    超高齢社会の日本において死因の一位はがんである。がんにおいて骨転移の治療に用いる骨吸収抑制薬では、有害事象として顎骨壊死の発症が問題となっている。がん診療連携拠点病院内の歯科においては、薬剤関連顎骨壊死(MRONJ)を診る機会が比較的多く、2015年4月から4年間に骨吸収抑制薬使用前や使用中の口腔内管理、または顎骨壊死の症状で医科から診療依頼があった266件中30名にMRONJが認められた。今回、この30名の性別、年齢、原因薬剤を投与した疾患と薬剤の種類、発症部位、ステージ分類、発症要因、薬剤開始からMRONJ発症までの期間、当科での処置について調査した。MRONJと診断した30名は男性14名、女性16名、平均年齢69.6歳であり、原疾患は乳癌が最も多く11名、次いで前立腺癌が8名であった。顎骨壊死はステージ2で下顎に発症していることが多く、発症要因は抜歯が12名、骨隆起や義歯が7名、歯周炎などの感染が7名、不明が4名であった。薬剤投与期間が当院で把握できた19名中、1年未満の比較的早期にMRONJを発症した者が5名いた。MRONJの処置では、保存的治療が16名、外科処置も行った者が11名おり、治癒、または改善した者は合わせて16名であった。なお、調査期間中に原疾患にて10名が死亡している。今後、MRONJの予防対策や、全身状態に応じた治療方法についてさらに検討していく必要がある。(著者抄録)

    頭頸部がんに対する放射線治療では口腔・咽頭粘膜炎,口腔乾燥症,味覚障害などの有害事象が高頻度で発生し,摂食嚥下障害や低栄養,感染症などのリスク因子となる.今回,照射野が異なる上咽頭がん,中咽頭がん,下咽頭がんの3症例について放射線治療による有害事象の出現経過を調査したので報告する.口腔有害事象の評価は,放射線治療開始前から終了まで,診察時に自覚症状の記録により行った.評価項目は口腔内の疼痛,咽頭痛,口腔乾燥感,味覚障害の4項目とし,0〜10の数値評価スケールを用いて患者自身がその程度を評価し,歯科衛生士は,口腔粘膜炎の評価をCTCAEv3.0を用いて実施した.3症例とも放射線量20Gy頃に口腔粘膜炎と疼痛の出現が認められた.鎮痛剤の使用により,口腔粘膜炎の悪化と口腔内の疼痛は必ずしも一致しなかった.照射野が変更になる治療の後半において,疾患別,照射野別に口腔粘膜炎の経過に差が認められた.味覚障害と口腔乾燥感は,治療の比較的早期から同時期に出現していたことから,放射線による味蕾細胞の物理的障害と共に,口腔乾燥から影響を受けている可能性が考えられた.放射線治療による有害事象の出現時期や経過を把握することは早期の対応を可能にすると共に,患者への的確な情報提供にもつながるため,重要であると考えられる.(著者抄録)

    An external dental fistula is a skin manifestation caused by an underlying dental problem. We report a rare case of external dental fistula associated with a fixed cantilever denture. A 77-year-old Japanese woman presented with an enlarging reddish granulomatous lesion on her right cheek that was diagnosed as an external dental fistula. A fixed cantilever denture had initially been attached to her upper jaw with her seven bona fide teeth. However, six teeth were completely lost and the denture was attached to only one tooth, which showed apical periodontitis. Subsequently, the external dental fistula developed. We should keep in mind that a patient with a fixed cantilever denture can suffer from apical periodontitis and a subsequent external dental fistula due to a failure to maintain appropriate oral hygiene.

    Recently, the prevalence of periodontal disease in the elderly has been increasing in Japan. The purpose of this study was 1) to investigate the periodontal condition in elderly residents at nursing homes, and 2) to register the baseline data for a longitudinal study. A clinical examination was carried out on 215 elderly subjects from 54 - 101 years of age, resident in 9 nursing homes in Tokyo and Yamanashi prefectures. The parameters evaluated were: remaining teeth, plaque index (PlI), probing pocket depth (PPD), clinical attachment level (CAL), and bleeding on probing (BOP). The subjects were divided into the four age groups, < 65, 65-74, 75-84, > 85 years. The results revealed that in the 75-84 years age group and > 85 years age group, the remaining teeth were significantly fewer and the mean PlI scores significantly higher than in the other 2 groups. The mean values of PPD, CAL and BOP were 2.6 ± 0.8 mm, 3.0 ± 1.1 mm and 32.0 ± 29.7%, respectively. Of the total, 39.5 % of the subjects had sites with a PPD ≥ 6mm, 76.5% had ≥ 5 mm; 17.8% had an attachment loss of ≥ 9 mm. It is necessary to identify the population at high risk of periodontitis and professional to provide them guidance on oral hygiene, to prevent worsening of periodontal disease. <BR>Nihon Shishubyo Gakkai Kaishi (J Jpn Soc Periodontol) 51(3) : 229-237,2009.

