus with lung metastasis for which surgery was performed. There are no evidencebased treatment strategies for postoperative recurrence of basaloid carcinoma of the esophagus. Treatment strategies such as adjuvant therapy centered on chemotherapy and surgical indications should be established. © The Japanese Association for Thoracic Surgery 2012.

高齢頭頸部癌患者は増加しているが、明確な治療指針はなく、治療の限界を症例毎に考えているのが現状である。そこで、過去10年間に当科を初診した75歳以上の頭頸部癌患者の173例を対象に診療内容を解析した。喉頭癌に対しては80%に根治治療が可能であった。一方、口腔・下咽頭・中咽頭癌に対する治療法の選択は複雑で、特に80歳以上で根治治療が可能であった症例は半数以下であった。両群とも根治治療が不可能な理由は併存疾患の存在であった。以上から、高齢頭頸部癌患者への対応では併存疾患の評価が重要であると考えられた。また頭頸部癌治療後の、併存疾患の経過も加味した癌治療適応の判断が必要であると思われた。(著者抄録)
