    BACKGROUND: Zonular apparatus capsular bag (ZACB) insufficiency causes late postoperative spontaneous in-the-bag intraocular lens (IOL) dislocation, a serious complication of cataract surgery. Spontaneous posterior capsule rupture (SPCR) with IOL dislocation is a rare complication. This study reports and discusses clinical features and outcomes of ZACB insufficiency with SPCR. METHODS: We retrospectively reviewed 7 cases of SPCR that developed after cataract surgery at Nippon Medical School Musashi Kosugi Hospital between March 2021 and February 2025. Clinical records and surgical videos were examined to collect data on patient age, sex, associated systemic and ocular conditions, medications, dates of IOL implantation and explantation, signs and symptoms prompting explantation, and surgical outcomes. The stage of capsular rupture and location of intraocular lens dislocation were classified on the basis of intraoperative findings. RESULTS: The anterior capsule rim and zonule were intact in 5 SPCR cases, with the exchanged IOL fixated in the sulcus. Two cases of SPCR exhibited both anterior and posterior capsule dislocation requiring intrascleral IOL fixation. The average age of the patients was 52 years (range: 29-83 years; 6 men, 1 woman), and 5 (4 men, 1 woman) had atopic dermatitis. Time since the initial cataract surgery ranged from 10 to 20 years (mean, 13.8 years). CONCLUSIONS: Most SPCR cases were associated with atopic dermatitis, with minor trauma possibly contributing to ZACB insufficiency. Capsule morphology was preserved in most SPCR cases, with the exception of posterior capsule rupture.

    Background/Objectives Conventional eye drops are the primary therapeutic option for ocular diseases; however, their clinical utility is hindered by several drawbacks, including limited bioavailability and suboptimal patient compliance. To overcome these challenges, we designed a sustained-release contact lens (CL) device loaded with tranilast (TRA) and determined whether the TRA-laden CL could provide sustained drug delivery to the lacrimal fluid and aqueous humor. Methods TRA nanocrystals were prepared using the bead-milling approach. Using three types of CLs (nonionic, anionic, and cationic), we prepared TRA-laden CLs by employing a combination of solid TRA nanocrystals and soaking methods under high-temperature and high-pressure conditions in an autoclave (the hThP method). Male Japanese albino rabbits (2-3 kg) were used to evaluate the CLs. Results Bead milling reduced the size of the solid TRA nanoparticles (STNs) to approximately 35-180 nm. The TRA-laden cationic CLs prepared using STNs and the hThP method contained a higher amount of TRA than those prepared using the corresponding conventional soaking method. The CLs prepared using the hThP method remained transparent after drug loading. Compared with nonionic and anionic CLs, cationic CLs had the highest drug-loading capacity and allowed for sustained drug release. Moreover, STNs were observed in the released TRA, with no corneal damage or light scattering detected in the rabbits' eyes. TRA-laden cationic CLs prepared using the hThP method achieved sustained and higher drug delivery into the lacrimal fluid and aqueous humor than those prepared using the conventional soaking method. Conclusions Our findings suggest that TRA-laden cationic CLs prepared using STNs and the hThP method can overcome the challenges associated with the conventional soaking method, including low drug uptake and high burst release.