    Purpose The aim of this study wasto clarify the efficacy of the management of oral health care carried out by dental hygienists at a nursing home.<BR>Methods: A total of 142 subjects, living in twofacilities (facility A; 82 subjects, mean age 85.9±7.5 years, and facility B; 60 subjects, mean age 85.6±8.3 years) for dependent elderly persons in Yamanashi Prefecture, participated in this study. The intervention period was 8 months, and the evaluation of oral hygiene conditions by visual examination and oral bacteria test was performed three times: before intervention, after 4 months, and after the end of the intervention. In facility A, dental hygienists performed general oral cleaning for 20 randomly-chosen subjects directly, and gave advice aboutoral health care to the other subjects. In facility B, dental hygienists prepared an interventional oral care plan for the subjects, based on the risk assessment of each subject's oral condition, and conducted oral care intervention in cooperation with the facility staff. In addition, a survey of the attitude of nursing stuff toward the interventional plan was conducted.<BR>Results: In subjects at facility A, there was no significant difference in the number of oral bacteria within the intervention period, while in subjects at facility B, the number of oral bacteria at the last intervention significantly decreased (p<0.05). The results of the survey indicated some differences between the two facilities in the attitude of the facility staff to the interventional plan.<BR>Conclusion: Management of oral health care at a nursing home was proven to be mo t effective when a total oral health care plan was made, considering the facilities, equipment and the dental hygienists' interventional method. We confirmed the importance of the management of oral health care.

    Oral bacteria counts are used as an assessment index for oral health care. In this study, we examine the effect of pressure applied to the swab in order to standardize the collection of swab samples.<BR>Samples were collected from the tongues of 35 healthy adults. The swabs were attached to a device to achieve sample collection pressures of 5, 20 and 40 g. Bacterial counts were made on each swab by a bacteria counter using the dielectrophoretic impedance measurement method, and compared.<BR>The results showed that higher bacterial counts were obtained with higher sample collection pressures (p<0.01); further, bacterial counts were significantly different for swabs taken at sampling pressures of 5 and 20 g and 5 and 40 g (p<0.01). As variations in sampling pressure applied to the tongue are inherent in manual swabbing, it is necessary to standardize sample collection pressure for measuring oral bacterial counts as an assessment index. Considering the stability of swabbing and the patients' comfort, pressure of 20 g is suitable for sampling from the tongue.

    We evaluated the effect of dentifrice containing 3% vitamin C on periodontitis. Institutional ethics committee approval was obtained prior to the study. Sixty patients (26 men and 34 women) with periodontitis were enrolled in this double-blind clinical trial. We prepared three types of dentifrice: one containing 5.0% zeolite, 0.68% sodium monofluorophosphate, 0.05% cetylpyridinium chloride and 0.05% β-glycyrrhetinic acid (D-1); one with 1.0% vitamin C added to D-1 (D-2); and one with 3.0% vitamin C added to D-1 (D-3). Dentifrice was randomly assigned among subjects, each of whom was required to brush twice daily for 4 weeks. Periodontal indices were measured at baseline and at 2- and 4-week evaluations. The results for mean gingival index (GI), probing depth (PD) and bleeding scores in all groups revealed statistically significant differences between at baseline and at the 2-week evaluation (p<0.01) and between at baseline and at the 4-week evaluation (p<0.01). The results for mean GI, PD and bleeding scores with D-2 and D-3 revealed statistically significant differences between D-1 and D-2 and between D-1 and D-3 at the 2- and 4-week evaluations (p<0.01). Of the three dentifrices, D-3 was the most effective. Statistically significant differences in mean GI (p<0.05) and PD (p<0.05) were observed between D-2 and D-3. These results suggest that a dentifrice containing 3% vitamin C is more effective against periodontitis than one containing 1% vitamin C.

