    今年度は、院内通訳者、または、患者が同伴した通訳者が介在した診療場面の音声データの分析を行った。院内通訳者が介在した診療場面は48件であった。診療科別では産婦人科16件、小児科15件、整形外科17件であった。診療時間は、平均値14分49秒、標準偏差10分55秒、中央値11分40秒、最小値3分11秒、最大値50分37秒であった。患者が同伴した通訳者が介在した診療場面は2019年のデータとあわせて13件であった。診療科別では産婦人科6件、小児科2件、整形外科4件、耳鼻咽喉1件であった。診療時間は、平均値9分29秒、標準偏差3分36秒、中央値9分37秒、最小値4分22秒、最大値18分01秒であった。
    音声データは、ポルトガル語と日本語ができる研究協力者に逐語録作成とポルトガル語発話の日本語翻訳を依頼した。さらに、別のバイリンガルの研究協力者にその日本語翻訳をポルトガル語にバックトランスレーションを依頼し、翻訳内容を確認した。次に、通訳者による通訳変更をコーディングするために、通訳変更の分類と定義を検討し、研究代表者1名と研究分担者1名が予備的に3診療について別々にコーディングして分類別一致の割合を検討した。その結果、分類によってコーダー間の不一致が生じたため、客観性をあげるためにコーダーを1名追加し、分類と定義を再検討して、コーダー間評価の信頼性を検討した。また、分析過程で生じたポルトガル語の発話に対する疑問について、内容を確認しながら分析作業を進めている。

    This study aimed to investigate the usefulness of trained medical interpreters from the perspective of risk communication. We conducted a survey using questionnaires and an IC recorder for Brazilian patients, hospital interpreters or interpreters who accompanied patients (ad-hoc interpreters), and doctors at a hospital where is a regional base accepting foreign patients. In the case of medical encounters with hospital interpreters, patients were satisfied and understood physicians’ explanations well. Moreover, frequency of negative interpretation alterations (interpretation errors) and, among them, those which had potential clinical consequences by the hospital interpreters were much fewer than in previous studies. Our study also suggested that negative interpretation alterations were much fewer when the interpreters divided consecutive interpretation into shorter sentences.

    The purpose of this research is to develop quality indicators and for continuity and coordination of care in elders and to establish the system embodied the quality corresponding to the super-aged society. We completed quality indicators for continuity and coordination in line with the current situation of elderly care in Japan, and identified nursing actions in hospital settings to improve collaboration between healthcare professionals and the continuity of care for elderly. In addition, we evaluated the patient-centered developed system for regional collaboration and information sharing application from the viewpoint of the elderly person and clarified its application possibilities and issues.

    In Japan, approximately 75% of deaths occur in hospitals. However, in recent years, the number of elderly individuals who die in nursing homes has been gradually increasing. Although rating scales have been developed for the bereaved families of cancer patients to provide high-quality end-of-life nursing care, no such scales have been developed for the families of elderly individuals who die in nursing homes. To implement desirable end-of-life nursing care at nursing homes, we developed a rating scale for bereaved families and investigated the reliability and validity of the scale.We developed a questionnaire containing 43 items, and conducted a mail survey of 427 bereaved families. Item analysis, exploratory factor analysis, criterion-related validity analysis, and the estimation of reliability coefficient were performed to assess the reliability and validity of the scale.

    The purpose of this study is to assess the usefulness of our developing educational program for medical interpreters so that foreign residents are able to be involved as medical interpreter human resources in their communities. We conducted our program with 28 Brazilians residing in Shizuoka prefecture,Japan,between August and December,2013. The program totaled 13 days,i.e.,32.5 hours of lectures and role-playing, and 23 hours of clinical practice in medical settings.
    The following shows the effectiveness of the program. 1) For process evaluation, the percentage of program participation was 94.2 %; all participants found the program useful. 2) For outcome evaluation, written test results showed that the program can be useful in improving basic knowledge in medical interpreting regardless of participants' Japanese proficiency. We are analyzing other outcome indicators, and we are investigating the usefulness and points for improvement of our educational program for medical interpreters.

    The purpose of this study was 1) to clarify what sort of nursing care fulfills the care expected from a family when one of their members passes away at a nursing home; and 2) to determine whether dispatching home-visiting nursing services to nursing homes can play an effective role in realizing satisfactory nursing care when the end is near.
    Semi-structured interviews were conducted with the bereaved families of 18 in- dividuals who had passed away at three nursing homes, and the opinions of the staff working at these nursing homes were also surveyed.
    From this study, we learned that the care that the family of a deceased individual felt to be necessary was categorized into two types. One was the care given to the individual at the nursing home that allowed them to die in peace with no pain or suffering. The other was the care given to the bereaved family that gave the family the opportunity to unde r- stand that death was nearing and to know what sort of changes in condition the deceased was undergoing. As there are very limited requirements for medical support, those who work at nursing homes commented that dispatching home-visiting nursing services to nursing homes was not necessary.

    In this study, we conducted a questionnaire survey and two types of focus group discussions with medical service providers in order to determine how to provide safe and secure medical services to foreign patients in Japan. We investigated as follows ; 1) Status and associated factors of communication gaps between foreign patients and medical service providers(medical doctors and pharmacists), 2) Nurses' cultural competence in nursing care of patients with communication disabilities, 3) The critical issues and improvement strategies for providing safe medical services to foreign patients. It was suggested that Japanese medical service providers were weak in communication with non Japanese speaking foreign patients ; thus the quality of communication with foreign patients was poorer than that with Japanese patients. In order to resolve that problem, it was suggested to use medical interpreters who have been properly trained, and to develop cultural competencies.