    PURPOSE: Comparison of dynamic pupillary parameters (DPPs) between healthy volunteers and multi- or monofocal intraocular lens (IOL) implanted eyes, and examination of the correlation between postoperative visual acuity (VA), contrast sensitivity (CS), visual dysphotopsias (VD), and these parameters in IOL-implanted eyes. METHODS: This cross-sectional study classified and analyzed DPPs from healthy volunteers (308 eyes) according to age. In the multifocal IOL group (119 eyes), 51 eyes were implanted with Lentis Comfort® (L group), 32 with PanOptix® (P group), and 36 with Synergy® (S group). The monofocal IOL group included 33 eyes implanted with Vivinex Impress® (V group). DPPs by the pupillometer (PLR-3000), distance, intermediate and near VA, and CS and VD were examined one month postoperatively in 152 eyes. RESULTS: Significant decreases in maximum or minimum pupillary diameter (INIT, END), percentage of pupillary constriction (DELTA), latency between light stimulation and constriction (LAT), and average and maximum constriction velocity (ACV, MCV), average dilation velocity (ADV), were observed with aging in the healthy eyes group. The preoperative group showed a significant age-related decrease in DELTA and increase in LAT. The postoperative group showed a significant age-related decrease in INIT, END, and ADV. The L group exhibited a significant positive correlation between ADV and uncorrected intermediate VA (UIVA), while the P group exhibited a significant positive correlation between ACV and UIVA, and the S group exhibited a significant negative correlation between ACV and uncorrected near VA. For CS, the S group exhibited a significant positive correlation with ADV at 1.0 and 0.7 degrees, and the P group exhibited a significant negative correlation between ACV and glare. CONCLUSION: DPPs are greatly affected by age. DPPs for multi- or monofocal IOL eyes correlate with uncorrected VA, CS, and VD.

    Objectives: This study evaluated the design of a sustained-release contact lens (CL) device loading tranilast (TRA) and determined the usefulness of these CLs in Japanese albino rabbits. Methods: The sustainable CLs in this study were prepared by combining three CLs with different water contents and soaking methods under high-pressure and high-temperature using an autoclave method (AC-method). Results: Both the CLs prepared with the conventional soaking method (stir-method) and AC-methods were transparent in all three types of CLs. The loaded TRA contents in the CLs when using the AC-method were higher than those prepared using the stir-method for all three types of CLs. TRA contents were also higher when loaded into the cation-type lenses as compared to the other lenses. Moreover, the sustainable release of TRA from the TRA-loaded cation-type CL using the AC-method was significantly higher than those found for the other CLs. No corneal wounds were observed in any of the rabbits given the three types of TRA-loaded CLs for 7 days. Furthermore, the TRA-loaded CL sustainably released TRA into the lacrimal fluid in the rabbit. Conclusions: The TRA-loaded CL prepared using the AC-method overcame the limitations normally associated with the stir-method, such as the high burst release and low drug uptake.

    INTRODUCTION: Intrapapillary hemorrhage with adjacent peripapillary subretinal hemorrhage (IHAPSH) is a syndrome associated with intrapapillary hemorrhage and peripapillary subretinal hemorrhage that commonly occurs in myopic women. There have been no reports of patients younger than 10 years. We report a case of typical IHAPSH that occurred in a 9-year-old girl in the advanced stages of myopia. CASE PRESENTATION: A 9-year-old girl presented with gradually decreasing uncorrected visual acuity in the left eye over the past year, without complaints of floaters or blurred vision. Uncorrected visual acuity was 20/16 in the right eye and approximately 20/285 in the left eye, improving to 20/16 with -4.50 diopters sphere and -0.50 diopters cylinder at 165°. Ophthalmoscopic examination revealed intrapapillary hemorrhage and nasal peripapillary subretinal hemorrhage in her left eye. The left eye had a small, tilted papilla. Fluorescein angiography showed no leakage from the papilla. The length of the ocular axis was elongated to 24.87 mm in the left, compared to 23.15 mm in the right. We diagnosed IHAPSH and decided to follow the patient. After 8 weeks, the hemorrhage was almost completely absorbed. No recurrence has been observed since then. CONCLUSION: To our knowledge, this is the youngest reported case of IHAPSH. This case underscores the potential contribution of optic disc morphology and progressive myopia to IHAPSH pathogenesis and highlights the importance of considering this condition in the differential diagnosis of optic disc hemorrhage in school-aged children.

    Zinn's zonule is a fragile and thin tissue, and little is known about its pathogenesis. The aim of this study was to develop an experimental setup for a comprehensive analysis of Zinn's zonule. Rats were divided into two groups: a control group (n = 4) and an alkali injury group (n = 4). Seven days after injury, the eyes were enucleated, the anterior eye was dissected and embedded in gelatin, and macroscopic observations were made. The gelatin specimens were then embedded in paraffin and observed in detail by low-vacuum scanning electron microscopy, immunofluorescence, and quantitative reverse transcription polymerase chain reaction (RT-qPCR). The results show qualitative changes in Zinn's zonules in both macroscopic and microscopic observations. In addition, macrophage infiltration and increased matrix metalloproteinase 2 (MMP2) expression were observed in the injured group, consistent with the RT-qPCR results. The experimental system in this study allowed us to capture the morphological and molecular biological changes of Zinn's zonule and to gain insight into its pathogenesis. In conclusion, this study presents a new experimental setup for the comprehensive analysis of the rat Zinn's zonule. The results suggest that this system can be used in the future to study and analyze a variety of paraffin-embedded tissues and specimens.