    In the present study, the relationships between the counts of putative periodontopathic bacteria in saliva and theoral and general status of the elderly in nursing homes were investigated. The subjects were 37 residents (13 men and 24 women; mean age, 82.1 years) of six nursing homes. Saliva samples were collected from subjects, who providedconsent. Bacterial counts per 10, μl of saliva were measured using the PCR-Invader method for the following species: <I>Aggregatibacter actinomycetemcomitans (A. a), Prevotella intermedia (P. i), Porphyromonas gingivalis (P. g), Tannerella forsythia (T.f) </I>, and <I>Treponema denticola (T.d) </I>. The relationships between the measured bacterial countsand the following subject characteristics were investigated: age, gender, care level, BMI, periodontal parameters, amount of tongue coating, and amount of food residue. The severe periodontal disease group had significantly highercounts of <I>P.g, T.f</I>, and <I>T. d</I> than the mild or moderate group (p<0.05, p<0.05, and p<0.01, respectively), while nosignificant relationships to bacterial counts were observed for care level, BMI, and amount of food residue. Theseresults suggest that the periodontopathic bacteria detected in saliva are primarily related to the severity ofperiodontitis of the elderly and are independent of their general status.

    We evaluated the effect of dentifrice containing sodium ascorbate on periodontitis. Institutional ethics committee approval was obtained prior to the study. Subjects were 53 patients (15 men and 38 wemen, average age : 44.0 years) with periodontits enrolled in a double-blind clinical trial. We prepared 4 types of dentifrice, i.e., that containing zeolite and sodium monofluorophosphate (D-1), that adding 1.0% sodium ascorbate to D-1 (D-2), that adding 0.1% dl-α-tocopheryl acetate, 0.05% cetylpyridimium chloride, and 0.05% β-glycyrrhetinic acid to D-1 (D-3), and that adding 1.0% sodium ascorbate to D-3 (D-4). Dentifrice was randomly assigned to subjects, who were asked to brush twice daily for 4 weeks. Periodontal indices were measured at baseline, 2-, and 4-week evaluation. As a result for mean bleeding scores of D-2 and D-4, the dentifrices containing sodium ascorbate, a statistically significant difference was seen between baseline and 4-week evaluation (p<0.05, p<0.01). Of the 4 dentifrices, D-4 was the most effective. Statistically significant differences in mean gingival index (p<0.01), probing depth (p<0.01), and bleeding score (p<0.01) were seen between baseline and 4-week evaluation. These results suggest that a dentifrice containing sodium ascorbate is effective against periodontitis.

    We studied risk indicators in saliva for periodontal disease in the elderly. Thirty-six subjects over 65 years old (15 men and 21 women) were divided into 4 groups based on clinical evaluation: 7 with healthy gingiva or gingivitis, 11 with moderate periodontitis, 9 with severe periodontitis, and 9 edentulous subjects. Whole saliva samples were analyzed for the presence of <I>Actinobacillus actinomycetemcomitans</I>, <I>Porphyromonas gingivalis</I>, <I>Prevotella intermedia</I>, <I>Bacteroides forsythus</I>, and <I>Treponema denticola</I> using polymerase chain reaction (PCR) method. We studied the relationship of periodontal parameters to the presence of these periodontopathic bacteria, lactate dehydrogenase (LDH), aspartate aminotransferase (AST), and interleukin-1 (IL-1) gene polymorphism. The prevalence of <I>Bacteroides forsythus</I> in subjects with severe periodontitis was significantly higher than in those with healthy gingiva or gingivitis (p<0.05). LDH activity increased proportionally with the advance of periodontitis (p<0.05). AST and IL-1 polymorphism did not correlate significantly with periodontal status. This suggests that the presence of Bacteroides forsythus and LDH activity in whole saliva are possible to be risk indicators for periodontal disease in the elderly.     It is well known that there are many substances related to the diseases at very low concentration in human body fluid. An attempt to search the new marked substances in human body fluid related the prediction of diseases has been carried out by high sensitive and simple detetecting method. In this report, the reactivities of fresh human saliva were examined by the peroxidase activity (POD) and the diminishing of the specific color (at 515 nm) of diphenylpicrylhydrazyl (DPPH)/MeOH solution used as the marker in addition of saliva. It was observed that the POD was depened on the progressing of dental caries, and that the behavior of DPPH was not similar to that of POD. Then, the color diminishing of DPPH was monotonically decresed according with the POD increase. It is expected thet this examinating method will be useful to the prodiction and the prevention of the diseases.