    背景:外傷性眼球破裂は眼科緊急疾患のなかでもっとも予後の悪い疾患の一つであり、速やかな治療介入を要する。眼球破裂では、創傷を受けやすい眼球部位があることは知られているが、縫合治療後の創傷部の強度に関してはよくわかっていない。今回、同側に二度の外傷性眼球破裂を発症したが、二度目の創傷部が初回とは異なる部位であった症例を経験したため報告する。症例:94歳、女性。2020年8月25日、転倒による右眼球破裂にて日本医科大学附属病院眼科を紹介受診。2018年にも転倒時に右眼球破裂を起こし、他院にて強膜縫合術を受けた既往があった。同日、角膜・強膜縫合術を施行した。所見:鼻側角膜輪部強膜に裂傷、一部ぶどう膜脱出を認めたが、前回破裂部位の耳側角膜強膜には10-0ナイロン糸で縫合が密に施されており、同部位の眼球形態は保たれていた。今回の破裂部位に対して角膜・強膜縫合術を施行し、術後経過は良好であった。結論:外傷性眼球破裂の既往がある眼球に、再度外傷による破裂を起こした症例を経験した。前回、破裂を生じた強膜部位は二度目の破裂時には保たれており、縫合により強度が高められていると思われた。創傷部を適切に縫合することで外傷強度が向上することが示唆された。(著者抄録)

    目的:日本医科大学武蔵小杉病院におけるPanOptix挿入症例の術後成績について検討する。対象と方法:2019年10月から2020年4月に日本医科大学武蔵小杉病院眼科で行われた白内障手術症例のうち、PanOptixを挿入された23名39眼を対象に、術後1ヵ月の視力、焦点深度曲線、コントラスト感度を測定し、満足度調査を行った。結果:裸眼視力の平均値は、遠方(5m)0.90、中間(50cm)0.84、近方(30cm)0.53であった。焦点深度曲線は遠方と50cmにピークを持つ2峰性の曲線を示し、中間部の落ち込みは少なかった。コントラスト感度は全周波領域において40歳以下の正常範囲に収まっていた。眼鏡使用率は限定使用も含め35%であった。結論:PanOptixは近方30cm視力において、遠方視力や中間視力に比べやや劣るものの、患者の生活様式に適合する場合は有用であると思われた。(著者抄録)

    背景:虹彩異常に対し視機能改善を目的として人工虹彩を使用した報告が散見されるが、なかでも人工虹彩と多焦点眼内レンズ(IOL)の併用を試みた報告は本稿が初めてである。症例:35歳、男性。ボールが右眼に当たり受傷。右眼の虹彩断裂、外傷性散瞳、前房出血、硝子体出血、高眼圧症にて保存的加療が行われた。受傷後3ヵ月、白けた見え方が持続していたため、当院を紹介受診した。初診時、右眼の虹彩離断、外傷性散瞳、後嚢下白内障を認め、視力は1.0p(1.5)であった。右眼水晶体再建術および人工虹彩挿入術を施行、IOLはTFNT00(アルコン)を用い、人工虹彩IPS SE1(Ophtec社)を1枚、5時方向の虹彩離断を覆うように挿入した。手術1週間後、視力は0.3(1.2)、遠方、中間、近方視の違和感(waxy vision疑い)を訴えたため、右眼IOL交換術および同型の人工虹彩の追加挿入した。IOLはPCB00V(ジョンソン・エンド・ジョンソン)を用い、虹彩離断部と対側に追加の人工虹彩を配置した。手術後4ヵ月現在、視力は1.2p(n.c.)、自覚症状はやや改善した。結論:虹彩欠損や瞳孔異常を伴う白内障手術では、術後に患者が期待する程度の複機能を獲得できない可能性を考慮すべきであり、とくに多焦点IOLの使用を検討する場合には慎重に対処する必要がある。(著者抄録)

    Purpose: To evaluate visual outcome and patient satisfaction following Lentis Comfort intraocular lens (IOL) implantation. Method: This retrospective case series examined 68 eyes of 41 patients (mean age 72.0 ± 8.1 years) who underwent Lentis Comfort (LS-313 MF15, Oculentis GmbH, Berlin, Germany
    Santen, Osaka, Japan) im-plantations. Patients were evaluated for visual acuity (VA) at several distances (0.3, 0.5 and 5 meters), refractive error, defocus curve and contrast sensitivity, in addition to answering a questionnaire on photic phenomena, visual discomfort and patient satisfaction. Results: Uncorrected visual acuity was 0.05 ± 0.13 (logMAR) for distance, 0.23 ± 0.17 (logMAR) for in-termediate, and 0.52 ± 0.20 (logMAR) for near. Defocus curve showed the binocular visual acuity at-tained was almost 20/20 within the range of +0.5 D to −1.5 D. Contrast sensitivity was within the normal range. The Lentis Comfort IOL tolerated astigmatism to some extent. Patient age could potentially be related to uncorrected visual acuity. Questionnaire results showed almost all patients were satisfied with Lentis Comfort IOL implantation. Conclusion: Lentis Comfort IOLs provided better visual function at far and intermediate distances.

    PURPOSE: The hexokinase 1 (HK1) gene encodes one of the four human hexokinases that play essential roles in glucose metabolism. Recently, several cases of E847K mutation in the HK1 gene were reported to cause inherited retinal dystrophy. The purpose of this study was to identify the phenotypical characteristics of patients with a recurrent E847K mutation in the HK1 gene. METHODS: Three generations of one family with autosomal dominant retinitis pigmentosa were examined. Whole exome sequencing was performed on the DNA. Fundus imaging by an adaptive optics fundus camera was used to obtain high-resolution photoreceptor images. RESULTS: Fundus examination of the proband showed degeneration of the mid-peripheral retina, and SD-OCT images showed an absence of the ellipsoid zone (EZ) and interdigitation zone (IZ) in the parafovea and more peripherally. SD-OCT images of the mother of the proband showed an absence of the EZ and IZ, and fundus autofluorescence images showed hypo-autofluorescence surrounding the macular region. One daughter of the proband had only mild night blindness, however, the density of the cone photoreceptors was reduced in the parafoveal region. Whole exome sequencing identified a heterozygous variant, E847K, in the HK1 gene. This variant was found to co-segregate with the disease in three family members. CONCLUSIONS: Although the systemic phenotypes were found to be associated with the HK1 mutations, only the E847K mutation can cause a non-syndromic photoreceptor degeneration. Our study strengthened the hypothesis that the amino acid E847 might play a critical role in the maintenance of the morphology and function of the photoreceptors.

    目的:日本医科大学武蔵小杉病院におけるSymfonyとFINE VISION挿入症例の術後成績の比較検討。方法:2014年6月〜2019年7月に、日本医科大学武蔵小杉病院にて白内障手術を施行され、Symfonyを使用した症例45名71眼と、FINE VISIONを使用した症例51名83眼の術後1ヵ月後における裸眼および矯正での遠方(5m)・中間(60cm)・近方(33cm)の視力、コントラスト感度、焦点深度曲線を測定し、満足度調査を行った。結果:Symfonyの裸眼視力平均値は近方0.31(0.49)、中間-0.14(1.36)、遠方0.03(0.94)で、FINE VISIONは近方0.16(0.70)、中間0.12(0.75)、遠方-0.02(1.05)であった(logMAR(小数視力))。近方はFINE VISIONが、中間はSymfonyが良好であった(p<0.05)。コントラスト感度は、Symfonyは全視覚において40歳以下の基準内に収まっていたが、FINE VISIONは高周波領域において低下傾向を示した。結語:近方はFINE VISIONが視力良好であり、遠方から中間はSymfony、FINE VISIONともに良好であった。(著者抄録)

    目的:プリロードIOL(p-IOL)挿入時の筋疲労を数値化する試みとして表面筋電位の測定、並びに機械的特性を評価するため放出抵抗を測定した。これらよりプッシュ式p-IOLの客観的評価を試みた。方法:4種のp-IOLを用いた。IOL挿入時の術者の前腕4ヶ所の表面筋電位を測定し、筋活動量(RMS)と、最大随意筋収縮時の電位相対値(%MVC)を求めた。また放出抵抗を経時的に測定してピーク荷重、平均荷重と荷重積分値を求めた。併せてプランジャーの押し出し距離とプランジャーヘッドの大きさを測定した。結果:RMSは短母指屈筋が最大(有意差なし)、手根屈筋が最小(p<0.05)であった。短母指屈筋においてRMSはYP2.2が最大、SZ-1が最小(有意差なし)、%MVCはPN6ASが最大、YP2.2が最小であった(有意差なし)。放出抵抗は有意差がみられた(p<0.05)。結論:RMSや%MVCと放出抵抗に相関はなかった。p-IOLの操作ではインジェクターの構造などが複合的に関与し、術者の身体に影響している。(著者抄録)

    A rare case of palpebral cellulitis with simultaneous frontal sinusitis and osteomyelitis is reported. A healthy 45-year-old man presented with left upper eyelid swelling. He was given intravenous meropenem at the local hospital, but he failed to improve. Magnetic resonance imaging showed left frontal and maxillary sinusitis and upper palpebral cellulitis with an abscess. His temperature was 37.6°C, C-reactive protein was 1.36 mg/dL, thyroid hormone was elevated, left best-corrected visual activity was 1.2, and intraocular pressure was 25 mm Hg. He was then given cefazolin intravenously for 3 days but with no improvement. Therefore, the eyelid skin was incised. Postoperatively, the swelling improved significantly. Computed tomography demonstrated osteomyelitis of the left frontal sinus and osteolysis of the inferior wall. This case was considered a variation of Pott's puffy tumor. Bacterial cultures from the cellulitis abscess and sinusitis were negative. As for sinusitis, endoscopic sinusitis surgery (frontal sinus single sinus surgery [Draf III] and Kilian surgery) was performed. During 10 months of follow-up after the skin incision, no signs of recurrent eyelid swelling were observed.

    Background: To evaluate and compare posterior capsule opacification (PCO) in silicone intraocular lenses (IOLs) with different edge and haptic design in rabbits. Materials and Methods: Japanese albino rabbits (n = 51) were used. Four types of IOLs (CanonSTAAR Inc.) were prepared: AQ2013V (a three-piece IOL with a rounded edge), AQ310NV (a three-piece with a sharp edge), AA4203F (a one-piece plate-haptic with a rounded edge) and a modified CanonSTAAR IOL (a one-piece plate-haptic with a sharp edge). After phacoemulsification, the IOLs were implanted. Three weeks postoperatively, the enucleated eyes were observed using the Miyake-Apple view. The PCO percentage area was calculated and histopathological examination was performed. Results : The PCO area for Group AQ2013V was significantly larger than that of Group AQ310NV (p&lt
    0.05) and the modified IOL (p&lt
    0.05). The PCO area for Group AA4203F was significantly larger than that of Group AQ310NV (p&lt
    0.01) and the modified IOL (p&lt
    0.01). In the Miyake-Apple view, there was no formation of Soemmering's rings in the plate-haptic section in any specimen of Group AQ4203F or the modified IOL. Regarding histopathological findings, in Groups AQ2013V and AA4203F, migration of lens epithelial cells (LEC) and lens fiber cells onto posterior capsule was noted in most eyes (95%). In Group AQ310NV and the modified IOL, migration was inhibited in many eyes (67%). Conclusion : The effectiveness of silicone intraocular lenses with sharp edges preventing PCO was confirmed. Our findings suggest that plate-haptic IOLs can effectively suppress PCO by sharpening the optical edge.

    目的:灌流吸引(I/A)チップからの灌流を用いる切開創ハイドレーションHydration Using Irrigation Port(HYUIP)法における角膜浮腫を定性的に評価すること。方法:フローレスブライトで染色した灌流液を用いて豚眼の前房を灌流した後、I/Aチップの灌流口を5秒ずつ切開創の左右と中央に押し当てるHYUIP法を行った群(H群)、27G鈍針でハイドレーションを行った群(N群)、行わなかった群(C群)の3群で、創口におけるフローレス粒子の拡散状態を細隙灯顕微鏡で評価した。結果:N群では局所的に強く角膜実質に浮腫が生じているのに対し、H群ではびまん性に弱く角膜浮腫が起きていることが観察できた。結論:通常の鈍針によるハイドレーションは過度の侵襲を起こしている可能性がある。HYUIP法では角膜実質に適度な浮腫が起きており、ハイドレーション方法として有用であることが示唆された。(著者抄録)

    背景:近年、網膜復位術に際しシャンデリア併用広角観察システムを用いた手術が広まってきた。裂孔とバックルの位置確認や、手術助手への術野共有において有効性が高い。今回、同方法による術後、細菌性眼内炎を発症した症例を経験したので報告する。症例:52歳、男性。2016年6月11日、左眼網膜剥離にて近医より紹介され、日本医科大学付属病院を初診。矯正視力(0.3)、左眼黄斑部を含む上耳側の裂孔原性網膜剥離と診断、同14日、シャンデリア併用網膜復位術を施行した。復位良好にて退院予定の17日に前房蓄膿、硝子体混濁が出現、感染性眼内炎と診断し、緊急で水晶体再建術、硝子体茎顕微鏡下離断術、硝子体腔への抗菌薬灌流を施行した。所見:バックル留置部網膜は復位していたが、シャンデリアポート部の硝子体混濁、網膜ダメージが強く、同部位からの感染が疑われた。初回術中にシャンデリアポートが脱落し、眼瞼に接着したことが原因と思われた。硝子体液よりcoagulase-negative Staphylococcus epidermidis(CNS)が検出された。術後、炎症は軽快し、眼内レンズ挿入術施行、8月29日現在、矯正視力(0.5)である。結論:シャンデリア併用網膜復位術を施行する際は、術中ポート作製部の管理が重要であることが示唆された。(著者抄録)

    背景:片側性急性特発性黄斑症(unilateral acute idiopathic maculopathy:UAIM)は,感冒様の症状に続き,健常な成人の片眼に急激な視力低下が生じ,黄斑部に黄白色の滲出斑が現れる疾患である。目的:UAIMと推定される1症例の報告。症例:38歳男性が左眼の急激な視力低下で紹介受診した。感冒様の症状はなかった。視力は右1.5,左0.15で,左眼の中心窩下に1.5乳頭径大の黄白色の病巣があった。光干渉断層計で網膜色素上皮の不整とIS/OS線の消失があった。フルオレセイン蛍光眼底造影で,早期では不均一な過蛍光,後期には組織染があった。UAIMと診断した。視力は1ヵ月後に0.5,2ヵ月後に0.9,4ヵ月後に1.2に自然寛解した。結論:急激な黄斑部の黄白色の滲出斑を呈する病変では,感冒様症状が先行しなくても,UAIMである可能性がある。(著者抄録)

S. aureus was recovered from one water-jet nozzle and 9 toilet seats of which MRSA was found in a water-jet nozzle and a toilet seat of different toilet. P. aeruginosa was isolated from 6 water-jet nozzles, no isolate was shown to be multi-resistant. PFGE pattern analysis showed no similarity among the P. aeruginosa isolates. Both a nozzle and a seat of same toilet were contaminated with E. coli which were CTX-M-9 type ESBL producers and presented the identical PFGE pattern.
    Our results suggest that the frequency of fecal contamination of water-jet nozzles in a hospital is high. Furthermore, the findings are consistent with transient contamination of electronic bidet toilets including MRSA and ESBL-producing organisms with identical PFGE patterns.
